Women, Infants, and Children (WIC) Nutritional Services is a federal program that provides health care and proper nutrition to low income pregnant, postpartum, and breastfeeding women during a crucial time for both mothers and babies. The mission of WIC is to “safeguard the health of low-income women, infants, and children up to age 5 who are at nutrition risk by providing nutritious foods to supplement diets, information on healthy eating, and referrals to health care.” (Food and Nutrition Service, 2010).

Horizontal violence is a negative phenomenon that is increasing significantly in the hospital setting. It is defined as bullying, verbal and physical aggression that occurs to employees in the workforce. Horizontal violence has harmful effects on nurses as it lowers their self- esteem, and makes them feel as if they have no power in their career. This phenomenon also negatively impacts patient centered care and safety as nurses are more vulnerable to making medication errors and careless mistakes. Infant mortality rate constitutes the death of a baby before their first birthday. Mortality rates around the world differ tremendously with America leading the first world countries at an alarming rate of 6.1 deaths per 1,000 births. Conversely, Finland and Japan secure the last, most desirable position, with deaths totaling 2.3 per 1,000 births, as of 2010. (Ovaska-Few, 2015) In 2014, over 23,000 babies died in the United States. (CDC, 2016) Exploring the mortality rates in America brings light to a dire need for additional interventions and research as to why this developing nation has the highest rate of infant deaths before the age of 1 year old. African Americans face the worst outcomes of infant death compared to whites, Latino, and their Native American peers in North Carolina. (Ovaska-Few, 2015) This paper will explore why African American are the leading race for infant mortality and the steps that health communities need to take to address this devastating occurrence.

“Compared with European Americans, African American infants experience disproportionately high rates of low birth weight (LBW) and preterm delivery and are more than twice as likely to die during their 1st year of life”(Giscombé, C. L., & Lobel, M., 2005). The infant mortality rate for African Americans is 13.7 deaths per 1000 live births, more than twice the rate (5.7) for White Americans in the U.S. (Kung, Hoyert, Xu, & Murphy, 2008). A lot of the racial disparity in infant mortality can be explained by low birthweight and preterm delivery, which are also disproportionately and often experienced by African Americans (Martin et al., 2007).

Those who cannot afford the high prices of housing are often forced out into the streets where they face a very uncertain future due to the number of abuses they encounter daily from all walks of life, with the most damning being the vagrancy laws that come into vogue in areas that are getting gentrified, which many cities have passed to “protect” their newfound assets and tax base from the “lowering” of property values. Furthermore, when cities such as Los Angeles demand that property developers set aside affordable housing for lower income people, they get sued in court, such as in 2009, when real estate developer Geoffrey Palmer successfully sued in order to overturned an ordnance which required that. This was also the same man who also proposed building a footbridge connecting two of his buildings to minimize contact with people he deemed undesirable (Davis).

It should be noted that these services vary according to population needs, with some studies even indicating that services vary based on the racial background of the participants (Evans, Labbok, & Abrahams 2011). This should be a cause for concern, and discrepancies in services provided must be avoided to limit further disparities. Data from the Centers for Disease Control (2016) found that in 2010, the rates for breastfeeding initiation was 74% for Native American women, which was lower than other groups. In order to better combat breastfeeding disparities, the WIC program continues to tailor their breastfeeding support services to achieve the Healthy People 2020 goals by increasing the number of trained staff, holding community involvement sessions, and constructing additional educational components.

Those of us who chose nursing as our profession generally care for others and want to make a difference in the world. New nurses are excited to become licensed and to start working in the profession of nursing. When new nurses enter the field of nursing, they are often met with more experienced nurses who have practiced longer and subsequently has more experience. This maybe an opportunity for the new graduate to learn and grow or it can lead to horizontal violence in the work place. Major disparities continue to exist in spite of America’s “Affordable Care Act”, Although minorities suffer disease five times greater than the rest of the nation, minorities represent the majority of the disparity. Infant Mortality has always been America’s indicator as to the state of the countries health status. America is one of the wealthiest and most powerful nations in the world, However, America ranks number 24th in the world for infant mortality. Puerto Rican and Indian women rank highest among ethnic classes with low infant mortality rates. Some researchers have found lack of prenatal access as the key contributing factor to low mortality and birth weights among these two ethnic classes.

Premature birth is an important public health priority in terms of health of women and infants. Every year an estimated 15 million preterm babies are born and this number is still rising (WHO, 2015).In 2014, 1 of every 10 babies born in United States were premature and black infants were 50% more likely to be born premature than white, Hispanic and Asian/Pacific islander infants (CDC,2015). Almost 1 million children die each year due to complications of premature birth (WHO, 2015). Major survivors face lifelong disabilities like learning disabilities, hearing, visual, feeding, digestive, breathing and respiratory problems (CDC, 2015) and low birth weight (March of Dimes, 2014). The books Child of the Dark by Carolina Maria de Jesus and Testimony by Victor Montejo describes the lives of two individuals from different societies. In both of these societies there was much hardship and violence. The two main characters who wrote these books describe life through their point of view and explains the hardship and challenges they had living in a society filled with violence.

Structural violence is the way in which a social structure will harm people by not providing, by limiting or by barring people from receiving basic needs. Structural violence impacts people on the bottom rung of society. People who live in poverty or are not considered being of a high social standing. This could be because of a person’s age, sex, ethnicity, race, sexual orientation, or any other aspect that makes them different from the majority of the population or different from what people consider to be the norm. However, structural violence does not necessarily need the majority of the population to define it, it can manifest by a few people in the country or out making decisions for the populace.
