Capstone Research Paper: Therapeutic Services For many Americans, there comes a time when people are in need of therapeutic services and need help finding the right treatment for them and or loved one. Often times therapy is very beneficial to people, but is not always offered with their insurance. Therapeutic services should be provided to all patients as a non-evasive treatment option because these services provide nontraditional alternatives that enhance patient care and therefore, insurance companies should cover these services. Music Therapy is used for many reasons and can be helpful in many ways. Music has been proven to be a therapeutic service to people because it’s helped people to contain themselves and even express themselves through the music that they listen to. This treatment is beneficial to many humans because it’s a good way to help one’s emotions without any medication (American Music Therapy Association). For example, music is used to treat pain and reduce stress. In Amy Novotney’s article about music therapy, she says, “The beep of ventilators and infusion pumps, the hiss of oxygen, the whir of carts and the murmur of voices as physicians and nurses make rounds — these are the typical noises a premature infant hears spending the first days of life in the neonatal intensive care unit (NICU). While the sounds of such life-saving equipment are tough to mute, a new study suggests that some sounds, such as lullabies, may soothe pre-term babies and their parents, and even improve the infants' sleeping and eating patterns, while decreasing parents' stress (Pediatrics, 2013)” (Novotney). Another example of how music is a reliable therapeutic resource is for people with autism. Yet many people still aren't aware that the law exists or how it affects them. In fact, a 2014 APA survey found that more than 90 percent of Americans were unfamiliar with the mental health parity law (American Psychological Association). Many people might end up not receiving the appropriate therapeutic services that they need because their medical insurance does not cover it. However, medical insurance does cover some of it. For example, for physical therapy, some insurers require a referral from a doctor. People consider physical therapy to be a necessity as long as it's prescribed by a physician and significantly improves physical functions that were limited because of a disease, illness or surgical procedure. On the other hand, others do not consider physical therapy to be necessary for someone whose condition is getting worse or if the patient is not showing improvement. (Holmes). Many people question and ask themselves why doesn’t my insurance cover these needed services, and what can I do in order for my insurance to fully cover it? I knew about music therapy before my paper because I was actually going to major in it for college. It is interesting because I honestly knew so little on how and what music therapists really do in their sessions. One thing I learned and found extremely interesting was how music therapy can help children with autism identify emotions. Music therapy can help individuals identify the correct emotions and expressions of themselves and others around them. An example is if the music therapist plays a sad song, the child will internally feel a little sad inside, they can sing words or say expressions that relate to sadness, and then they can look in the mirror and see their own sad face while also looking at the music therapists sad face (Music Therapy Center of California, 2005, p. 4). I thought it was so interesting to know that music can help children understand emotions. Music therapy can help to lessen negative self-stimulation and it can increase participation and social skills among peers in more acceptable ways (American Music Therapy Association, 2012, p.1). Another big thing I learned from my research is how much music therapy can help a child with autism’s language skills. Music therapy can help children who do not have verbal language express themselves and communicate. The music can also let them participate freely. The connection between the actual music and the interaction of play, such as listening, taking turns and responding helps them with their communication skills (American Music Therapy Association, 2012, p.1). Some personal experiences that I can account for is when I volunteered for a musical theater production called Razzle Dazzle. This was a music production with adults with disabilities. Their disabilities ranged from Down syndrome, to autism, to simply just an intellectual disability. One man by the name of UNKNOWN, was a dancer in our show.
“The American Music Therapy Association (AMTA) defines music therapy as ‘an established health profession in which music is used within a therapeutic relationship to address physical, emotional, cognitive, and social needs of individuals’” (Barnett & Shale, 2013, p.48). Music Therapy (MT) is shown to be able to help people with many different kinds of mental health problems such as anxiety, stress, and minor cases of depression. There have been many studies done showing that MT is an effective form of therapy that can improve someone’s overall Quality Of Life (QOL). MT is known as an alternative technique therapy. It is often used instead of, or along with, medication to produce a result in the patience who need it. MT involves all aspects of the music process, including listening to, writing, singing, and analyzing music. Overall MT addresses physical and emotional problems and is used to enhance the life and health of the patient. There are many different ways that we benefit from music. Music can inspire better self-esteem, and confidence. It’s a great way to set the mood, and a wonderful tool. Music therapy is the clinical and evidence-based use of music interventions to accomplish individualized goals within a therapeutic relationship by a credentialed professional who has completed an approved music therapy program (Gram, 2005). It is surprising to realize the practice of one small therapy could impact the world in a big and positive way. Music therapy is the practice of using music to better the lives of people with motor, cognitive, and speech disabilities, along with surgical patients. This is achieved by redirecting neural networks in the brain with certain types of music selected for that specific patient. This is not a field for every musician however, only educated students who are board certified should practice music therapy for the safety for the patients. Although many people believe music therapy should not be considered a therapy, it should be considered a mainstream beneficial medical practice and used worldwide. It greatly increases the education of mentally disabled children, surgical patients have a safer option for pain reduction, and it creates employment for people who are interested in a career dealing with music.
According to Geretsegger et al. (2014), music therapy is an effective clinical intervention for a varied of medical conditions, which are profoundly supported by strong scientific evidence on their significance for mood enhancement and stress relief. The five conditions for which music therapy has been studied, backed up by good scientific evidence are; dementia, autism, sleep quality, depression and infant development.
An interesting finding by Huskamp and Iglehart (2016) is that between the Mental Health Parity Act (MHPA) and the Affordable Care Act (ACA), there should be a rise in people with mental illness seeking services, however, services are underutilized more than ever (Huskamp & Iglehart, 2016). According to Bendant (2014), in 2013 a total of 47 million people uninsured and 25% of those suffer mental illness (Bendant, 2014). These numbers are staggering for a service that is more inclusive now more than ever. The Center for Medicare & Medicaid Services (CMS) site indicates that the MHPA is regulated differently depending on the state (CMS, n. d.). The Department of Labor (DOL) has identified a few problems with the MHPA and addressed these issues to Congress via a 2017 report (DOL, n.
The Mental Health Parity and Addiction Equity Act of 2008 (MHPAEA) is an act that requires parity or equality between mental health treatment and medical/surgical treatment covered by private and public insurers with over 50 employees. That means that if an insurance covers mental health issues they can’t impose more stringent limits and financial requirements than medical/surgical coverage. The act was signed into law in 2008 by President George W. Bush. Before the act was signed into law, mental health care was not as affordable or accessible for individuals. Research has shown that a strong therapeutic alliance is necessary for establishing a beneficial contact between the therapist and the client. If the therapist does not encourage the creation of a reliable therapeutic alliance from the beginning of the treatment, it will be hard to develop a constructive relationship with the client later. Establishing the therapeutic alliance will increase the chances of achieving the goal of the treatment because the clients will be willing to cooperate if they trust and respect the therapist. Music therapy, a clinical use of music interventions to accomplish therapeutic goals, involves a broad range of activities including playing an instrument, singing, or listening to music. Similar to occupational and physical therapy, this expressive arts therapy remedies psychological conditions, such as depression, anxiety, or hypertension to maintain the well-being of an individual. Likewise, music has been a therapeutic tool that has shown positive effects to parts of the brain including regions involved in emotion, sensation, movement, and cognition. Although music therapy is a somewhat new-found treatment, it is used prominently today. Administered by a trained therapist, this type of therapy is used in correctional facilities, nursing homes, hospices, and special education schools.
Aside from emotions, music can also have an impact on mental illnesses like ADHD, depression, and dementia. To combat depression, Cadena states that “music therapy alleviates pain and promotes calmness by slowing the heart rate and other bodily functions” (2). It provides the patient with a means of escape from the stressful and painful ordeals of life, and can even encourage them to recover from depression. To help children with ADHD, music therapy and medications can be used conjointly. Music, Cadena affirms, “can provide a calming, sedative affect and assist a child with focusing on the task at hand” (2). In regards to dementia, a study was done to investigate the effects that music would have on people with the disease. The findings demonstrated that “mean agitation levels were significantly lower while listening to music than before listening to the music” (3).
A song or a piece of music has the power to transform an individual and take them not only to a different state of mind, but quite possibly a different consciousness. Making or singing music also has the power to transform an individual. “There is a resemblance of music in pitch, volume, rhythm, melody and structure to the natural human expression of emotion in voice, demeanor and behavior” (Ushedo, 2006, 1). Making music can create an environment where it indirectly teaches individuals communication and social skills through the emotions brought out by the music. The social interaction between individuals with behavioral problems has been aided by using musical activities. It can “encourage or motivate communication through musical activities, which are responsive to the child to promote eye contact, turn taking, sharing, joint attention” (Stephenson, 2006, 297). While creating a calmer and more relaxed environment with musical activities, behavioral problems can be aided through the use of this form of music therapy.
