On 4/28/2016, CM met with the client to develop a Bi-Weekly ILP Review. In the meeting client was dressed in proper attire for the weather. Client appears to be cooperative and friendly. Based on CM observation client wasn’t truthful or forthcoming in providing information , when CM inquire about her family and work history. Client continues to report she speak Spanish fluently. Client affect was inappropriate. She was uncomfortable during the interview session. CM inquires the reason client was uncomfortable client replies “she is not uncomfortable.”

WORKER ASSESSMENT: Based on this worker observation client appears to have some mild cognitive impairment. Client during the session will loses her train of conversations and she eagerly will switch the conversation to something else; which is convenient for her. She Client has the ability to identify and complete goal in an timely fashion. She presents herself well, and she appears to have the ability to live independently or in a SRO.
Plan of action: Client must seek and secure source of income. Client must participate in free immigration services, Client must participate in all scheduled meetings, client must adhere to the shelter rule and regulation including 10pm curfew and client must attend to all on-site and off-site medical appointments. On 6/30/2016, CM met with the client to complete to Bi-Weekly ILP Review. In the meeting, client was dressed appropriately for the weather. She was very loquacious and client. Client appears to have difficulty sustaining attention, client does not seem to listen when spoken to directly and she is unable to follow through on tasks. Client affect is inappropriate and she denied suicidal or homicidal ideation.
The client met with counselor to complete his Exit plan. The client reported that he will be going to his mother house when he graduates the program. However the client wanted to go to sober living but all of them where full and had a waiting list. Moreover the client will be going with his counselor tomorrow for an assessment to see if he be placed in the King of Kings program.
I: CM guided client through ISP goals. CM inquired about updates related to the client’s housing goals. CM used open ended questions to inquired about the client most recent drug use. CM reminded client that starting Monday the 2nd he would have to meet with CCM for weekly case management going forward. CM continued to assess for PTSD symptoms, substance abuse, and medication compliance.
Session 1: During this face to face session on 8/8/16, MHS addressed the following ADLs: Medication Monitoring, Community Involvement, and Functional Skills. MHS asked the client if he had taken his medication today. The client reported that he did take his medication. The client mood was cheerful and inviting. The stated he is still looking forward to working when he can. The client stated he gets bored sometimes just sitting at home. The client requested to be accompanied to the library. MHS praised the client in making progress for independent living (20 mins). During meeting Ms. Declaire stated that they have not apply for NYCHA. Client also stated that family has never been evicted from any apartment because family has not own their own apartment. As per review and discussing the case with Ms. Declaire they are not eligible for any program at this point due to the lack of income. The only chance that family may have is LINC, client was instructed to provide last four pay stubs to inquire via CMFT for the LINC program. Currently, the modification of the bill proposes to extend the maximum stay in BCG shelter from 21 days to 30 days. The amendment will also provide suicide prevention services, dual counseling services for mental health and substance abuse counseling for victims of human trafficking and sexual exploitation. It offers services for parents or legal guardians for family reunification with their children (114th Congress, 2016). The modification proposes to revise the application form for the shelter’s intake of clients; adding the youth’s age, gender, and will assist young people with filling out a Free Application for Federal Student Aid. Housing Update: client NY NY I, II was approved. Client is waiting for DHS manifest to tour apartment. Another alternative housing is MRT once client SSI is approved. Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice.
On 01/28/2016 Ms. Soraya Johnson came to the SSO to meet with CM Watterson for an ILP meeting. HS De La Torre inquired to Ms. Johnson to stay a little longer to develop a Housing Plan. Ms. Johnson agreed and the following was discussed during Housing Plan.
Social Service Meeting: On 02/28/2017 Ms. Williams met with her assigned Case Manager for the family weekly ILP Document Review. Ms. Williams’ next ILP Document Review appointment is on 02/07/2017. Case Manager asked Ms. Hair if she is aware that she is not in-compliance with the terms of her ILP, Ms. Hair stated yes. Case Manager asked Ms. Hair if is aware that failed to attend her ILP meetings since 01/23/2017, Ms. Hair stated that she was aware. Ms. Hair last ILP was conducted on 01/23/2017. On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
