As seen in this case study, TM was listed as a full code and no GOC conversation was had between the physicians and the patient until the day of his discharge to home. This led to an unnecessary swallowing study in which the patient needed to be uncomfortably scoped. Due to the lack of GOC conversation the patient also chose to go home with general nursing services instead of end-of-life (EOL) hospice support. In the inpatient medical oncology floors of a large teaching hospital like this one, most patients are covered by rotating interns and residents that are not comfortable having GOC conversations with patients and leave it up to the primary physician to come and discuss.
A broth in test tube labeled #2 was used to identify the unknown organisms. A T-soy agar plate was the first media I inoculated. I used the streak isolation technique to isolate a pure strain from colonies, so that organisms can be identified. I inoculated a MacConckey to help me identify my Gram negative organism and observed that the organism was a non-lactose fermenter, then I performed a Citrase test which was negative. The last test I performed was a TSI, the butt was positive for hydrogen sulfide production and fermentation of glucose. After Doris was sent home with two types of medication, she should really read closely to what the bottle says. Medication A is to be taken every four hours by mouth and medication B is to be taken three times a day by mouth. Usually when some medication is three times a day, it has to be taken with food, that way it can be taken with meals. The medication that is every four hours probably will be okay taken on a empty stomach. Since these medications are liquid, the doctor will advise nothing to be ate or drank within 30 minutes of taking. Liquid medications are usually made for coating the throat/intestines. The United States is the only country that is troubled with this situation because it is only here in America where our members of Congress, who are elected to represent us as citizens, chose not to represent the nation as a whole, but chose to represent their appetite for riches instead. European countries are not forced into the same predicament the U.S. is in because they have developed a more acceptable and beneficial approach to healthcare, thus creating an utopia compared to the troublesome Americans.
