On 08/20/2016, at about 1945 hours, in Durango 4 located at 325 W. Gibson Lane, Phoenix, AZ 85009, Officer Smith B3168 informed me that an inmate came to him claiming he was assaulted. I escorted (V1) Inmate Marino, Nicholas MCSO Bk# T292955 to Durango Medical Clinic to be evaluated for any injuries he may have received. While we were in transit to medical, I asked (V1) Inmate Marino what happened. (V1) Inmate Marino stated to me “That right after chow was served a couple of inmates came to his cell (cell #49) and told him to go to the bathroom. On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
On Saturday, September 12, 2015, I was assigned to the 6 Northwest as the security officer. At approximately 0500 hours while breakfast trays were being distributed in pod 100, an inmate, later identified as Kimmie Hawkins became insolent and advised me to get out of the pod. I immediately asked him whom he was talking to and simultaneously walked over to him to view his armband. As I reached for Inmate Hawkins’s left wrist, he snatched away from me, got in a boxing stance, and began bouncing rhythmically on his toes. Therefore, I retrieved my T.A.S.E.R. from its holster and gave Inmate Hawkins loud verbal commands to get on the floor. However, he refused to follow my orders; therefore, I pushed him toward the wall and continued to instruct
On 10/18/2016, at approximately 1840 hours, at the Durango Jail located at 3225 West Gibson Lane Phoenix, AZ 85009, upon working in Durango 8 Control, I noticed that the majority of the inmates in the housing unit were gathering around A-row for what appeared to be a fight. I notified my partner Officer Carroll B2173 that there was a fight going on in A-row. Officer Carroll then made a radio call to Security Control for additional officers and placed Inmate (S1) Meyers, Donald T281833 and Inmate (S2) Carter, Clay T310426 in handcuffs until additional Officers arrived. Durango 8 was called code 4 at approximately 1842 hours.
On 8/28/2015 at about 1608 hours at the Lower Buckeye Jail, located at 3250 W. Lower Buckeye Rd. Phoenix 85009 , I responded to an Inmate-Inmate fight radio callin T14 B16. Inmate Dibbern, Leon Booking Number T184564 and Inmate Garcia, Jonathan Booking Number T068124 were both involved in the fight.
Officer’s Norman B0245 and I assume custody of Inmate Freiday at the Lower Buckeye Jail Main Clinic. While at the Main Clinic I asked Inmate Freiday if his chest pain was the result of any criminal action against him while in MCSO custody. He replied “No.”
On 04/22/2018, at approximately 1900 hours, while in Durango Jail located at 3225 W Gibson Ln Phoenix, AZ 85009, I was assisting medical with medication distribution in Durango 5 A-pod, I noticed Inmate Staggs, Joshua T450226 come out of D5 A-pod and told me he needed to go to medical for his jaw. I asked him what happened and Inmate Staggs told me that he fell in the shower.
On 07-05-2017, at approximately 1611 hours, I responded to a man down call that Officer Steffa B2480 made to Security Control in Durango 7 B pod bathroom of the Durango jail located at 3225 W. Gibson Ln. Phoenix AZ 85009 for an attempted suicide. As I arrived on scene I noticed that Inmate Olson, Russell T376200 had cut his wrists multiple times with a razor blade during razor distribution.
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.
(S1) Inmate Dennis was placed against the chow cart that was next to us and informed him once again to stop resisting and that if he continued he would receive additional write-ups. I adjusted my escort grip and we continued to escort (S1) Inmate Dennis to intake. (S1) Inmate Dennis continued his verbal tirade of name calling and threatening to kill me. At approximately 1924 hours, myself and Officer Iwan B2792 departed Durango Jail with Inmate Clarke via AMR Ambulance unit 128 en route to MIHS located at 2601 E. Roosevelt St., Phoenix, AZ 85008. We arrived at MIHS at approximately 1942 hours.
On 11/07/17, I D/O B. Bailey #D1200 was assigned to the Medical Clinic as a floor officer. At approximately 0230 hours. Inmate Bowman, Torry BKN #1720190 refused medical treatment. As a result, he was given a refusal to sign so he could be returned to his floor. Inmate Bowman was given a pen so that he could sign the refusal. However, Inmate Bowman did not return the pen. Inmate Bowman was given several verbal commands to return the pen; however, he failed to comply. Lt. Steele was notified. Consequently, 8 male inmates in 3LB 23 were searched also a strip search was conducted. No pen was recovered.
On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
On 02/06/2017, at approximately 1715 hours, while I was conducting meal distribution and ID Headcount in Durango Jail, 3225 W. Gibson Lane, Phoenix, AZ 85009, Housing Unit D-3, Pod D. When Inmate Peterson, Jordan T324870, cell 57-2 received his meal tray, I noticed he had dried blood stains on his shirt, his left eye, left cheek and left corner of his lip were bruised and swollen. At that time I asked the inmate what happened and he replied, "It’s nothing." I told him to step out of the pod and have a seat in the multipurpose room of the housing unit.
This is a report of an AOA that occurred on 4/22/17 at Windmoor 11300 US Highway 19 N, Pinellas Park FL 33782. The complainant Suzanne Weaver stated a subject by the name of Jonathan Elder, left the facility through an emergency door and started heading towards US Highway 19 N. Sgt. Lantco made contact with the Elder at US 19/126th Ave N. Sgt. Lantco placed him in custody and brought him back to the facility. While walking with the subject into Unit 14, Elder became uncooperative and started to offer resistance by tensing up and pulling away. Elder verbally threatens officers at this time but continued to tense up not allowing himself to walk forward.
