Patient is a 51-year-old female accounts payable representative who sustained injury to her neck, back, left shoulder, bilateral knee/foot and lower extremity while she was coming out of the elevator when she slipped and fell forward. Per OMNI, she was initially diagnosed with upper/lower neck, bilateral shoulders/hands, and bilateral ankle and foot sprain/strain.
Based on the progress report dated 05/23/16, the patient complains of constant pain in the mid back, that she describes as sharp. There is radiation of pain into the lower extremities. The patient complains of constant pain in the right wrist and in the left hip travelling down to her left leg with a burning sensation affecting her left knee and ankle, causing numbness and weakness. There is palpable tenderness of the left hip, left knee and bilateral legs (left greater than the right). There is a positive crepitus and mild joint effusion on the left knee.
Diagnoses include thoracic spine musculoligamentous injury, bilateral wrist carpal tunnel syndrome and left knee/ankle internal derangement.
She was given a prescription for Naproxen 550 mg 1 capsule every 8 hours as needed #90 and pantoprazole sodium DR 20 mg 1 tablet twice daily.
Treatment plan includes chiropractic therapy 2 times week for 3 weeks for the thoracic spine, left knee and left ankle, left knee and ankle support, and follow-up visit in 4-6 weeks.
Per verification to the PT facility, the patient has attended 4 PT sessions to the cervical/thoracic and lumbar spine, right wrist and left knee from 03/26/16 through 06/17/16. It was verified from the provider’s office that this is an initial request for chiropractic treatment.
