Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice. Exit Plan Strategy Plan: Ms. Medina’s new Exit Strategy plan is to continue seeking employment and obtain employment by 12/2016, have enough savings to cover one month’s rent and one month’s security by 03/2017. Session 1: During this face to face session on 8/8/16, MHS addressed the following ADLs: Medication Monitoring, Community Involvement, and Functional Skills. MHS asked the client if he had taken his medication today. The client reported that he did take his medication. The client mood was cheerful and inviting. The stated he is still looking forward to working when he can. The client stated he gets bored sometimes just sitting at home. The client requested to be accompanied to the library. MHS praised the client in making progress for independent living (20 mins). I sent an email to the Director Mr. Robert and the supervisor Mr. Ashwin about the results that I have found after analyzing data. I have found interesting results. I found there are a statistically significant variation in the number of ER visits and inpatient hospitalizations for Zip Codes 65616. I met with the Director Mr. Robert and the supervisor Mr. Ashwin and discussed these results.
Housing Update: client NY NY I, II was approved. Client is waiting for DHS manifest to tour apartment. Another alternative housing is MRT once client SSI is approved. Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
On 01/28/2016 Ms. Soraya Johnson came to the SSO to meet with CM Watterson for an ILP meeting. HS De La Torre inquired to Ms. Johnson to stay a little longer to develop a Housing Plan. Ms. Johnson agreed and the following was discussed during Housing Plan.
Exit Strategy: Ms. Williams is linked to an apartment and is expected to move into permanent by 03/31/2017.
Client stated that she understands. Case Manager also explained to client that once she becomes eligible she will be expected to obtain permanent housing by 08/2017. Client stated that she understands.
This officer has had contact via face to face and phone with the mother, Ms. Evelyn Edwards. Discussions have taken place with her regarding Marquel’s Comprehensive Re-Entry Case Plan. She has participated in a video conference with Marquel since his commitment. She is aware of his placement in the Virginia Beach CPP Program. She has not visited him at the facility, but she has had contact with him via phone. She is currently unemployed. Upon Marquel’s release from DJJ, the anticipated parole plan (CRCP) will be for him to return to her home with intensive supportive services via 294 funding. On March 13, 2017 Donald Foster request Family Medical Leave of Absences to take time off to assist his wife who he said had been experiencing difficulties with severe illness . At the time his request he had just returned to work after exhausting previous approved FMLA. To facilitate Officer's Foster's request he was provided FMLA documentation by security ERC Mrs. Tara Crane. Foster was instructed to take the paperwork to his doctor and have them complete the FMLA paperwork and return the paperwork the next day.
On 8/7/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting; she was alert, satisfactorily groomed, and casually dressed. In the meeting she was cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly ILP Review. Client reported she is exhausted from running around obtaining relevant documents to be submitted to Lemle & Wolfe, Inc. by 8/3/2015. She continues to reports she has Straight Medicaid and she is searching for a Managed Cared that cover oxygen tanks.
Ms. Pollard wanted to know when Javon'Tay would receive the DD waiver. ISC explained the process, than she asked about receiving a case manager. She said she was told by the other lady he would receive a case manager, which would take him out to his appointments and take him places.
On 11/13/15, there was a mandated meeting amongst Supervisors and Housing Specialist. The meeting was expected since the end of the month of October. It has been slow in the matter of helping families to transition into permanent housing.
Public Assistance: Ms. Gosk public assistance remains open. Client did not inform me of any changes.
At this time, the timeliness of your move in, is contingent on you making your scheduled appointments with me so that the required paperwork can be completed. We have several items that need to be completed. That cannot be done, unless you come to the office and complete them. I usually create my schedule a week or two in advance, and can fit unexpected meetings between prescheduled one’s. Since this program primary focus is on self- sufficiency, I work with participants with appointments only. Therefore, I ask that you contact, before you attempt to come to the office, since I may have other appointments scheduled at that time.
