The worker met with Ms. Yara Richardson, Emerson Batista, and Ursula Richardson at the Thelma Scott's home, maternal aunt. This home is located at 139 16th street Paterson New Jersey. The purpose of the meeting was to engage Ms. Richardson in having a Family Team Meeting, to discuss Ms. Richardson positive urine screen and sporadic attendance with options counseling. Richardson what were the barriers that prevent her from attending Family Center. She shared that Mr. Pugh her paramour took her car to North Carolina. She stated that she does not have transportation services. She stated before he took her car. Ms. Richardson's car was giving her problems so she could not take them. Ms. Richardson stated that then the boys were at summer camp for two weeks. The worker asked Ms. Richardson if would like for the worker to referred her family to receive counseling in the home. She stated that she is willing to do what ever the Division offers. She than shared that FPS referred her to FIS. She stated that the lady named is Tiana and she is an intervention specialist. She stated that she meet with her 9/29/2016. She stated that they were involved to help her with Ruddy. The worker told Ms. Richardson that she would speak to Tiana to get more information as to what services she will provide to avoid duplication of services regarding the possible referral for in-home services. UCM: CPSW did a home visit to Ms. Brenda Borkovec's home. Both children were in the living room playing together. Also, Ms. Brenda was in the living room holding Aydden and feeding him. Ms. Brenda stated that she has not received any papers regarding her foster care license. Ms. Brenda stated that she is frustrated with the process and that it has taken a longer time. Ms. Brenda mentioned that it has taken a long time for her to complete her fingerprint. She reported that currently she is waiting the background study and fingerprint forms to be sent to her. Ms. Borkovec's phone ring and it was Elizabeth Borkovec. Ms. Brenda asked CPSW if Elizabeth can come and join the meeting. CPSW approved it and Elizabeth Borkovec showed up less than 5 minutes.
On this date worker visited Shadescrest NH, for the purpose of monitoring Ms. Salter's current placement and delivering Christmas presents. When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
Action: MHP and MHS discuss Donovan’s behaviors in the home and community. MHP and MHS review monthly paperwork that is needs to be submitted. MHS and MHP complete a walk-through of Donovan’s bedroom and play room. MHP and MHP discuss upcoming medical appointments. UCM: CPSW received a call from Ms. Messerli reporting that she needs the lock number for her storage from foster provider family. She also stated that she needs the boy's cell phone back. CPSW reminded Ms. Messerli that this writer will ask the foster provider about the phone and storage numbers. CPSW reminded Ms. Messerli to focus on her recovery and not to disrupt her children as well as the foster provider and if she needs anyting to let the CPSW aware of it. Also, CPSW reminded Ms. Messerli not to contact or call both children until the theripist approves it. Ms. Messerli stated that she will try. Howerver, Ms Messerli sounded unhappy and really upset about not contacting her children Nick and Alexander.
The worker meet with the above participants on the above date. Present at the meeting was Ruddy Batista, Emerson Batista and Yara Richardson. The therapist from FIS was at the home. The purpose of the visit was to speak to Ms. Richardson regarding her recent incarceration her relationship with Getwan Pugh and his whereabouts, and her being discharged from Options Counseling and FPS. Also the worker wanted to discuss with Ms. Richardson her budget and the household members who resided in the home. Lastly, the worker wanted to speak with Emerson and Ruddy and allowed them to asked for worker any questions regarding their case.
The worker contacted Janice Nicolas; who is a friend of Kerri Gorman. Mrs. Nicolas stated “Kerri is a good mother, she has a happy family and all of Kerri’s children are very smart and well mannered.” Mrs. Nicolas stated “Kerri does go to the methadone clinic for opiate dependency. Mrs. Nicolas stated that Kerri told her she became dependent on pain killer after she was prescribed them after a car wreck she was involved in.” Mrs. Nicolas stated she had no safety concerns for the children living in the home with their mother and she last same all the children yesterday morning at her house during breakfast and all the kids were doing
Stephanie was referred to FACT for CMO services by Trinitas Hospital – CCIS unit. Stephanie was admitted to CCIS on 4/18/17 for running away from her mother’s home and making threats to hurt self if she were to return to reside with her mother. While in the hospital, Stephanie has also been refusing to visit with mother. At the time of the FCP meeting, Stephanie (Youth) resides with her Natalia Ramos (Mother), Elsa Sanchez (Maternal grandmother) and Johnathan Sanchez (Uncle) who disabled. Mother and stepfather recently divorced. Her father Hamilton Ramirez (biological Father) lives in Elizabeth with his girlfriend and younger daughter. Each biological parent has children with other people. Parents do not get along at all. Youth wants to live with father.
Intervention: Ms. Smalls (MHP), Mrs. Clark (MHS) and Taliana discussed Taliana adjusting to the home. MHP encouraged Taliana to ask questions if she does not understand something. MHP praised for willing to communicate with MHS and family. MHP, MHS and Taliana discussed Taliana’s emotions about family visits and no contact with biological mother. MHP praised Taliana expresses her feelings. MHP explained to Taliana and MHS that the DSS case worker will visit next week. MHP provide an update from DSS worker about clothing and eyeglasses. MHP administrated the Casey Life skills. MHP allowed Taliana to share stories about her biological mother and family.
An interprofessional team consisting of Medical student, Pharmacist student, Nursing student and social work student met with Patient (A.O.) to perform a collaborative family assessment in a home visit. A.O. went to foster care for two months, but his father gained custody in July 2012. The father mentioned the foster care had an effect on A.O. A.O. lives with his father and his grandmother. The father works at a thrift store. A.O.’s Mother has Neurofibromatosis Type I too. The father mentioned that A.O. had a speech delay. A.J.’s father mentioned that they received assistance from the government. A.O.’s grandmother and father reported that he eats a good variety of food as he eats fruits and vegetables. A.O. reported liking pizza and McDonalds
This worker met Ms. January Hollins in the children hospital. Privacy practices and Tennessen notices were reviewed and signed. The interview was recorded.
Working with children, young people and their families can be seen as difficult and complex for many different reasons. However, before these topics can be discussed and explored fully, it is important to completely understand the difference between the words ‘difficult’ and ‘complex’. ‘Difficult’ implies that things are not easily or readily done, where it requires much effort much effort, labour and skill to be performed effectively. In the same way, ‘complex’ implies a hard to deal with situation, but also involves many interconnected parts or complicated arrangements that consist of emotional attachment.
I selected my Family Collaborative Project as an artifact that represents my competency to assess the ongoing learning of children (including those with disabilities) birth-to-three in a collaborative manner with paraprofessionals and families. I used two different screening tools based on a systematic approach of assessment: Developmental Screen Ages & Stages and the Assessment, Evaluation and Programming System (AEPS). The Family Collaborative Project encompassed my ability to collect screening data, assess and record children’s on-going development, use developmentally appropriate learning outcomes that align with Kentucky Early Childhood Standards. Further the results of the assessment provided me the opportunity to work as a team with families, paraprofessionals and teachers to design learning outcomes and plan future services and instruction.
The worker contacted Harriet Belfiore who is the mother of Ralph Vernon. Mrs. Belfiore stated “I am very concerned about the safety of my grandson 4y/o Ralph. His parents Lisa and Ralph sr. are currently homeless living “pillar to post”. I helped Ralph SR. and Lisa find a transitional program in OKC because they were struggling here in New Jersey and in Texas. They were doing well for at time but then Ralph was extradited back to Texas and had to do 8 months in prison for a felony charge. During the time Ralph was imprisoned Lisa had gotten her section 8 and was working as a paralegal and the family was stable. But soon after Ralph release from prison he started acting erratically not wanting to leave the home afraid he would be arrested again
