On Wednesday 01/04/2017 at about 2227 hours, FHEO Security Officers were dispatched to the Children's Emergency Department (CEC) room #5 for a Signal (42T) Threats/Intimidation Against Employee. Security Supervisor Steven Evans along with Officers Corey Ritchie, and Omar Alonso, responded at 2229 hours and met with Doctor Yaron Ivan and Assistant Nurse Manager Kimberly Brooks. They both stated to us that the father of the Baker Act patient in CEC#5, Kiana Hughes (DOB: 07/04/2002-FIN #86674239), whose name is Finas Hughes, was creating an issue of safety concerning the transport of the patient from here to her assigned Psychiatric Facility. He stated that he felt he was being inconvenienced for no reason at all, he also asked for a copy of the Policy and Procedures set forth by the State in reference to the transport of Baker Act patient's. There was an Orange County Sheriff's Deputy inside CEC who was asked to explain to Mr. Hughes the process for transporting a Baker Act, which he did but Mr. Hughes still felt that he, as a Federal Marshal, had the right to transport his daughter and he again asked to see a written copy of the policy and procedure which was printed out by Assistant Nurse Manager Kimberly Brooks and then handed to Mr. Hughes and the patient's mother, Kenia Matos. Mr. Hughes had stated to Security and Nursing staff that he already had a room reserved for his daughter at UBC (University Behavioral Center) yet that was the same facility that had been assigned to the patient by our placement staff, and our staff, including Nurse Brooks, had been advised by UBC that they currently did not have an open assigned room for the patient. OFC Montejano made contact with the Hegle at Primary Care Desk B lobby. Where the she gave an omission and stated she had an appointment and because the Johnson was not at her desk she missed it. Further, she claims Dr. Little told her she could get the Tens Unit right away. I did however confirm that Hegle did not have and appointment for the use of the Tens Unit. On Monday, March 7, 2016 at approximately 0139 hours, FHEO Security Officers were dispatched to room #5109 for a (53D) Disorderly Patient in Medical Unit who was acting in a belligerent manner and attempting to leave his room to go home. I, Steven Evans Shift Supervisor of Security at Florida Hospital East Orlando (FHEO) arrived at 0140 hours along with Officer Thomas Mejia and met with Charge Nurse Michelle Viena and Patient’s Nurse Lisa Guo who were trying to calm down the patient, Ramon Jr. Bermudez (DOB: 05/24/1961; FIN# 85321318), who had been attempting to get out of bed and wanting to leave. On Saturday, December 26, 2015 at 2029 hours. FHEO Security Officers were dispatched to the East tower, room #409 bed 2 for a Disorderly Patient (53D) who was combative with nursing staff. All Security staff responded and on arrival we were met by Administrative Nursing Supervisor Mary Cisco and Charge Nurse Darren Trofibio outside of room 409. The Charge Nurse, Daren explained to us that patient’s boyfriend who was later identified as Monty Monroe was very aggressive and threatened him with bodily fluid and to punch him. Neal’s mother and ESA Social Service Representative, Novella White failed to adhere to ESA policy and procedures when handling applications for processing of a family member. According to the report, on Saturday, December 5, 2016, Ms. White allegedly slid Mr. Neal’s recertification application, dated December 4, 2015, onto Social Service Representative, Deborah Bush desk while she had been preparing several other cases for processing. Upon receipt of the application, Ms. Bush placed the case in a basket in Ms. Burt’s office for review. On December 7, 2015, Ms. Bush informed Ms. Burt that she did not want to process the case because she has a personal relationship with Mr. Neal and aware that ESA policy and procedure prohibits employees from handling cases of customers who they personally know. Received a call from an attorney asking for information about his client's husband. The attorney had called the SPD Branch and he was told they cannot share any information because the are prohibited by HIPAA Law. He was informed it is because the DHS workers are not allow to share information due to public assistance laws
At approximately 0813 hours, on 31-Jul-15, the writer, SSS. E.Durani and Sh/S A.Singh responded to call from POC to attend 3-East. The officers attended and upon arrival the officers were informed by the unit clerk that the patient has left the unit. The officers were also told that the patient was going to Cafeteria. The officers cleared from 3-East and attended Cafeteria. Upon arrival the officers met with the patient’s Care Aide, who asked the officers to escort him back to his unit. On 10/15/2015 at approximately 1959 hours Florida Hospital Security Operation Command (SOC) radioed FH East Orlando Security for a 51D-Disordely Patient Ed in room # 37. I, Steven Evans arrived at 2000 hours and met with ER Nurse Juana Morales, who informed that patient by the name of Scott Thomas Brady (MRN: 484896/Fin: 84708322) who just been discharged, was agitated and upset with medical staff. CM met with Thomas, Kevin/ Open Arms Case Manager and Amy/ ESP Staff to conduct Thomas's discharge, case management services and ID Waiver Intake. Thomas appeared tired as his eyes closed and head lowered for some of the Intake. He complained early during the meeting that he didn't want to answer a lot of questions, however with some verbal encouragement he went on with his responses. Thomas talked much about his medication and seemed at times to get his current medications confused with his old medications. Due to a history of overdosing and taking medications that weren't prescribed to him; his roommate makes sure Thomas is taking the correct medications. On Friday 12/16/2016 at about 0135 hours, Security Staff was dispatched to the Special Care Unit, ED. room #37 for a code (51D) Disorderly Patient in E.D. I- CM inquired about client’s meeting with LA Family Housing (3/7). CM updated client’s medical and housing appointment list to assist him with remembering his schedule. CM informed client that he has scheduled transportation to and from the VA hospital for his upcoming colonoscopy appointment (3/11). CM informed client that he would be provided a sandwich from the kitchen when he returns from his colonoscopy appointment. CM informed client that his transportation is only going to “transport” him to and from the hospital, and not stop to buy him dinner, because a sandwich is being provided when client returns. CM continued to assess client’s mental health and medication compliance.
On 6-24-16 I spoke to Joel Brand the attorney of Hugette Moothery DOB 6-28-76 in reference to the allegations of harassment by the case officers. When speaking with Joel he stated he was not aware of Hugette filing any complaints against officer of the Maricopa Police Department, in fact he stated he only was representing here for her civil custody case. I read over the two incidents related to the complaint and reviewed four videos and I did not observed any forms of harassment.
On Thursday, December 22, 2016 at approximately 2147 hours, Security Operations Center (SOC) notified FHEO Security in reference to a signal 54B (Missing or Eloped Baker Act Patient), from the East Tower room #502. Security Officers Omar Alonso (420) and Christopher Paz (408), responded to the scene. While enroute, SOC advised Security staff that the Baker Act Patient, who was later identified as Daniel GonzalezRamos (DOB: 11/04/1987 – Fin #86614786), was exiting the building from the South wing, stairway A, located in 5 East. As Officer Alonso and Paz ran towards stairway A and attempt to stop the patient from leaving, Assistant Nurse Manager (ANM) Tonya Smith rushed in the Emergency Department triage stating that the patient had ran towards the main lobby, west tower. Alonso and Paz conducted a search of the area until they observed a Hispanic male with black hair, wearing paper blue scrubs standing next to a visitor, later identified as Maurice, talking on a cellphone. Both Officers approached Patient, GonzalezRamos and advised him that he had to return to his room. Mr. GonzalezRamos was very uncooperative and refused to go back to his room. As Paz attempted to walk closer to him, he bolted into Lake Underhill Road. Officer Paz extended his right arm and attempted to pull the patient from going into incoming traffic, as he grabbed the back of his blue scrub shirt, the shirt ripped off. The patient
On Sunday, January 10, 2016 at approximately 2135 hours, FHEO Security Officers were dispatched to the Special Care Unit (SCU) room E.D. #38 for a Disorderly Baker Act patient attempting to leave. Security Officer Omar Alonso along with Officer Thomas Mejia and Supervisor Steven Evans, arrived at 2136 hours and met with Nurse Kristen Strauch who was standing next to the patient. The patient, Tyler Hull (DOB: 01/07/1987; FIN# 85068604) was in a state of anxiety and stating that he could no longer take being coped up in a room and he needed to leave. Security staff explained to the patient what the Baker Act Law meant and why he was not allowed to leave until he had been medically cleared and seen by a Psychotherapist.
On June 10th, 2016 at approximately 2205 hours, FHEO Security was notified by clinical staff in reference to an incoming (51D) Disorderly Patient in E.D., located in the EMS offload. Upon arrival, Security staff made contact with Orange County Fire Rescue (OCFR) Engine #83 and restrained patient James Patrick Hogg (DOB: 03/26/1968 - FIN: 85758458). OCFR staff stated patient, Hogg is Altered Mental Status (AMS) and was uncooperative with rescue personnel. Security staff escorted OCFR and patient, Hogg to the Special Care Unit (SCU) room #37. Patient Hogg agreed to remain calm and be cooperative during his care at the hospital. Clinical staff removed the soft restraints off of Mr. Hogg without incident. All cleared, the final call disposition
