Ms. Pedroso is a pleasant 36 years old, pregnant females. She became pregnant after a first in-vitro attempted. Currently, the patient is 25 weeks of gestation and has come to the clinic with a chief complaint of recurring heartburn, which she described as a flame-throwing sensation in the epigastric area, abdominal bloating, and a sour taste in the back of her mouth. Ms. Carrillo states noting her symptoms two weeks ago and verbalized the symptoms worsen after eating; particularly after a heavy meal and with certain foods. She describes her pain 8/10 on the pain scale. The patient denies any chest pain or shortness of breath. Ms. Pedroso only known health problem is primary hypertension.

Mr. Clifford a 57 years old Hispanic patient presents to the urgent care, complaining of episodic epigastric discomfort, which he describes it usually begins a few hours after eating a meal. The patient, also reports other symptoms such as heartburn, bloating, abdominal fullness and indigestions. In 2003, Luis Alberto Jimenez, an undocumented and uninsured immigrant, was involved in a car accident where he sustained traumatic brain injuries with an intoxicated Floridian driver. After the accident, he was hospitalized at Martin Memorial Medical center, a private community hospital in Stuart, Florida. Because Jimenez required ongoing care without medical insurance, Martin Memorial was not able to find a rehabilitation facility willing to accept him. Instead, Jimenez remained as a patient of the hospital for many years inducing more than $1.5 million in costs. Of the $1.5 million, Martin Memorial only received $80,000 from Medicaid for the emergency services provided to Jimenez and absorbed the remaining costs associated with his care directly into their budget. Faced with the option of continuing Jimenez's medical needs and the financial costs borne of this care or deporting him from the state, Martin Memorial secured a state court order to authorize the transport of Jimenez to a medical center in Guatemala. Under this court order, which was later deemed invalid upon court issue, the hospital leased an air ambulance at its expense and forcibly transported Jimenez back to Guatemala. I understand that you are going through a series of frequent and agonizing heartburns. The heartburn you are encountering is from eating substantial dinners during the evening. The physician examined you with an endoscope, which is a thin and flexible tube equipped with an optical device (DeBruyne and Pinnap, 2014, p.494). The results of the examination indicates evidence of a medical complication of reflux esophagitis and esophageal stricture. According to DeBruyne and Pinna (2014) , the term reflux esophagitis is inflammation in the esophagus related to the reflux acid stomach contents (p.493). Due to the inflammation in the esophagus, it begins to narrow and tighten, which is what we call esophageal stricture.
