On 04-20-17 at approximately 0400 hours, Zhang telephoned the police station regarding her vehicle. Zhang advised PPD Dispatch that she recalled parking her vehicle on Del Mar Boulevard just south of her residence. Zhang walked to Del Mar Boulevard and located her vehicle. The vehicle was entered into NCIC as being recovered (see attached printout).
On Sunday, 04/29/2018, at approximately 1024 hours, I was in full police uniform, driving a marked Daly City Police patrol vehicle. I was driving south on Sullivan Avenue, north of the interstate 280 south on ramp. At that time my attention was drawn to a gray, 2015, Volkswagen, Passat bearing California license #5TUF434. I saw the vehicle was occupied by a male driver and female passenger. I conducted a records check on the computer in my patrol vehicle and it revealed it was a stolen vehicle out of Oakland.
On 07/15/2015, at approximately 2305 hours, your Affiant was on routine patrol on Main Street (SR924) when I observed a silver Chrysler sedan parked in the One Hundred Block of South Main Street (westside of the street) near the intersection of Oak Street. The vehicle displayed a Pennsylvania registration of JRJ-5342. The registration was checked utilizing the Pennsylvania Department of Transportation vehicle record abstract which revealed that the license plate was assigned by the department to KYLE ROBERT & SARAH LYNN FOGARTY for a 1998 Chevrolet. I then verified the status of KYLE and SARAH FOGARTY'S driving licenses both of which were suspended by the Department of Transportation.
On 12-05-17 at approximately 1017 hours, I responded to 442 N. Los Robles Avenue regarding a psychological evaluation. PPD Dispatch advised that a female, White was yelling incoherently at the above location. Officer Gawlik #3116, Officer Herrera #3595 and Officer Killgore #7231 were on scene prior to my arrival.
On Thursday, June 23rd, 2016 at approximately 2315 hrs while driving a Midland Police vehicle and wearing a standard issued MPD uniform Officer Jimenez was dispatched to 4201 Harlowe Dr. in reference to a Burglary of a Habitation. MPD Dispatch advised Officer Jimenez that complainant wanted to report that someone broke into his house and took several electronic items without his effective consent.
On 02-04-16 at approximately 1030 hours, Phillips drove “Brandon” to the Department of Public Social Services office located at 980 N. Lake Avenue in Pasadena. “Brandon” wanted to pick up a cellular telephone through the Lifeline Telephone Program. “Brandon” returned with the cellular telephone and asked Phillips if she would drive him to his mother’s residence. Phillips agreed and drove “Brandon” to 1518 N. Fair Oaks Avenue.
Sixteen-year-old Jordan Matthew Simonds died Saturday, March 11 when the 2004 Jeep Liberty he was driving careened off an embankment on Salem Church Road.
On Wednesday, March 30th, 2016 at approximately 2056 hours while driving a marked Midland Police vehicle and wearing a standard MPD issued uniform Officer Jimenez was dispatched to 5266 W Loop 250 N #905 A(Newport Apartments) in reference to a disturbance.
On March 11, 2016, I responded to 10819 Grambil Road Manassas, Virginia to speak to representatives of TGM Sudley Crossing. I spoke to the Leasing Manager Bobbie Buday who immediately identified the photo of Sarafina Khatman. Ms. Buday told me she had spoken to Detective Betty Sixsmith about Sarafina and the issues caused while she lived in their property.

Anyone who has driven on a San Antonio road knows how unpredictable drivers can be. The Impatients, and aggravation sometimes get the best of a driver. For a San Antonio police officer accidents are nothing short of ordinary. There are however a small few that stand out above the others. This hot Saturday in July was busier than usual. With months without rain, the roads were extremely dry. The day was relatively busy, but there were no major incidents. That is until four o'clock when calls started flowing in for a major accident. Officers thought it couldn’t have been anything too massive considering the light traffic and ideal road conditions. This accident however was far from normal. This strange wreck was something you would only see in the movies.
Michigan's second diverging diamond intersection is located at the I-96 interchange with Cascade Road from approximately Cascade West Parkway to 400-feet east of Kenmoor Avenue in Grand Rapids Township, Michigan. The project length on Cascade Road is approximately 2700 feet. As a sub consultant to TranSystems Corporation, C2AE was responsible for the design of Cascade Road's horizontal and vertical alignment, intersection improvements at Cascade West Parkway, Ramp Terminals, and East Paris Avenue, maintenance of traffic and staging for Cascade Road and I-96, utility coordination, drainage systems, guardrail and roadway safety, sidewalk and ADA ramps.
R/Os were instructed by dispatch to respond to the listed address in reference to a disturbance. While en route, dispatch informed that the suspects had left the residence in vehicles described as "Hyundai" and "Nissan". R/Os drove around the area looking for the vehicles meeting negative results. R/Os went back to service with no further
The facility has a potential growth by acquiring IT systems. As mentioned, the major challenges are staffing, technology, and limited financial resources which is expected since the merger Suburban’s administration never performed any improvements or major capital investment. In my opinion, the selling of the facility demonstrate failure but also without the acquisition of technology the hospital could not expand their capabilities neither improve their services.
