Nursing’s path
Some say that we are better at the individually focused aspects of this policy than at social justice, that our past shows a commitment to social justice not currently found in our profession (Bekemeier and Butterfield, 2005).I disagree with Bekemeier and Butterfield, the new version of the Code sets out a role for nursing in terms of social justice and this is the right path for our profession to follow. Historical public health nursing leaders such as Lillian Wald, Florence Nightingale, and Lavinia Dock provide great examples of nurses who look to improving social conditions that have deleterious effects on health in the vulnerable population. The ANA points to ways in which nurses can promote health by educating the public and joining legislative forces to promote health. For example as a bedside nurse, I see daily how a social problem becomes a health problem such as homelessness, lack of cultural sensitivity, and abuse.

For example, my role as a nurse in altering the patients environment to promote healing by making sure they have food, water, clean room, and minimal noise is my goal and I am happy to ensure patients needs are met as I provide excellent care. As Jones points out, we need to “value all people equally and providing resources according to need” As a nurse, setting aside a few hours a week to volunteer to help the vulnerable population will have a positive impact in the community. For example, when I provide holistic care to my patients on the daily basis, I don’t look at the gender, culture, race, or religion but focuses on the patient as a whole as I promote health, prevent illness, and restore health and their
8.2 Responsibilities to the public-Nurses, individually and collectively have a responsibility to be knowledgeable about the health status of the community and existing threats to health and safety. Through support of and participation in community organizations and groups, the nurse assists in efforts to educate the public, facilitates informed choice, identifies conditions and circumstances that contribute to illness, injury and meet national health objectives. In addition, the nurse supports initiatives to address barriers to health, such as poverty, homelessness, unsafe living conditions, abuse and violence, and lack of access to health services.
A career trajectory is a formalized plan for a successful transition into a career path that an individual has chosen. Likewise, the goal of nursing is to improve health through the advancement of knowledge therefore, improving the safety, effectiveness, and the access for healthcare. In combining these two definitions, a career trajectory in nursing advancement is to allow for clarity of direction and expected outcomes in a career by meeting the goals to advance in the healthcare field. In order to build a successful career as an advanced practitioner an individual must consider the requirements of the position and make strategic decisions in order to facilitate progression towards this goal (Feetham & Doering, 2015).
P.R. is a 34 year-old male from Guatemala who went to a lake for cliff diving. He dove off of a cliff 20 feet from the water, hitting a rock, and fractured his neck at C6. This left P.R. as an incomplete quadriplegic, with partial gross movement of his upper arms. P.R. is able to move his shoulders to slightly lift his arms, but has no movements in his legs or the trunk. P.R. requires total assistance for all activities of daily living, and is incontinent of both bowel and bladder function. He speaks primarily Spanish and cannot communicate in English. Nurses have been successful advocates for improvement of the individuals, communities and indeed Nations. However much more work needs to be done to reduce health disparities, improvement of quality and safety in the health system. As well as improve access to care and formulate policies in organization that focus on the need of patients. In my opinion, nurses sometimes believe in a common mistake that nurses lack the power to be effective in the legislative arena. However, I just want to emphasize that nurses are the largest group of health care providers, and we can generate enough power to successfully reform the health care system based on numbers
As the society acknowledges the importance of human rights and dignity, the issues among the poverty, homeless, violence victims are now more considered. Although there are difference in roles between nurses from acute care, intensive care, and emergency settings nurses, community nurses engage those issues to become advocates for social justice. Health care nurses collaborate to identify strategies for incorporating social justice aims into their work environment (Johnstone, 2011). As a public nurses, there has been many researches to address vulnerability, diversity, health literacy, health care access, health care disparities and issues of particular populations such as ethnic and racial minority health, gay and lesbian health and the health of children, women and older adults (Johnstone, 2011). For example, affordable health care 2010 was implied for equitable delivery of healthcare for all individuals in United States. Although there are pro and cons on success of the system, it was aimed to promote the justice of human need to have least of health care when necessary. Among with the issues of inequality and unfairness of worldwide insurance system, this act was to provide fair access to health care. To achieve the aims of a just, accessible, affordable health care system for all, public health nurses make an effort to engage the professional advocacy.
Bekemeier and Butterfield’s article “Unreconciled Inconsistencies: A Critical Review of the Concept of Social Justice in 3 National Nursing Documents,’ highlighted multiple literary discrepancies on the subject of social justice in nursing. These fundamental nursing documents are: Nursing's Social Policy Statement, Nursing: Scope and Standards of Practice, and the American Nurses Association Code of Ethics for Nurses with Interpretive Statements. I find myself agreeing with the viewpoints of Bekemeier and Butterfield. They bring to light many entries where nursing care is focused more in response to an injury or disease process, rather than providing nursing on a larger scale with a preventative connotation. The American Nurse Act made it clear what the role of the nurse should be---to consider him/herself in carrying out their professional duties as their practice is embedded in the ethic and human right. Nurses who appreciate the history of advocacy that our founding nursing leaders encouraged and practiced are better prepared to take on the challenges of our present changing paradigm of healthcare .The rich history of nursing founded by “The Mother of Nursing “Florence Nightingale was credited for helping to develop nursing education programs.( Nickitas, Middaugh & Aires, 2016,p.4 ) She was one of the first nurses to advocate and utilize the political system to accomplish her goals of improving the nursing education system . In addition , she started what is now known as the Modern Nursing Movement .(Nickitas, Middaugh & Aires 2016) Other nursing leaders have advocated change in the health field and suffered the ill effects such as Margaret Sanger who went to jail and was convicted for her support and beliefs in the planned parenthood program. Nursing's roots are firmly planted in service to others - individuals, groups, and communities. For centuries, nurses have worked to help people and have served the health needs of society. I believe that we are socialized through education, experience in work settings, and through our individual roles to acquire the values, skills, behaviors and norms appropriate to service society.. Nursing is a valued profession that serves to protect, promote, and optimized the health and wellbeing of individuals, families, communities, and populations through care, regardless of age, race, sex, diagnosis, financial stability, and abilities or disabilities. As time progressed, nursing has been regarded as a human science, in which it not only requires one to be knowledgeable and skilful in the field of health care, but they must also possess a strong desire and passion to care and help patients who are in their most vulnerable states. The primary goal of nursing is empowerment of patient through advocacy, health promotion and education, along with direct care.
The theoretical framework constructed from this study can be used to guide nursing education, research, and practice of social justice, thereby strengthening the profession’s ability to identify and redress the societal conditions that negatively impact individual and public health.
76 yr old Spanish speaking patient admitted to telemetry unit with complaints of dizziness. Patient was connected to the cardiac monitor. Vital signs checked. Body weight was checked. 2liter oxygen was given via nasal cannula.Cardiac monitor showed sinus brady cadia, with 2 sec pauses.Patient was asymptomatic at that time.Vital signs were stable.Primary Physician recommended the Cardiologist to see the patient.Defibrillator with pacer leads were kept next to the patient.The writing RN , kept the atropine injection and normal saline flush ready .Patients heparin lock site was patent.Pacer leads were not attached to the patient , because according to the hospital policy,Doctor’s order is required to attach the pacer leads to the patient. Cardiologist came and saw the patient. Cardiologist said that this patient needed pacemaker 2 years ago. He sent the patient to the hospital 2 years ago recommending pacemaker for the patient. Patient and her 4 daughters decided not to have any invasive procedure on the patient. They did not make the patient DNR. Cardiologist was supprised to
