At Amery Behavioral Health, the staff all work together to plan the care of the patients. There is a nurse manager who is in charge of the nursing staff on the unit, but she was more than willing to help out on the floor to get people out of bed, feed patients, hand out medications, and answer any questions. The charge nurse selects the care of a certain amount of patients, while the LPN takes a couple more than the RN depending on the amount of patients. There is also two CNA’s to help on the floor. Around 6:45 a.m. is morning report for just the nursing staff where they listen to report from night shift and then divide up the patients. At 8:30, there is a meeting with the social worker, occupational therapist, psychiatrist, and charge nurse.
In every profession there are changes that propel how tasks are done; nursing is no stranger to this. One of the biggest changes that have come into nursing’s daily events is how report hand-offs are being done. Gone are the days of taped report that each off going nurse must tape about each patient and the oncoming nurse must listen to. Nurses are now being encouraged to move their report to the bedside, in front of the patient (Trossman, 2007). A common goal all healthcare providers share, is the desire to provide excellent patient care. The delivery of care is constantly changing in healthcare, however, the patient will continue to remain the focus of care. The success of nursing care thrives off the ability to fulfill patient needs and to maintain patient safety and satisfaction. When patients are admitted to the hospital, their need for an increase in their level of care and attention, due to the decline in their health status, and inability to preform normal daily activities of daily living. The loss of independence places the patient in a vulnerable state of mind, causing the individual to rely on members of the healthcare team to assist with basic self-care needs while in a stable and well-organized environment. A structured environment can be accomplished through the practice of hourly rounding on all patients.
AT 7.15AM I CLOCK IN AND TAKE MY COAT AND BAG OFF AND LOCK THEM IN MY LOCKER SO I AM READY TO START MY SHIFT STRAIGHT AWAY, EVEN THOUGH I SOMETIMES SOCIALISE WITH SOME OF THE STAFF I WORK WITH I ENSURE I AM IN THE RIGHT FRAME OF MIND TO WORD PROFFESIONALLY WITH THEM AND NOT PERSONALLY SO THAT WE ARE ABLE TO WORK AS PART OF A TEAM AND ARE ABLE TO DO THE JOBS THAT WE ARE EMPLOYED TO DO, THEN I WILL WALK INTO THE DINING ROOM TO WAIT FOR THE NIGHTSTAFF WHO ARE ON, ONCE ALL THE STAFF FOR THE MORNING SHIFT HAVE ARRIVED THE SENIOR MEMBER OF THE NIGHT SHIFT WILL GIVE HANDOVER TO ALL THE STAFF WHO ARE STARTING THE MORNING SHIFT. THEY WILL GO THROUGH THE LIST OF RESIDENTS
Every morning the charge nurse from the night shift does a huddle between 7:30am and 8am. All of the staff members participate in the huddle, including the ones coming and leaving. The charge nurse gives a report of the main points of what is going on in the unit, how many patients they have, who is going to be admitted, or transferred. Also, if any patient is on one to one care. The charge nurse mentioned the acuity of the patients. They give thanks to nurses and patient care technicians who did an excellent job. The manager gives a brief up date of how the day is going to be, and any news she has for the staff.

I supervise the unit’s operations and oversees specific employees at the BHT II and BHPP II level. I am also part of a multi-disciplinary clinical team to provide recovery-based services for consumers in varying levels of crisis. Others include performing nursing assessment, developing a medical service plan and obtains and executes medication orders from psychiatric prescriber. As an RN supervisor, I oversees and maintains medication and medication records in accordance with Agency policies and State regulations. I also coordinates medical care with psychiatric prescriber and PCP. It is my responsibility to serve as a role model of professional behavior at all times.
Miscommunication and missed information, resulting in potential errors, have been on the rise at Pelham Medical Center. In the past, the primary nursing staff was giving verbal report to oncoming nurses at the nursing stations. There are many disadvantages to this practice. Verbal report at the nursing station is distracting with so many nurses talking at the same time and is frequently interrupted by other staff, call bells, and family members. Our country’s current economic crisis is drastically affecting not just individuals, but businesses and organizations as well, especially when it comes to those that are non-profit. Funding for necessary programs held by non-profit organizations, such as Moses Cone Behavioral Health, have been cut annually, making it more difficult for their institutions to prosper. Due to these substantial declines in donations and funds, local volunteers are needed now more than ever. I chose to volunteer at Moses Cone Behavioral Health Hospital to help combat this loss in funding.

As part of my practice I had the opportunity to witness a shift handover between supervisor and nurse manager who then verifies the population of each department, the staff had in the shift, if there were absences, the category of patients and if necessary reassigned staff from one department to another where there was the need always taking into consideration the competence of the employee to be moved that will be able to provide the service. If absences arises for other shifts the program is verified for possible movement and if not being able to perdiems are approved or overtime but the staff is completed to not affect the service and expose personnel to make mistakes that with good programming can be avoided.
This can be a challenge for night nurses especially when they need to get hold of their managers when they need consultation about certain issues or policies. Although there may be an administrative supervisor at night, they might be very busy and it may not be easy to get hold of themespecially when one needs help immediately. In most hospitals there is one house supervisor who is in charge of everything during night shift. If multiple units or floors need help at the same time, the house supervisor is not able to help both units at the same time. He or she is to pick what unit has the most crucial needs. Dayshift to has a lot of employees available these include managers and supervisors. This makes it easier to get help during the day compared to nights. Night shift tends to have more new graduate nurses and fewer resources than day shifts.(Mensik & Sigma Theta Tau International, 2012)Itis important to ensure that each night shift has an expert that can help with triage or incase if a patient starts to deteriorate. Expert nurses can help the new nurses better manage their patient load and also provide mentored experience. Resource nurses,who are experts and have the experience and competency to assist the new graduate nurses, tend to be very helpful. Day shift has more expert nurses who have a lot of experiencecompared to nights. An advantage of this is that they are able to make decisions faster due to lots of experience especially if its in an emergency situation.Night shift has several psychological effects on night workers. This includes restlessness, sleepiness on the job, fatigue, and decreasedattentionand disruption of the body’s metabolic process.
