At around 0953 on June 8, 2017, I contacted Mrs. BV the director of the geriatric social worker who is in charge of older adults home, long-term and nursing facilities visitations in the community. She has few different ways of collecting her data, one of them is by interviewing residents face-to-face in their homes. She stated that this method has been efficient because she gets honest information from the residents by speaking to them directly. She also reviews long-term and nursing facilities including the city health department data to compare with her assessment findings. After evaluating all the data, community-acquired pneumonia was identified as the priority health problem due to the lack of knowledge on signs and symptoms of the disease.

During my undergraduate career, I worked with the Martin Center. The Martin Center is located in Indianapolis, IN, It is a center for Sickle Cell patients to receive help in terms of finances, food supply, mental health, transportation, and various forms of assistance needed. The Organization is struggling terribly with recognition, receiving sufficient funds, and providing services to patients due to a lack of resources. This project focused on creating awareness of the organization, and also how to increase the funds the organization receives.
In order to work effectively and appropriately with vulnerable and marginalised groups in society such as individuals with mental illnesses, it is fundamental for a social worker to have a comprehensive knowledge base and proficient skills.
Over the last century health and social care has changed and developed immensely with the implementation of the NHS and becoming a welfare state. Prior to this health care was only available to a minority of people, those who could afford it. In 1815 the old poor law was passed, it stated that each parish must look after its own poor and those who could not work were provided enough money to help them survive. By 1830 the poor law cost around £7 million, which came from taxing the middle and upper class, causing a sense of resentment towards lower class, unemployed people. In 1834 the new poor law was passed, its aim was to reduce the cost of looking after the poor. Workhouses helped with this, they provided clothes, food and healthcare in return for manual labour. Stereotyping is when a judgement is made on a group of people because of the actions of a few. Stereotyping happens because of misinformation and other people’s opinions.
Providing quality care to those who need it shouldn’t be difficult but sadly at times it can be; in an ideal world some effective ways to assist carers would be assessing the situation as a whole not just the pwd. Within a community there should be relevant teams that can be contacted such as district nurses, charity organisations to offer support, social services should be able to help with extra care if and when its needed, local care facilities can be helpful to provide respite all these areas would need to assess the pwd and should then be able to put to together a care plan that suit the pwd and the carer. Supporting the pwd as well as their carer is important and there for help may be needed for the carer to be able to go out if they choose or may be overnight care so they can get some well earned rest.
As I observed the N.A. group, I compared Hepworth 's, Direct Social Work Practice, five stages of group development. The Preaffiliation stage involves observation and feeling out the environment of the group; members may be hesitant to speak or test out certain behaviors to see reactions from other members of the group or the facilitator (Hepworth, Rooney, Rooney, & Strom-Gottfried, 2017, 2013). Returning group members greeted each other with hugs, handshakes, pat on the back, while newcomers sat quietly observing others. Social process can be defined as the pattern of social interaction that in turn has a changing effect on society over a set period (Bain, 2017). It has an important role in molding society. It has an impact on service users within the health and social care sector. This can lead to marginalisation, isolation and exclusion. For example, unemployment, disablement, poverty, poor health lack of education and other sources of disadvantage can have a negative impact on the demand for health and social care services.

On this date worker visited Mr. Thomas Bagwell at his residence, for the purpose of monitoring his situation. Mr. Bagwell greeted worker at the door. The home was not cluttered and did not present an odor. Mr. Bagwell was appropriately dressed with good personal hygiene. Mr. Bagwell stated he was no longer receiving dialysis and Healthsprings was offering transportation to and from doctor appointments. According to Mr. Bagwell he likes living in the apartment and believes with rent and utilities lowered he will now be able to purchase groceries. Mr. Bagwell stated someone from M4A is working on getting him QMB approved and someone would be out to talk with him about the waver
There were many Black males from the ages 18+ that were taking the information and were easy to communicate with. Based on what my group did the majority of the bags we gave out was to Black males. When it came to testing, I noticed that the majority that got tested were Black women even with incentives being given. This told me that there are many men having sex, but not many who know their status. That being my opinion, and not a fact based on my observation. Based on what I observed women are more likely to get tested and take care of their health than men.
My placement is at Gateway Youth in Elmont. Gateway is a prevention program specializing in working with youth between the ages of 5-17 who are in conflict with their family, in need of academic or emotional support, or who are involved in the juvenile court system. Services provided include case management, crisis intervention, and advocacy with the schools, court, and community. As a social work intern, I am placed at Gotham Ave Elementary School working with third graders. At this school, Gateway Youth provides an after-school program for students that need supervision with their homework and inclusion in activities before their parents can pick them up.
