The reporting party (RP) received call from Diego Baca with Kings County CPS, 1400 W. Lacey Blvd., Hanford, CA 93230 (559) 852-2345 who reported that on 12/21/16 at approximately 3PM while conducting a placement visit for youth Andrew Yanez DOB: 12/18/01 the youth and the social worker took a walk. During the walk Andrew disclosed that he and a staff member named Shawn Wilcox were involved in an altercation. According to Andrew at approximately 2:45PM he was in the office of the ground home with 2 other staff members named James Boston and Nicole Juarez. He denied pushing or grabbing Andrew. James Boston state he was in the back area of the office and didn't see the incident, however, he did hear Andrew using profanity. Staff member Boston stated Andrew can minimize his behavior and stretch the truth. Consequently the RP contacted the group home administrator Robert Carter at (661) 900-6342 and informed him what was being reported by Andrew. Mr. Carter disclosed there was video footage and the situation was being investigated. In addition he stated the language Mr. Wilcox should have used was guided not escorted out of the office. Furthermore Andrew has propensity to fabricate stories. The RP stated Mr. Care acknowledge Andrew was not being a danger to himself or other while in the office. When the RP disclosed to Mr. Carter the prior incident where Andrew reported his finger being fractured and the wrestling/fighting incident, Mr. Carter informed the RP there was never a visit to the hospital a fractured finger only a follow up appointment regarding a previous leg injury. Both youths were sending threats messages to one another, and approximately 1am, both youth got into physical, throwing punches. At this time, writer decided to discharge Stephen for 24 hours as well, and police was called to remove both off property because the atmosphere was unsafe for others. Police was called around 1am, and two officers with badge number 7342 arrived on site around 1:15am. At this time, DB decided to leave through the back door after he saw officers walking into the building. SB went upstairs even when he was told not to, and then locked himself inside one of the washrooms. Writer and two police officers saw Stephen passed out inside the washroom. Officers tried to get him up, but SB was not responding, but breathing fine. Officers called EMS for support, and they arrived 8 minutes later with truck number 976. On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.
The employees were accusing Aurora for being under the influence of Marijuana. Aurora denied the allegation and Kate stated "you're a F'n liar, and Shawna reiterated "you're a liar." Both staff stated "no wonder why you are in the system, because you don't listen." According to Aurora the staff threatened to "tear up" her room to search for drugs. Aurora was crying and told "shut up and stop being a baby." The RP stated Aurora had an encounter with a young boy who put a condom on a broom handle and it her on the "butt" and waved it in her face stating " you slut, you should use a condom. Aurora mentioned the incident to staff person Chris and was told "you shouldn't let that bother you." The RP stated Aurora recalled staff person Kate telling another resident to "knock the F off." She also recalled staff person Shawna saying to her "if I were to speak with my mother in such at tone my mother would kick my ass." Other comments that were heard are "get your ass inside", "if you don't stay within my eyesight we are going to send your asses back home.", "there is no reason to act F'n crazy, because it is not going to help you with anything."
Intake Worker, FPS White received a phone call from the reporter on 11/14/17. The reporter stated that mom went to jail and got out yesterday. The reporter stated that they are not really concern as to mom going to jail due to it was driving with invalid license. The reporter stated that Jessica Fields' oldest son, Raymond Fields called and told the reporter that the reporter needed to come get the children. The reporter stated that Raymond stated that Jessica was on drugs, pills and meth. Jessica drinks all the time. Jessica got beaten up by the boyfriend, Michael Hays in front of the children. The children are being left unattended and with strangers. Jessica didn't have any power or food and told Raymond to come get the kids. The reporter
CPS received CPS referral #0677-3482-7506-1042519 regarding minor Natalie Thompson (DOB: 10/22/01). On 4/7/17, minor threatened to kill herself by jumping in front of a train because she was upset over what happened in her new placement at the group home. Minor stated that she no longer wanted to live and did not want to return to group home. Minor stated that she expressed to staff that she wanted to be left alone, but staff disregarded her request and held her. Two female staff held her back so she proceeded to strike them. During the restrain, staff grabbed minor by her arms, legs and shirt. One of the staff members (unknown) pulled her hair and held her tight so during the restraint causing the minor to sustain multiple bruises on her body.
IRTC spoke with assigned CPS, Leslie Johns, who reported that she made a visit to case address and she was able to observe one subject child who was free of visible injuries. CPS stated that the alleged subject was arrested due to concerns regarding another adult. CPS added that she tried meeting with the other children but the parents of three of the children are not retuning her call and another parent had to confer with her husband before make a decision regarding a visit. At this time, this case will to be enhanced to an IRT.
On 10/22/2017 I, Deputy Mignone #95 was assigned to Intake/Release at approximately 0130 hours, I was pulling an inmate out of DMH to be released. When I opened the door, I witnesses inmate Victor Collazo J#9739840 yelling and walking over to the left side corner where inmate John Nalls J#11602851 was sitting in the corner. Inmate Nalls jumped up and they got into each other’s face, I then instructed both of them to move away when inmate Nalls struck inmate Collazo in the face with a closed fist to the right side of his face.
I responded to Suntrace MHP Lot 37 in reference to a domestic call. Upon arrival, I made contact with Jessica Ray. Ray stated that this moring around 04:00 her boyfriend (Carlos Stewart) hit her in back of the head. Ray stated that he always hit her but this time sheve had enough and want something done about it. Ray stated that Stewart did hit her on back of the head this morning but officers didn’t see anysigns of a physical altercation on Ray. Ray did have some bruising but she advised that it was from a past altercation.
The worker contacted Irma Drew; who is maternal aunt of Saniya Daniels. Mrs. Drew stated “Saniya was at her great-grandmother Norma Drew’s home Mrs. Drew noticed Saniya had a knot on her forehead being concerned she asked Saniya how she got the knot on her forehead. And Saniya said she was hit. Norma the asked Gerald Daniels how Saniya got a knot on her forehead and Gerald said she fell playing with her cousin. Beforehand I asked Saniya too and she told she fell. Well my mother didn’t like the reason she was giving by Gerald and reported the incident to DHS. The reporting party (RP) stated on 4/22/19 at approximately 4:30PM 10 year old day care child Tobias (last name unknown) was observed sleeping on the corner of Oak Lane & Cypress Dr. in Huntington Beach. The RP stated Tobias was observed at the same corner all week. The RP came up to Tobias and asked him who was he waiting for and he responded that the bus drops him off there after school at 2:30PM. Tobias disclosed he was waiting for his day-care/babysitter. Tobias' mother Sylvia (last name unknown) was contacted regarding his safety in turn Sylvia contacted the licensee Blanca Galindo and asked that her son be retrieved. The person who arrived to retrieve Tobias was rude to the RP who then refused to release him. According to April Wright
The reporting party (RP) reported receiving a call from the father of consumer Julianna Cardiel age 32 DOB: 3/11/85 (father's name not provided) CCIB LPA left a voice message for reporting party regarding father's name. According to the RP, the consumer and her father met, at which time he revealed he spoke with the Adult Day Care Center caregiver (Ruth Mendoza). Subsequently, during the conversation the caregiver became argumentative and disrespectful when the father questioned the care his daughter Julianna was receiving. According to the father, Julianna would return home appearing bothered and crying. Consequently, the father decided to speak with a supervisor who replaced the caregiver.
REPORTER: The reporter/father (W. J. Coleman Jr.) called to report verbal and physical abuse. On Sunday, the reporter contacted Law enforcement, and they went out to the home; the police officer suggested that the reporter contact DHS. According to the reporter, Tonya (mother) slaps the child in the face (unknown when/on going). It is unknown if the child had any visible marks or bruises. The reporter said Tonya verbally (cursing) abuses the child. Also, the grandmother (Shirley) and mother (Tonya) was disowning the child and threatening her because she said she wanted to stay with her father. Jeremiah Malley was residing at the home. Jeremiah sexually abused Jimmy Melton. Unknown if the children were properly being supervised. Jeremiah has a history of sexually abusing children and it is unknown if the grandparents were aware of those allegations. The grandmother, Peggy Wganer, disclosed the abuse to me. The reporting party is being added to this complaint, as the information provided is in addition to the information provided previously regarding an allegation of Physical Abuse. According to the reporting party Isaac has disclosed he was socked on his right jaw and stomped on by foster mother, Anita Caudillo adult son Robert Renteri due to the son's suspicion of inappropriateness with his daughter. When the examiner asked Rosslynn information pertaining to the alleged sexual inappropriate incidents, the youth reported her stepbrother had spoken to her about having sex, she noted she later went into his room and stepbrother pulled his pants down. She further admitted to also pulling her pants down any laying down. According to Rosslynn, Robert got on top of her and his penis touched her vagina. She denied any penetration. Rosslynn reported after the incident had taken place they both went outside to play. The youth did not report any inappropriate sexual behaviors occurring between her and her younger sister. Given it was the intake session, the youth was not pressed to provide any detailed information. The mother reported as a result of the allegations, she has taken the necessary precautions in order to ensure the safety of her children. Specifically, she indicated she referred Rosslynn to the Wellness Family Center to seek therapeutic intervention for her sexual behaviors. Additionally, Rosslynn is not allowed to play or spend time unsupervised with her siblings or go outside. Mr. Ziegler noted he has installed an alarm sensor on her bedroom door to monitor when she leaves the room. The mother reported she was recommended to seek therapeutic interventions for Rosslynn and agreed to allow her to participate in the SAFE program.
