This enables professionals to provide holistic support by giving the individials the right to make their own choices reguarding all aspects of their care. Therefore, the individual will be able to tell members of staff if they are unhappy about any part of the support they are receiving. This will then be investigated and an appropriate measure will be taken to make sure that the individual will be happy and cared for appropriately. The individual will be able to voice when they are happy and when they are not happy, they will also be able to voice their own opinions and preferences which will be taken into account when their care is being performed. The individial's care, wellbeing and preferences will always come first. For example, in a care
It is important to review care and support plans as people’s needs change. By including the person, their family then everyone knows what is happening and the family can help to monitor mood swings and behaviour. The individual and the family can express their views and preferences and any relevant risk assessments may be done with everyone involved. By monitoring the individual, a decision can be made as to whether the changes are effective and if the best care is being given to encourage independence and promote dignity.
with individuals - It is about finding out what is important to the person and helping them to turn their choices into reality. By listening and working with the person and their families in partnership to help this happen. It is part of your role to support the person to make their own choices and to ensure that these choices are heard and recorded to form the basis of their care plan. You should then support
As a support worker, it is imperative that we look at all aspects of health and social care with a holistic approach when planning and provision is concerned. Holistic means looking at a situation as a whole and not just part of it.
2) Describe two ways of making sure that the history, preferences, wishes and needs of an individual using the service are recognised in support or care plane.
Good communication with the individual should be enhanced. Both individual and carer must compromise and negotiate to what would benefit most for the individual as long as it is safe. Thorough information should be given and must acknowledge the benefits of their choices. This is a way of recognising rights and choices of the individual. One example is the resident’s choice not to use his/her walking frame. This is one conflict of decision – making. Decision should be tailored to the needs of the resident
2.2. Describe how to take into account the history, preferences, wishes and needs of an individual when planning care and support.

circumstances can be taken into account when planning care that will empower individuals. I will use a variety of examples from health and social care. I will also extend these examples by assessing the potential difficulties in taking individual circumstances into account when planning care that will empower an individual, and make suggestions for improvement.
It is important to identify individual’s specific and unique needs so that they can receive the best care possible. Taking a person centred approach is vital, treating a service user as an individual person and acknowledge that
But this approach can also be negative and this is because someone can feel that they are having a lot of pressure put on them if they have to make a decision and it could lead to them getting stressed out and sometimes people might refuse to make a decisions if care professionals are expecting a decision from the person being asked. But as I have said before everyone takes things differently. This would be applied as well if Frieda was getting care from careers, Frieda could experience the positive effects or she could experience the negative effects depending on how she feels after the fall. Another way to support the independence of a person it let them have control of the things they can in their lives. So this can be from letting someone control their money, letting someone choose what they want to buy when they do food shopping. Another way to support independence is to maintain the person’s dignity, so this can be done like if someone wants to a care professional to go to the toilet with them the care professional could go with them but the maintain their dignity they might not look at the person while they are using the toilet unless they need any help. This approach can be effective because it gives people a sense of control and it gives a person a sense of having control of their life and their ageing. If someone does feel that they are in control of their life they might cooperate with care professionals more and they might even become more involved
. 1) Provide the health and related support services that foster independence, for the client as well as the family. 2) Achieve cost effectiveness by maximizing the use of resources, and 3) enhance quality through appropriateness and continuity of care. Some clients may only use only select components of the system and may remain involved with the organized system of care for a relatively short period of time; others may use only a limited and stable set of services over a prolonged period of time (Williams, 2008).
It also ensures people to have positive experience of care. It reduces the amount of patient readmitted to the hospital or coming back to the hospital for treatment. It helps people to take control of their illness e.g. diabetes, stroke, etc., to live independently for longer period of time REF
“Self-care agency to meet individual’s needs for self-care action in order to sustain life and health, recover from disease or injury, and cope with effects”(Meleis, 2012)
A holistic care approach was taken while caring for Philip, taking care of the whole body and all of his individual needs. This places Philip in the middle of his care and he was assessed by a healthcare team to identify his strengths and capabilities, the client and the care providers must work side by side to make sure the client receives the best possible care that is most suited to him. Philip needs to be reassured that he will be cared for in a safe and caring environment and that he will be protected by all means. Respect must be withheld at all times and we must not be judgemental of his way of life or his beliefs, Philip’s dignity must be preserved at all times, and comfort plays an important role in his care.
