Altered mental status is a decreased state of consciousness which result in decreased activity and altered perception among other symptoms (Goolsby & Grubbs, 2011). Altered mental status is more of a symptom that is very vague, it that can be as a result of several diagnoses such as delirium, stroke and normal pressure hydrocephalus (Goolsby & Grubbs, 2011).
