On July 31st I received a notice of review of support. It has come to my attention that Mr. Daniel C. Buell is no longer employed at Clearwater Polk Electric Co. and is requesting this review. I’d like to give you a brief summary of Allison and my life since moving to Lincoln, Nebraska, in September of 2008. I started my employment with the state of Nebraska at the Lincoln Regional Center. The Regional Center is a mental health facility for criminals and people with mental health issues. Since my employment, due to unforeseen circumstances I had to move from third shift to first shift which caused me to lose a dollar an hour. At first, I just had health and dental insurance for Allison and I. I have since added vision, extra life, and long term disability to my deductibles. I have also upgraded the vision and dental. The reason why I did the extra life insurance was due to an incident where a doctor had been killed by a patient. The reason why I have the long term disability was because the probability of getting hurt at work is higher than at any other job I’ve had in the past. As of July 1st, 2015, I have received a .32 per hour raise, but since they no longer are offering the high deductible insurance I had, I needed to switch insurance policies. She is an excellent student. While attending Lincoln Public Schools, she has gotten awards for good character and she’s been on the enroll numerous times. While in middle school, she became involved in choir. Southwest also has a nice choir and she is on the show choir crew. Allison is a vegetarian so we need to buy her special foods for her diet. It is standard that when you begin working at a job with benefits, there is a ninety day waiting period for the insurance to become effective. By this time the employee had been employed for six months and one day his young son goes in for a check up and the child was diagnosed with leukemia. Prior to having a high deductible plan I had traditional personal choice insurance plan that I was able to get from my employer for my family of five for $240.00 a paycheck which was twenty six bi-weekly costing me $6,240 out of immediate salary with an eighty to twenty deductible. The first year my premium went down to $121.78 a biweekly paycheck. The high deductible was $2000. My employer paid $1000.00 and once deductible was met all costs under insurance were covered. Since then the premium went to $137.74 with a $3000.00 deductible. Currently my premium is $160.96 a bi weekly paycheck with a $4000.00 deductible and now an eighty/twenty deductible at office visits, prescription plans. There are allotted certain automatic fees for hospitalizations and high costs testing. A few preventative care costs are included for example, once a year well visits. If I pay into my HAS account I have started with fees as listed earlier then an ongoing fees of $5.00 a month to maintain it. In order to just cover the $4000.00 deductible I have to take out of my pay $153.85 bi weekly just to meet. In order to use advantage of tax free income the benefits of putting more into HAS do not outweigh other needs and costs. I have put in more than needed to meet deductible to help with copays. Overwhelmed by the accident and in the midst of recovery, medication and doctor visit's and treatment I lost track of some responsibilities. With the help of my supervisor Charles Tate, assisting me with communication and forms needed during the process, god bless his heart, documentation and forms were sent/received to and from my employer and the proper channels. An FMLA form was filed and approved, please see attachments, this is the first hospitalization in my personal life, also my first time out of work for injury/accident so this process is all new time. As a contractural employee I receive no compensation for the time documented or excused, caused by the incident under FMLA policy and thefore have had a financial hardship trying to keep up with all responsibilities over these few past months. I was under the assumption, as I did not receive Healthcare documentation, payment vouchers, nor monthly late payment reminder letters in the mail, as I have previously each month, and making all my payments online from my work desk, that my employer understanding that I was involved in an accident and approved for FMLA and receiving no payroll, I assumed that Healthcare benefits had been suspended from payment and covered under FMLA. In early 2014 I suffered a heart attack and I was only able to work 5 months in 2014 at reduced hours which seriously cut into my income. Moreover, once my heart sufficiently healed, I underwent extensive spinal surgery (to correct for an ongoing chronic situation) on February 18, 2015. I have now exhausted my company's short term disability and my EI Sick Benefits is set to end on October 3, 2015. Please keep in mind that my EI sick benefitsI equals $271 per week. At this juncture I am still several months from returning to work and regrettably even when I return to work I will only be working part time until most likely February, 2016 at the earliest, while I continue to heal.
