On August 28, 2015 at approximately 0325 hours, Security Operations Center (SOC) radioed East Orlando Officers regarding a patient assist in the Emergency Department room 30. Upon, arrival I met with Nurse Jon Jose who stated that patient, Heather Locklear (FIN: 70596879) was discharged but refusing to leave. According to nursing staff patient wanted to stay in the room till 0600 hours. I went inside the room and speak to the patient. She stated that she was waiting for a bus pass. Upon receiving the bus pass from nursing staff, patient agreed to leave without incident. At approximately 0330 hours I escorted patient outside the building where Officer Lourdes and I observed patient left premises without incident. Security did not have to go
On Thursday 11/12/2015 at 1905 hours, during shift briefing, Officers Jason Peterman and William Miller pass down information concerning what was believed to have been an elopement from CDU by a male Baker Act patient. He was reported missing by Registered Nurse Ophie to Doctor Gomez at approximately 1530 hours. Doctor Gomez who was concerned for the patient’s safety and wellbeing then contacted Primerose Vernet from the Florida Hospital Risk Management Department assigned to our facility and she in turn contacted the Orange County Sheriff's Office and reported the issue.
On May 15, 2016 at approximately 2335 hours Security Officers Steven Evans and Omar Alonso were in the E.D. Security Office when we heard someone yell for help inside of the E.D. entrance doors in regards of a “Gunshot Wound” patient. Out of the Office, we observed a Hispanic male hoping towards the PFS counter with blood on his left abdominal area. Security Supvr. Evans grabbed a wheelchair and wheeled him to one of the E.D. triage room, while his friend, Joseph Park (witness) register him with PFS. Security team in coordination with E.D. Charge Nurse Robbie Phillips placed the Emergency Department in a lockdown, Security Officers responded and kept watch over both E.D. entrances. Upon arrival, I had initially provided support in standing by while the Narcotics team went through the house getting everyone out. I then was asked to assist in search the two female subjects that were detained. While searching the females I went back to my vehicle to grab another set of gloves. While I was doing this, I had seen Officer Fredrick #1111 had one of the subjects identified as Keith Gray W/M (DOB: 07/09/1958) on the ground. Medic 1 was dispatched by 911 to 1234 Main St for a possible code blue for a 5 year old Male. Fire Dept was on scene as this was the location of a structure fire. Along with Fire Dept the local first responders was on scene. The first responders arrived on scene and requested ALS from dispatch. Medic 1 arrived on scene to find a child lying supine across the street from where the fire was located. There was four to five fire firefighters located around the patient assisting first responders. As Medic 1 arrived on scene was was given a history on the patient from first responders. Mr. Jones stated, he had never seen the individual before and was unable to provide an accurate physical description. At 0650 hours Orange County Deputy Stephen Williams (Badge #25569) arrived on site. D/S Williams conducted an initial investigation with the Stab Wounds Victim and after questioning, he determined there was no immediate threat to Florida Hospital staff. He then contacted Security Supervisor Steven Evans and concluded the lockdown could be lifted. Supervisor Evans then radioed Dispatch and the Emergency Lockdown was lifted at 0705 hours at which time both Supervisor Nurse Administrator and Security Manager King were notified. Security stood by and was informed D/S Williams that the incident would be turned over to Orlando Police Department (OPD) which has jurisdiction over the incident. The reporting party (RP) stated resident Sofia Miramontes age 27 DOB: 11/23/88 has developmental delays, deaf in right ear and is bipolar. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The Officer informed the Director of agency Sherry Winston that the resident was coherent and walking to her boyfriend’s home. The RP contacted the facility and spoke with a staff
On Thursday 07/21/2016 at approximately 2223 hours, Security Supervisor Steven Evans was contacted by Assistant Nurse Manager Robbie Philips via landline and asked to conduct a (44V) Enforcement Escort Visitor Off Property for the discharged female patient in E.D. #48. The patient, Susan Harris (DOB: 03/22/1952), had been quarrelsome and refusing to leave. SOs Christopher Paz and Ariel Weiland responded to the scene. Upon arrival, we observed the patient laying down on her bed, we approached the discharged patient and spoke with her. Mrs. Harris agreed to leave without further incident. On Thursday 11/17/2016 around 2300 hours, I, Security Steven Evans was informed by Dispatch to make contact with 5 East Unit Charge Nurse Tonya Smith in regards of Code (34) Customer Service Assist. I arrived at 2302 hours and met with Charge Tonya Smith who explained to me that the Correction Officers for the prisoner in room # 504 had given her hard time few Months ago on the patient, Angel SantiagoGonzalez (Fin# 86424089) last visit. Nurse Smith further stated that she had a personal problems with one of the Officer and that she was informed by Lake Correctional Institution that this Officer would not be working at all on 5 East. On 10/14/2015 SO EMT Perez was dispatched to HS-406 to apply a bandage. SO EMT Perez knocked and anounced his presence at the door and was verbally invited in by the resident. The resident, a Mrs.Martha Doyle was sitting in her living room chair and was bleeding from the forehead. Mrs. Martha Doyle was washing her face when she attempted to remove a bandage over a previous wound and she wanted it re-bandaged as she stated. SO EMT Perez asked Mrs. Martha Doyle if she wished to go to hospital before he assesed her to which she denied. Mrs.Martha Doyle also denied any care besides the application of a new bandage. At approximately 1040 hours, on 27-Jul-15, site security was dispatched by POC to attend Acute 3 for Domestic Dispute. The writer, S/O P.Jo, S/O A.Toor and S/O M.Moradi responded to the call and upon arrival the officers met with one of the clinical staff members, who informed the officers that a lady walked into the unit with two kids, one of the kids was about five year old and the other one was in stroller. The lady told the staff member that someone has been following her for three blocks and they need to call RCMP. After telling this the lady walked into the washroom with her kids, the clinical staffs ran after her and asked her to come out, but she didn’t listen and slammed the door on her face.
On 7/6/2015 S/O EMT Perez was dispatched to WC-407 regaurding a pendant alarm. S/O EMT Perez knocked and annoounced his presence at the door, when there was no answer S/O EMT Perez proceeded to search the apartment. S/O EMT Perez found the resident, a Mrs. Mary Beiter toppled over her walker by her bedside. Mrs. Mary Beiter was trying to get inter her bed when she lost balance and fell and could not get up under her own strength. Mrs.Beiter did not complain of any hip pain during the pelvic exam but did state that she has a bad knee and hip that makes it rather hard to stand on her own. On 10/05/2016, I, Jeffrey Lee Albert, was working as a police officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately 1614 hours I was dispatched to Grace Wilke to offer a ride for a student Eryn Noelle Miller to Comcair Crisis Center at 635 N Main, Wichita, KS 67203. I arrived at approximately 1627 hours at Grace Wilke and met with Amy Marie Barfield on the thrid floor. Barfield introduced me to Miller and I started my transport at approximately 1644 hours. On December 23, 2016 at approximately 2127 hours Florida Hospital East Security Officers were dispatched to the E.D. D-POD in reference to a 34 - CUSTOMER SERVICE ASSIST. Security Supervisor Steven Evans and Officer Miguel Pagan responded and upon arrival, we met with RN Jim Mickel. Jim stated he had $575.86 belonging to patient Elizabeth Sullivan and wanted the money secured in a safer location. Money was counted by Jim and Pagan and witnessed by Evans.
