On September 6. 2015 at 1956 hrs. Security Operation Center (SOC) radioed East Orlando Officers in regards of a combative patient at the Emergency Department by EMS off load Service entrance. Security Officers Carlos Ayuso, Allan Topher and I, Lourdes Garay, responded to the call. Upon arrival, Security observed the EMS Team arrived with the patient Michael Lewis Driscoll in soft restraints. ER Charge Nurse Robbie Phillips attempted to ask the patient some questions but patient was intoxicated and acting out and very verbally aggressive towards nursing staff. Security was informed by Physician Douglas Haus OD, that patient would be kept in restrains and ordered some medication for him.

On Thursday 11/12/2015 at 1905 hours, during shift briefing, Officers Jason Peterman and William Miller pass down information concerning what was believed to have been an elopement from CDU by a male Baker Act patient. He was reported missing by Registered Nurse Ophie to Doctor Gomez at approximately 1530 hours. Doctor Gomez who was concerned for the patient’s safety and wellbeing then contacted Primerose Vernet from the Florida Hospital Risk Management Department assigned to our facility and she in turn contacted the Orange County Sheriff's Office and reported the issue.
On Saturday 10/26/2016 at approximately 2328 hours, Security Officers Christopher Paz, Ariel Weiland, Omar Alonso along with Supervisor Steven Evans were dispatched to the EMS Off load Ramp for an incoming (51S) Patient Standby In E.D. Upon arrival at 2328 hours Security met with E.D. Charge Nurse Johnathan Bacal who stated that there was a combative male patient being transported by the Orange County Fire Department. At 2330 hours, the patient, Alan Castillo (DOB: 08/03/84; Fin #86501337) was brought by Orange County EMS (Engine #83) with an escort from Orange County Sheriff's Deputy. On May 15, 2016 at approximately 2335 hours Security Officers Steven Evans and Omar Alonso were in the E.D. Security Office when we heard someone yell for help inside of the E.D. entrance doors in regards of a “Gunshot Wound” patient. Out of the Office, we observed a Hispanic male hoping towards the PFS counter with blood on his left abdominal area. Security Supvr. Evans grabbed a wheelchair and wheeled him to one of the E.D. triage room, while his friend, Joseph Park (witness) register him with PFS. Security team in coordination with E.D. Charge Nurse Robbie Phillips placed the Emergency Department in a lockdown, Security Officers responded and kept watch over both E.D. entrances. During transport, another vehicle went in front of me, and I pressed on the breaks to avoid an accident. Immediately, the inmate complained of her head being hurt, noticeable bleeding appeared on inmate’s forehead. I advised the dispatch of the incident and advised Western hospital of the delay. The inmate was transported to the nearest hospital for injury assessment and treatment. Upon arrival to the hospital, the nurse asked for handcuffs to be removed from the inmate, deputy J advised the nurse that per policy the removal of handcuffs is prohibited. Upon arrival, I had initially provided support in standing by while the Narcotics team went through the house getting everyone out. I then was asked to assist in search the two female subjects that were detained. While searching the females I went back to my vehicle to grab another set of gloves. While I was doing this, I had seen Officer Fredrick #1111 had one of the subjects identified as Keith Gray W/M (DOB: 07/09/1958) on the ground. Mr. Jones stated, he had never seen the individual before and was unable to provide an accurate physical description. At 0650 hours Orange County Deputy Stephen Williams (Badge #25569) arrived on site. D/S Williams conducted an initial investigation with the Stab Wounds Victim and after questioning, he determined there was no immediate threat to Florida Hospital staff. He then contacted Security Supervisor Steven Evans and concluded the lockdown could be lifted. Supervisor Evans then radioed Dispatch and the Emergency Lockdown was lifted at 0705 hours at which time both Supervisor Nurse Administrator and Security Manager King were notified. On Thursday 11/17/2016 around 2300 hours, I, Security Steven Evans was informed by Dispatch to make contact with 5 East Unit Charge Nurse Tonya Smith in regards of Code (34) Customer Service Assist. I arrived at 2302 hours and met with Charge Tonya Smith who explained to me that the Correction Officers for the prisoner in room # 504 had given her hard time few Months ago on the patient, Angel SantiagoGonzalez (Fin# 86424089) last visit. Nurse Smith further stated that she had a personal problems with one of the Officer and that she was informed by Lake Correctional Institution that this Officer would not be working at all on 5 East. On 10/14/2015 SO EMT Perez was dispatched to HS-406 to apply a bandage. SO EMT Perez knocked and anounced his presence at the door and was verbally invited in by the resident. The resident, a Mrs.Martha Doyle was sitting in her living room chair and was bleeding from the forehead. Mrs. Martha Doyle was washing her face when she attempted to remove a bandage over a previous wound and she wanted it re-bandaged as she stated. SO EMT Perez asked Mrs. Martha Doyle if she wished to go to hospital before he assesed her to which she denied. Mrs.Martha Doyle also denied any care besides the application of a new bandage. On the night of 5-26-17 at approximately 2145 security recieved a call from Casper Police Department dispatch asking for the ambulance bay door to be opened as Police were responding to a disturbance in the Emergency Department. Previous to this call security knew nothing of a disturbance. Security Officer Ubbes escorted Casper Police Officer who walked through the front entrance. Upon arrival Ubbes was informed by one of the ER nurses that the patient from bed 11 was extremely intoxicated, he was leaving his room wandering the ER without any clothes on and entering other patient rooms. On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. On 7/6/2015 S/O EMT Perez was dispatched to WC-407 regaurding a pendant alarm. S/O EMT Perez knocked and annoounced his presence at the door, when there was no answer S/O EMT Perez proceeded to search the apartment. S/O EMT Perez found the resident, a Mrs. Mary Beiter toppled over her walker by her bedside. Mrs. Mary Beiter was trying to get inter her bed when she lost balance and fell and could not get up under her own strength. Mrs.Beiter did not complain of any hip pain during the pelvic exam but did state that she has a bad knee and hip that makes it rather hard to stand on her own. Monroe was escorted off the unit by Security Supervisor Steven Evans and Officer James Johnson to Security Office while Security Officer Carlos Ayuso was instructed to stay in the unit and gathered witness’s statements. Security Supervisor Evans called the Police at 2044 hours and reported the threat and Orange County Sheriff’s Deputies, Kristine Helms and Jeff Payne arrived on the scene at 2100 hours to investigate. Deputy Payne met with Mr. Monroe at the Security Office while Deputy Helms was accompanied by Security Supervisor to the 4th floor and spoke with Charge Nurse Trofibio about the threats. Upon, further investigation, OCS Deputies took Mr. Monty Monroe into custody on assault charges against a Medical Employee. Consequently, he was also being trespass from Hospital property. Sheriff Deputy K. Helms filled out the Trespass Warning and provided case number 15-114720, witness statements were taken and will be included with this report. Security Supervisor Evans notified Administrative Nursing Supervisor Mary Cisco and Charge Nurse Darren Trofibio of the end result and informed the Charge Nurse that Security staff would be glad to escort him anytime requested. On 9/12/2016 at approximately 0740 hours Deputy Chavez B1871 and I responded to a Assault DV in 9615 South 7th avenue Phoenix, Arizona 85003. When we arrived at the residence we heard Amy Frieberg scream, we entered the house and Ron Frieberg and his wife where having a verbal argument and where in each other faces. We separated them both I took Amy outside to talk to her and my partner stayed inside to talk to Ron. I noticed that Amy had a swollen right eye and a red mark above her right cheek. On 8/11/15 S/O EMT Perez responded to a 911 escort for CL-322. S/O EMT Perez escorted Engine and Ambulance crew 841 up to the apartment. While 911 was assessing the resident a Mr. William V. Johnson S/O EMT Perez talked to S/O EMT Anti and inquired of the situation. S/O EMT Anti stated that the resident was complaining of chest pain and high blood pressure. The 911 crew after assessing then suggested to the resident that it would be in his best interest to be transported to the hospital and seek a higher level of care. The resident agreed. The aide then proceeded to call the residents son Donald Johnson to inform him of the transport. S/O EMT Perez talked to Donald also and stated to the Son that his father was being transported to the hospital
On 12/25/15 at approximately 1045 ICS was activated in Higbee for offender Dokken OID 237786 who reported that she had swallowed a tooth brush. Upon arrival at the scene, offender was sitting in west wing loung in no apparent distress. Upon interviewing offender, she reported that she swallowed a tooth brush. Offender complained of a dull pain in her chest which worsens with breathing. She stated that she was felt like the brush is stuck in her chest. She denied difficult breathing or shortness of breath. Assessment and vital signs were completed. On call provider was updated and gave an order to send offender to ER for evaluation via DOC
