Orientation Stage—involves nurses learning about patients and they initial concerns and needs. Patents also learn about the role of the nurse. Patents are informed about the general purpose of taking with the nurse. The initial purpose is plainly: identifying a problem on which you want to work, helping you figure out what has been happening to you lately, or getting to know what has been bothering you. This first stage is about building trust first before moving on to your initial assessment. Afterwards the nurses job is to manage the patients emotions and providing support and structure to the pt.

There are many ways of forming a relationship and gaining the trust and respect of the patient and I had to work out the different things that make a good therapeutic relationship. According to Hinchliff et al (2003) there are a number of important elements that make a good therapeutic relationship, but it is important to make clear that a therapeutic relationship is a formal relationship between a medical professional and patient. The Nursing and Midwifery Council (2008) maintains that at all times nursing staff must maintain appropriate professional boundaries in the relationships they have with patients and clients.

A therapeutic relationship is a key component in the nursing profession. Without therapeutic relationships, the best possible care can never be provided. The foundation in which trust is built upon is created from the nurse’s ability to truly listen and respond appropriately. Listening creates the base in developing a strong, trusting relationship. Sometimes it is simply hearing what a patient says that makes all the difference, empowering them to open up and become more comfortable with the nurse (Hawkins-Walsh, 2000).

When culturally competent nurses interact with other cultures, they observe how members of the culture communicate, watching for verbal and non-verbal cues. Eye contact is important to identify. For some cultures direct eye contact is not polite. Touch for some cultures is prohibited. If needed, nurses should explain the reason for touching the patient before proceeding. In some cultures males cannot be caregivers of females. Silence doesn’t always mean miscommunication or patient apathy. For some cultures is a positive non-verbal cue or a sign of respect or agreement. Space and distance is very important when providing care. Patients can place themselves close or far from the nurse based on their culture. Healthcare beliefs also vary from cultures. Some cultures are compliant with cares while others may not be very cooperative. Based on these observations, nurses can plan their plan of care based on the patient’s needs (Maier-Lorentz, 2008 Journal Of Cultural Diversity). Nurses also need to observe pain non-verbal cues, food preferences, family arrangements and general norms and interactions.

Creating a safe environment, promoting trusting nurse-patient relationship, and increasing patient’s quality of life are result of implementing this theory in practice. Even though creating a change in nursing practice might be challenging, but it is doable by identifying the goal, pathway to reach the goal, education, trainings, and using evidence based practice. Nurse and patient relationships are referred to therapeutic relationships, they are a person-centred approach to care (Berman Et al 2012). As a nurse, communication is an essential and important factor to building a therapeutic relationship between a nurse and patient as it is the difference between average and excellent nursing care, as it helps maintain a good quality of life and allows nurses’ and patients to interact and provide comfort when needed. Based on Peplau’s interpersonal theory, communication takes place in a nurse-client relationship where therapeutic process occurs involving complex factors such as environment, attitudes, practices, and beliefs in the dominant culture (seu.edu, 2015). The actions of each person in a nurse-patient relationship is measured on the collaboration of their thoughts, feelings, and experiences. Nurse’s work to attain, maintain, and restore the patient’s health until patient have fulfilled the health care needs. Patient must be guided and provided a well-respected environment until a better health and specific needs are fully considered in the relationship. In this kind of setting, nurse’s must create relationship with patients by communicating receptivity, assimilating the concepts of empathy, trust, genuineness, respect, and confidentiality into their interactions.

According to Pepau the goal of nursing is to help the patient find their felt problem. The nurse and patient work together therefore they develop relationship between each other’s. These two individuals with common aim have interpersonal relation. Nursing is therapeutic in that it is a healing art, assisting an individual who is sick or in need of health care ( Pepau 1952). Nurse plays several roles in this relationship. As a resource person, nurse gives adequate information that help patient understanding of issues. Within nursing, there is a very delicate balance between a nurse and her patient that must be maintained if the patient is to receive the care that he or she is entitled to receive.
