My favorite assessment tool lately is the “My Mood Monitor (M-3)”. I have a 40-year-old Caucasian female diagnosed with depression, anxiety and post-traumatic stress disorder (PTSD). She reported she recently reviewed notes from her previous therapist and discovered she has these three diagnoses. The goal of the patient gaining more confidence in health care professionals transferring him by 02/10/16, was partically met, however, he still gets anxiety and will occasionally move too quickly, therefore, he is still at risk for the patient falling. The patient met the goal of notifying the nurse or the nursing assistant when he needed to use the restroom. He would properly notify us as the healthcare providers by pushing his call light.

Thinking back over the years of being a nurse, I cannot say that one particular case has defined me as a nurse. However, I will say that that there was a 12-month period in which I worked on an Alzheimer’s secure unit a long-term care facility, that defined me a as nurse. I believe I had only been working as a nurse for about a year at this long-term facility, however, on another unit. A nurse quit that worked on the Alzheimer’s unit, so I applied for the position and go it, so I started working the unit the next following week. Following a thorough physical therapy examination, several primary and secondary impairments were found post-ACLR surgery. The primary impairments included: decreased hip and knee strength in bilateral lower extremities, left>right, bilateral tightness in the hamstrings, and limited active left knee range of motion. The secondary impairments included swelling in the left knee and slow cadence when ambulating due to the 3-point gait pattern with crutches. These impairments hinder the patient’s ability to perform transfers and prolonged ambulation, which prevented her from being able to participate in playing soccer, going to the mall with friends and running activities. I think the first thing to do after bringing the patient to a room that has been previously cleaned, is to sit down face to face with the patient. Clinic 275 is a confidential and complimentary walk-in sexual health service which provides medical consultation/advice, testing and treatment for sexually transmitted infections (STIs) (SA Health 2016a). Wellness remains vital for living a successful life in older adulthood, therefore conducting a wellness inventory aids in gaging areas that need improvement. Anne is a fairly healthy sixty-seven year-old married woman with three adult children. All of her children live out of the house in different towns. Anne lives in the Fortunes Rock community of Biddeford, Maine. Consequently, there are numerous outdoor recreation opportunities located on or near the seashore. Unfortunately Anne’s arthritis plus excess weight, creates an impediment on outdoor adventure. Anne stays extremely cautious about attempting new activities in hopes to prevent possible arthritis flare-ups. Furthermore, she even limits her stair usage in her split-level house.
