I am writing this statement because I want to share the events that took place during a recent unannounced inspection conducted by Dr. Munoz on May 18, 2018 at American pediatric Sedation Center. Dr. Munoz made some serious allegations about the safety of our patients. His remarks were authoritative and an act of psychological intimidation and threatening to gain compliance. I was notified by the front desk staff of Dr. Munoz’s arrival, and he stated that he was going to be conducting an unannounced inspection. He did not provide any further details except that he was sent by the Board of Dentist to proceed with this inspection. I asked him to wait in the lobby and that I would return shortly. He quickly became upset and told me to go outside the office. I followed him out because he started to make a scene in the lobby and I had patients and parents waiting. He stated, “I’m here to make sure that the children are safe,” and questioning the level of health and safety that takes place in our office and asking whether or not I was a parent. I started to feel uncomfortable and felt intimidated by his change of tone and the fact that he told me to go outside where there were no witnesses and I wasn’t sure what his intentions were. He also said that the reason I was delaying his inspection was due to, “us covering up something.” The allegations that he was making made me extremely uncomfortable. I repeatedly asked him to wait patiently and that I was verifying some information before letting him. When we went back inside the office, Dr. Munoz began questioning Dr. Liou’s right to practice in the sedation center. Instead of asking for documentation, he pulled out his cell phone to show me on what appeared to be the Floridahealth.gov website, that Dr. Liou did not have the sedation center office as a practicing location. Shortly after, an investigator by the name of Luis Inoa from the Department of Health arrived to join Dr. Munoz. I did not understand why Dr. Munoz contacted the investigator to participate in the investigation nor did he clarify the purpose of his visit. I asked Mr. Inoa to explain the reason for his visit and he stated that he came to drop off some paperwork for Dr. Huda. At the moment I advised Mr. Inoa that Dr. Huda was busy with patients and not able to see him. Mr. Inoa, asked if Dr. Huda could give me permission to receive the document, but Dr. Huda was busy monitoring his patient. At that moment, Dr. Munoz stated that if we don’t provide him with patient records that he will make a phone call that can immediately shut down our operations. At that point, he was threatening to shut down our office therefore, I asked Dr. Huda for permission. Upon arrival, I observed a (UAMS) registered nurse and other people standing around this patient. Mr. Anthony Niccum who is an employee of (UAMS) as Valet, approached me and advised me that he called dispatch. I asked him what happened. Mr. Miccum advised me that all he saw was the patient on the ground bleeding from the nose, so he contacted the (UAMS) Police Department. On Friday December 26, 2015 at approximately 0133 hours, FHEO Security Officers were dispatched to the Emergency Department (E.D.) for a Patient Standby (51S) room #26 same patient which we had a previous call earlier in regards of Found Contraband. Security Officers Carlos, Omar, Johnson and Security Supervisor Evans responded to the call. On August 28, 2015 at approximately 0325 hours, Security Operations Center (SOC) radioed East Orlando Officers regarding a patient assist in the Emergency Department room 30. Upon, arrival I met with Nurse Jon Jose who stated that patient, Heather Locklear (FIN: 70596879) was discharged but refusing to leave. According to nursing staff patient wanted to stay in the room till 0600 hours. I went inside the room and speak to the patient. She stated that she was waiting for a bus pass. Security Officers Omar Alonso, Brandon Rodriguez and I, Steven Evans responded. Upon arrival, Registered Nurse Brenda Long informed us that the mother of patient, Rafiq Veerani (MRN: 70898563/ FIN: 84825061) refused to leave the room. The patient’s mother insisted on sleeping over despite nursing staff request to leave made four hours prior. A patient arrives at the doctor office looking to find a solution for the symptoms they have been experiencing. However, one they are called to an operatory the doctor sends in a medical student to receive current and past medical history and wrote it on a post it note. Not to mention the doctor only stayed in the room a few minutes. Then, informed the patient they would receive a call once he/she corresponded with their previous doctor, but the patient never received a call and they called and told the doctor would contact them once off of vacation. So lastly she wrote a letter to the office expressing concern yet again requesting a call that she never received.
In the notification Patient states that during her visit with the provider to get a referral for pain Management, that she was allegedly kicked out of the office. Patient also states that the Doctor would not refill her medication that has been prescribed to her since December, year not noted in the complaint.
