On 1/04/2017 I email Jessica with a list of patients with BAD Address patients that she needs to updated on Ezclaim and on 1/6/2017 I email her one more time just for F/u on that and I'm still waiting for her respond me. I just check the patients one more time and the address still not change on Ezclaim. This is very important to be completed b/c I send statements for the second time and went to the bad address again. The new practice is that the Primary Care Team (PACT) is obligated to conduct a follow-up call on all patients three days after discharge from the hospital. The VA is a one-stop care model; therefore, PACT obtains a list of all veterans that were discharged in the last 24 hours. Also, the discharge unit scheduled a follow-up appointment with PACT for the veteran, preferably less than two weeks of discharge.
