This is 34 year old WF. Patient is a resident at the Lovelady Center. Patient stataes she had a baby 7 weeks ago. She is depressed and misses her bacy. She is to stay at the love lady for 90 days, but possibly for 9 to 12 months. The bay is well taking care of by her boyfriend (the baby's father). Patient denies thoughts of suicide or homicide. Wants to get through the program and get back to her baby. Patient denies chest pain, SOB, N/V/ D, or fever. Patient reports left knee pain history of MVA in 2009 and injured left knee. Patient also reports tooth ache 4/10.
At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
The patient is a 56 year old female who has recently lost her job and has been financially struggling. She has isolated herself from friends and family members. She claims to be close to her older sister but
Since I have last seen the patient, she tells me she has been to see [Mary Bluen at Orchard Park in Portsmouth.] She has been seeing her in counseling now for the last six to seven weeks and her next appointment is tomorrow. She says she has been struggling lately with her feelings. She established a relationship with a superior at work who is married and she herself is married. She says this is something that she is trying to work through to figure out why she is making these choices. Her plan is to ultimately end her marriage. She is not sure if she will continue to see this male at work again or not. She is just not sure what her choices are. She said on top of all of this, she just found out today that she has been given a 90 day noticed, so she is not sure if she will even be able to keep the same job and she is fairly certain that this is follow
His patient is young 21-year-old girl. She has three children from three different fathers. The first father impregnated her at the age of 16. He openly cheated on her and would beat her during his drunken rages. She left him and met her second child’s father. This man was an addict with a criminal history. He died in an accident while driving a stolen car. The father of her third told her to have his child or he would leave her. She lived with him and had his child. A week after giving birth he left her. She went to Dalrymple claiming to be depressed.
12/24/15 Progress Report describes that the patient has right knee pain. The pain is frequent. It is aching and burning in quality. The current pain level is 0/10 and worst pain is 4/10. Bending, squatting, walking, weight bearing, changing clothes and ROM aggravate the pain. Rest, ice,
Dr. Justin Clayton at the Mercy Clinic Orthopedic Surgery evaluated the claimant on April 6, 2018. Dr. Clayton stated the claimant may have rheumatoid disease and prescribe him Meloxicam. In addition, he reported the claimant would be referred for further arthritis evaluation (Ex. 23F).
Based on the progress report dated 08/23/16, the patient complains left knee pain upon walking. Discomfort was described as aching, tingling, intense, severe, continuous, pain, discomfort, increasing with movement and varying with activity. Pain is rated as 5/10 without medications and 4/10 with medications.
The patient tells me she is "doing terrible." She has been under a tremendous amount of stress. She spent approximately 10 to 15 minutes describing to me all of the different types of stressors that she is dealing with. She says that yesterday she spent 10 hours in an airport, which was very stressful. Patient is diagnosed with pain disorder with related psychological factors and complex regional pain syndrome I of the right upper limb, chronic pain syndrome. He will follow-up in 4 weeks.
As per office notes dated 5/4/16, the patient is seen for bilateral elbow pain and bilateral wrist pain. She rates the pain as 3/10 with medication and 7/10 without medication. She is active for at least six hours a day and has energy to make plans. Her activity level has
CM was on vacation for the period of 11/25/2016 to 12/12/2016. On 12/17/2016, CM met with the client to complete Bi-weekly ILP Review and to obtain information concerning TPS (“Temporary Protective Status”). Client was dressed appropriately for the weather. She was well-mannered and groomed. In the meeting client appears to be cooperative, friendly and forthcoming with information. Client speech was spontaneous and soft. During the meeting client become emotionally teary when CM inquiries about client TPS status. Client provided CM with a brief history how she came into the shelter system. Client continues to report her most important means of support passed away recently, which is part of the reason she came into shelter. Client continues to denied suicidal or homicidal ideation.
Based on the latest medical report by Dr. Sudberg dated 12/09/15, the patient complains of worsening right knee pain with clicking, right leg numbness and improving left hip pain. Pain is rated as 7/10.

Based on the progress report dated 02/02/16, the patient presents for evaluation of his bilateral knee pain. He was last seen on 01/05/2016.
DOI: 12/13/2012. Patient is a 64-year-old male security officer who sustained a work-related injury to the right knee when he missed a step and fell down the stairs. As per Omni, the patient had a right knee meniscus tear. The patient had right knee replacement on 11/19/14. Per QME Dr. Murphy on 08/11/15, the patient has a 20% whole person impairment rating. Future medical care includes 4-6 visits per year for recurrent symptoms, narcotic/nonsteroidal anti-inflammatory drugs (NSAIDS) in addition to gastrointestinal stabilizing medications, pain management specialist monitoring, office visit with an orthopedic surgeon once a year with X-ray, revision surgery in the future and bone scan.
