Program: OST Transition to H.S. Completed By: Cindy Reid Date: 7/27/15 Program Utilization: OST is contracted to serve 58 Students. Current Utilization: There are currently 0 students enrolled in OST. Accomplishments: Registration – At the onset of the month the Program Coordinator reached out to 66 prospective participants for the OST program. The program was only able to reach 23 parents that expressed interest in the program and requested an enrollment packet. The remaining prospective did not answer, contact numbers were wrong, or numbers were out of service. Overall, the OST mailed out the enrollment packet to over 60 prospective participants. We originally did not recognise that the child had Foetal Alcohol Syndrome. The child was a slow developer and one who would have huge tantrums. We didn’t think anything of it but just thought some children develop skills slower than others and tantrum wise we put it down to, some children are just difficult but would eventually grow out of it. We only started suspecting something was different when the child started going to pre-school. If I had not had to go to work; then the child would have not attended pre-school but gone into school, when they would reach school age.
On 12-11-16 I responded to the above address. When I arrived on scene, I was instructed by the primary officer to search for casing. while searching the area, I was told to follow the ambulance to MMC and stay with the victim Thom Luu until Detectives arrive to relive me.
On 05-21-2016 at 0834 hours I was contacted at the front desk by April Sutton in reference to a stolen motor vehicle and stolen wallet.
On Thursday, December 14, 2017 at approximately 9:40am I, Officer Jaren Wren was conducting Supervision Sanction Program (SSP) pill call and observed that Resident Jessica Harris ACC# 714001 hair was braided. I asked Resident Harris who braided her hair she stated she did. I told Resident Harris to step out of line, remove a braid and redo it. Resident Harris began taking the braid down, but couldn't re-braid it. I then gave Resident Harris a direct order to remove the braids, and in a very aggressive tone she said, "No!" I then told Resident Harris to face the wall to be placed in restraints. She then said, "Bitch don't put your hands on me!" and began resisting at that time.
Registration – The Program Coordinator has been meeting with parents and contacting student’s homes to get participants register for the program. Currently there are… student registered for the program. The Program Coordinator will continue to meet with more parents and contact prospective participants to get has many students registered for the program before the school year starts, because registering participants during that time is difficult.
On January 29, 2018 the dispatcher put out a call of shots being fired at College Villa apartments. I responded to the call and made my in that direction from the station. I was in route when the dispatcher said that individuals were getting into a white vehicle and leaving the apartments. Sergeant Holter was in front of me as we were driving south on Ladnier road. He spotted a vehicle leaving College Villa apartments and turn onto West Park drive. It matched the description given and he followed it. He followed it as it stopped at a house on West Park. He advised that someone was running from the vehicle and headed towards College Park school. I passed West Park and drove to the school parking lot parallel to Holter's position. I found no one in the area and proceeded to go to College Villa. I made my wy to the back of the complex and saw a black male standing outside of one of the apartments.
This week the COTA brought along a self-pedal crazy cart with us when we went to pick up MR from his classroom. The vehicle immediately caught MR’s attention, and as he approached the cart the COTA stopped him by requesting him to greet us. After a quick greeting, MR sat in the cart and swiftly used both his legs to pedal down the hallway while using both arms to steer with the levers. Once we got to the door of the therapy room, MR stopped pedaling, got out of the cart, and opened the door to enter the room. His session started with developing oral motor by blowing on a pinwheel 10 times. MR blew at the sides of the pinwheel instead of the front. We cannot provide guidance and training if we do not seek for self-improvement. In order to seek for self-improvement, CSM/SGM and senior NCOs have to prepare academically and professionally. Today, there are many ways to prepare ourselves professionally. The Army make available to the service members a wealth of grants and scholarships as US Army Tuition Assistance (TA), Yellow Ribbon Project, and Montgomery G.I. Bill, Troops to Teach Program among others. On November 9, 2017, 2200 hours I was travelling north on M ST NE approaching the intersection of M ST NE and Harvey RD NE. I noticed the vehicle in front of me WA/BDR3681, a 2004 silver Chrysler PT Cruiser. As the vehicle slowed for the red stop light I saw the tail lamps illuminate red, the rear center lamp did not illuminate.
I am writing to inquire about the possibility of securing the position of Program Aide/Learning to Work. An established rapport in all types of communication and a proven problem solver, my skills will benefit all staff at Queens Community House. With an extensive history working with youth, my dexterity in the field will only further the mission of furthering the youth’s education.
I spoke with Seydi in length yesterday evening in regards to Bill. I expressed our concern in the “body tune up” idea Dr. Choksi mentioned, and that Bill had stated “He re-injured himself doing the assigned work.” I explained to her his tasked involved counting glass, verifying stickers on inventory, and putting together very small parts in plastic baggies. She agreed that there isn’t really a mechanism for re-injury doing those tasks. I also told her we may keep him in this role. She mentioned how initially the discussion was revolving around terming him, and did mention there may be a way to do a settlement regarding both injuries and resignation and release. On 09/17/2017 at approximately 8:48 AM while assigned to the patrol division; I, Deputy John Cooper was notified by dispatch of a subject who was reporting a stolen firearm. I was contacted prior to making contact with the complainant, by Sgt. Inv. Bowring, who stated the complainant called him and stated the suspect was currently at the residence of 247 CR 4641 Winnsboro, Texas.
