R/s the apartment Manager Gloria has been notified but went off on the reporter. R/s it is hearsay that the carpet will not be removed. On this date worker interview, Mr. Ronald Darty, son of Ms. Linda Darty for the purpose of conducting a PARN interview. When worker arrived Ms. Darty was in a hospital, bed located in the living room and did not respond to worker. However, worker and Mr. Darty exited the room and the interview was conducted in a bedroom located in the back of the trailer. Mr. Darty stated he is 42 years old 4/29/75 and has lived with his mother for 42 years. He promised her when she got sick he would never put her in a nursing home. However, recently he isn’t able to provide the level of care she requires. Mr. Darty stated she has a bedsore on her back and when he changes her or rotates her, she screams out in pain. The complainant was flatulating throughout the night which caused Lynn to kick her out of the bedroom because of the smell. The complainant went into the living room and was unable to go back to sleep and became bored. The complainant decided to put Christmas decorations up throughout the house and on the Christmas tree. The complainant was in the bathroom when Lynn approached her and stated “Why you put decorations on that girl tree” referring to Kiyanna. The complainant told the Lynn that if Kiyanna didn’t like the decorations Kiyanna could take them down. The complainant and Lynn continued to argue and Lynn told the complainant to leave. R/s Donald Burgess has peripheral vascular disease, COPD, chronic pain, left side weakness, reoccurring blood cloths, and fluid retention in left leg. R/s Mr. Burgess is living in his enclosed garage. R/s Mr. Burgess warms by a space heater and an electric blanket. R/s it was reported that the space heater is no longer working. R/s it was reported that Mr. Burgess hadn’t bathe in some time now, his pants were torn, and the garage is very dirty. R/s Mr. Burgess uses a bedside commode. R/s Isaac Harris (69) has dementia, hypertension, coronary, and heart failure problems. R/s Mr. Harris is not oriented to place the date, time, or people. R/s Mrs. Harris is staying with his friend Carolyn, she can no longer care for him. R/s on 8/25/2015, Carolyn filed a missing person for Mr. Harris, 3 days later he showed up at Province Hospital. R/s Mr. Harris is known for wandering. R/s on 08/15/2015 he was at the ER, there were concerns about his failing mental status. R/s Mr. Harris leaves home without clothes. R/s Mr. Harris cuts his clothes and cuts were on his legs. R/s Mr. Harris has two bottles of Digitalis medicine that were fill on the same date, and there is a concern about him taking double dosage of the medications. R/s Mr.
The resident's door opens to the outdoors which left the resident and his roommate exposed to the cold of the night time environment. Both residents are non-ambulatory and therefore unable to close the door themselves. Subsequently the resident had urinated on himself due to his inability to get up and lack of help. The resident was soaking wet which exacerbate his hypothermic state. The RP stated the facility is substandard and lacking residents care. The caregiver took 20 minutes to retrieve any documents pertaining to the resident. The caregivers appeared very under trained. The facility is not up to code and that the Fire Prevention Division had been to the facility frequently to address the violations. The smoke detector in the room was hanging 12" down from the ceiling by the electrical wiring and the only working light into the room was from the bathroom light. It was reported that resident Susie Elbert (age 52, DOB 10/28/63) has been observed sitting in a vehicle (Silver Toyota Camery, License #7PDB535) parked outside of 10 San Pablo Ave., San Rafel for up to 5 hours at a time while caregiver Marc Anglade is inside of the apartment. It is reported that the apartment is the home of Marc's mother. The resident has Down's Syndrome and dementia as well. Caregiver Gloria stated that she has only seen this happening for the last wee, but neighbors told her that this has been happening for a really long time and nobody knew what to do about it other than to confront Marc. Today Gloria contacted RP, as she realized that she was a mandated reporter, to report that the resident had been sitting in the car

R/s today Des’tani’s teacher was advised by the bus driver on that on yesterday Des’tani’s step grandfather, Christopher smelled very heavily of alcohol. R/s Des’tani grandmother, Brenda and Christopher care for child while mom works. R/s Des’tani has severe behavioral issues and she is difficult to care for. R/s one time there was an issue with roaches and bugs crawling out the child’s backpack, but nothing recently it gotten better. R/s school district’s social worker reached out to mom to offer services to help her. R/s other adults possibly Jacinda’s sister and brother are living in the home.

On June 16, 2016 CPS Keturah and Cece both spoke with Archield Ms. Brogan regarding the allegation. Mrs. Brogan stated that she hasn’t had any altercations with anyone lately. Ms. Brogan’s two kids wasn’t at home, she stated that the boys were at the tire place around the corner helping the owner of the business wash cars, than later stated the boys were around the corner at a friend’s house . Ms. Brogan stated that they she has to provide services for the roaches within the home; she previously sprayed and set glue traps for the roaches. Ms. Brogan receives disability, but she will be going to court and they might be taking her disability away from her, due them feeling that Ms. Brogan is able to go back to work. On 9/8/2015, the insured reported that on 9/30/2015 they noticed a hallway carpet was wet. They then tracked water to AC closet. AC contractor was called and found that pump had failed and float switch was not working. We have attached his invoices for your review.
CCIB received the complaint via phone call from a Todd Sabin stating that on 4/19/2017 at 4:30 pm, resident Madelyn (unknown last name) fell out of her wheel chair and in the dining room. RP reported that Madelyn was bending over in her wheelchair and then fell out of her chair. RP reported that staff member Abraham stated in front of all the residents in the dining room that Madelyn “does this every weekend”. RP stated that he did not think that Abraham should have said that about her in front of others.
