REPORTER: The reporter/ Therapist (Felissa) called with concerns for the victim, Kalise. The reporter called to report lustful touching and possible physical abuse. According the reporter, in August of 2015, the child told her mother (Shanna) and grandmother (Chaundra), “J touched me, he pulled my diaper, he tore my gown’. They attempted to ask her more questions and she said “I don’t want to talk about it”. The reporter said the grandmother stated that the child didn’t want them to pull her bottoms down when she returned home. The incident took place at Jeremy’s (Father) home on 8/02/2015. The incident was reported to DHS in August of 2015, but there was not anything done about, per reporter. The child received a medical examination, and
On 9-17-17, the unknown male child's grandmother Faye; asked me to look at the child back. The child's left shoulder blade had a scar. The scar appeared to like someone used their two middle fingers and dug into his skin. The child was bleeding and scared. The child's grandfather (unknown) did this allegation. Faye called the child to come to her. The child was afraid to come to the grandmother. Faye stated told the child "I am not going to do anything to you." The child finally came to Faye. The child did not seek medical attention. The child's mother *(Santana) was not at home. It is unknown if Santana knows about this incident. IRTC spoke with CPS, Cari Covahey, who reported that she spoke with the subject child twice and there was no disclosure of sexual abuse. CPS expressed that she only conducted minimal fact finding interviews with the subject child and will inquire more pertaining to the alleged relationship between the subject child and her boyfriend. CPS expressed that the only person the subject child informed this information to was her 7 year old cousin. CPS expressed that the foster parent is unaware of the child having a boyfriend and the source indicated that the child was alleged telling her cousin a “dirty story.” CPS articulated that the subject child was medically assessed at Richmond University and there was no observed trauma to the vagina or the
3:20pm- IRTC spoke with assigned CPS, Oneeka Peters, who reported that the subject child was interviewed at the Brooklyn CAS on 3/21/17 and there was substantial disclosure of possible sex abuse. CPS expressed that Detective Ruby was present and he reported that he does not think the alleged subject is the foster parent’s ex-husband, but there was a brother (foster parent’s relative) who frequented the home during the timeframe the child was referring to. CPS expressed that the subject child stated that the alleged subject was the foster parent’s husband, but there were questions as to who she was actually identifying. Reporter stated she went to the family home today. She spoke with Jonnie and asked if anyone every touched her. Jonnie said that two of mom (Andria) boyfriends have touched her. Trimel and Jermaine. Trimel touched her vagina twice. He did it while mom was sleep. Mom called the police on him each time. Jermaine touch her vagina once. It is unknown if mom know about the incident. Jonnie said that Jermaine was killed by his brother for touching another child. Jonnie did not disclosed if she was penetrated. The reporter stated the children told their father they don’t know the last time they took a bath and whenever she bathed them the bath water was completely dirty. The reporter stated the children are underweight even though their mother receives food stamps. According to the reporter, all three children had sores around their mouths, in their heads, on their backs and buttocks, and on their arms and legs. The reporter stated on 12/05/15 the victim was brought to the emergency room and diagnosed with pneumonia. According to the reporter, Braxtyn’s condition is serious but it’s unknown if it’s life threatening. Ms. Donaldson stated South Central is unable to provide Braxtyn with the care he needs so he’s being transported by ground to UMC in Jackson, MS. At this time er doctor’s have expressed concerns about his mother accompanying him to UMC. Gwenyth came to school on 09/03/15 with bruises on her left arm and the child indicated to the reporter that she had bruises on her buttock area; the bruises do not require any medical treatment. Gwenyth told Mr. Temple on 08/31/15 her mother asked her if she’d put clothes in the wash. Gwenyth thought she had and told her mother yes but later remembered she hadn’t; Ms. Daly thought the child lied to her and whipped her with a belt on her buttocks area, hit her with a wooden spatula on the arm until it broke, and grabbed her by the neck threw her on the floor and this is when she punched the child in her nose. The RP stated medical office has received 4 alleged abuse complaints from 4 different children. Each child stated Ms. Lisa was the perpetrator. The RP stated a 3 year old Hispanic girl was initially potty trained. The RP stated the child was eager and happy to attend the preschool however; the child began to soil herself and refused to back to the preschool. According to the RP the child stated that Ms. Lisa put her finger in her vagina.
On Oct 8, 2012, a counsellor from Terry Fox Secondary School brought forward concerns of Sally Brown being abused. Mr. Highcliffe reported that he noticed some bruises on Ms. Brown's upper arm and shoulder. When Mr. Highcliffe asked Ms. Brown about these bruises, she replied with a vague story of being involved in a game with her friends and collided with a wall. To investigate, I visited Ms. Brown at her residence and was informed that her parents were away for an overnight prayer vigil at their church. From initial appearances, Ms. Brown presented as depressed and insisted that I leave. I was able to establish a rapport with Ms. Brown and discussed my concerns about her noticeable bruising. Upon agreement, I took Ms. Brown to the emergency ward of the Prince George Hospital where Dr. Rose, examined her arm and shoulder. From Dr. Rose's medical opinion, the bruising did not appear to be consistent with her story of running into a flat surface such as a wall, rather appeared to have been caused by a blow by a heavy instrument. I confirmed with Dr. Rose that he would be willing to testify in court to his findings about the bruises. On return, Ms. Brown told me that she had not been abused by her parents and that she felt safe at home. From further
The reporter stated on 08/09/16, Tynia disclosed to her physician that at the age of 2 her 30 (+) adoptive brother raped her; it's unknown if the adoptive brother was 30 when the incident occurred or if he's 30 now. The reporter stated it's unknown if the child was in MI or LA when the abuse occurred, if she still has contact with the perpetrator, or if the abuse occurred more than once. The reporter also stated it's unknown if the incident was reported to law enforcement or DHS. REPORTER: The reporter/not related (Amy) called with concerns for the victim, Dayne. According to the reporter, Mindy (mother) doesn’t work, and she wears a neck brace. Mindy has dizzy spells, but she continues to drive with the child in the vehicle. The reporter said Mindy is not caring for the child. Mindy claims that she is sick to get rid of her child, and Gary keeps the child every night but he is not abusing or neglecting the child. The child has bruises all over his legs and arms; the bruises look as if he is being grabbed on. It is unknown who is grabbing on the child. Reporter believes this because it was stated by some children in the neighborhood that they had been messed with. Reporter is not sure if the step-children have been abused or neglected. REPORTER: The reporter/ Outpatient Therapist (Shirley) called with concerns for the victims, Jonah and Ethan. According to the reporter, on 11/03/2015, Jonah (reporter’s patient) reported that Ethan touches his private parts, during a psychological evaluation. The mother (Alissa) said the boys have been flicking and pinching each other in the private parts, and she has been redirecting them (unknown when). The reporter spoke with Alissa about speaking to the boys about “good touch, bad touch’, and mom agreed. The reporter met with the children separately, and Jonah reports that Ethan touches his private parts all the time. REPORTER: The reporter/ Medical Social Worker (Laura) called with concerns for the victim, Beulah. Beulah suffers from Major Depressive Disorder, Osteoporosis, Osteoarthritis, and Disc Generation. She has a history of falling, and she needs assistance with her ADL’s. On Monday (03/14/2016), the victim told her nurse (Madeline) that her Penny (granddaughter/caretaker) pushed her down. It is unknown if she has visible marks or bruises. Today (03/17/2016), the victim said that Penny (granddaughter) didn’t push her down; the victim said she bumped into her granddaughter’s butt and fell down. The reporting party (RP) stated when she arrived to pick up her 3 year old daughter Hadley she was shock at her appearance. The RP was informed that Hadley had fallen off the slide in the morning (exact time unknown) and sustained scratches, abrasions, and a bump to her face and head. The RP questioned the teacher Ms. Usha regarding what care was administered to the child. The RP was informed the child received no care due to staffing issues. According to the RP she was informed the teacher could not leave the other children to attended to her daughter’s needs. The child sustained a bump to her left temple and received no ice or cold pack. The RP stated she contacted the facility to informed them the child need to remain at the facility a while
