The nurse-client relationship is the base for all psychiatric nursing treatment approaches regardless of the specific aim. It should be based on trust, understanding, genuineness, empathy, and reliability. Moreover, the relationship should have clear and appropriate boundaries established at the beginning, that are based on the needs of the patient solely. With this in mind, the nurse creates a nursing care plan that is unique and individualized for the patient allowing for interventions that are tailor to fit his or her needs. I had the opportunity to create a nursing care plan for a patient that I recently cared for. My patient was diagnosed with bipolar type I and major depressive disorder and had been admitted for worsening of depression symptoms. She has been admitted for electroconvulsive therapy (ECT) and had a total of eight sessions. But after the last one, she reports no improvement of symptoms and continue with passive suicidal ideations. There were several nursing diagnoses that fit this patient; however, I chose hopelessness related to major depression as evidence by decreased affect, lack of judgment and interest in life. I attempted to engage in a conversation with her to determine what were her strengths and coping skills used in the past. The patient reported that she had been dealing with this illness for many years and that after undergoing ECT and taking Lithium, she was better for a while. This was the reason she wanted to have another course of ECT due to great results in the past except for this time, the results were not what she expected.
