In every setting social workers work in, they must use appropriate theories, treatment models, and research literature when working with a patient or patient systems. One social work setting I will be discussing is the Margaret Tietz Nursing and Rehabilitation Center that I am interning this year. To address the needs of any patient at the Margaret Tietz Nursing and Rehabilitation Center, social workers must complete a Psychosocial, Brief Interview for Mental Status (BIMS) and The Patient Health Questionnaire (PHQ-9) to find out what each patient needs. After they have completed those assessment, social workers need to work in an interdisciplinary team to conduct care plan meetings to address all their patient's presenting problem. The purpose
Social services within the outpatient and inpatient programs are facilitated by the social workers. The social workers host group therapy sessions, complete treatment plan, facilitates the aftercare planning meetings between the patient and their family members or friend, locate housing centers for homeless patients as well as those that are in unsafe living conditions, determine patient eligibility for program services, conduct biopsychosocial assessments, conduct CDC with patients, evaluate patients support systems, physical and emotional functioning of the patient’s, financial stability, safety, suicidal/homicidal ideation, implement treatment and discharge plans that coincide with the patient’s medical treatment, and offer referrals to residential housing and community resources.
Social workers in the mental health wing of the Woodstock General Hospital (WGH) are an integral part of an interdisciplinary mental health team that provide patient-centered care through both inpatient and outpatient services. They offer capacity-building program interventions to individuals, couples and families by providing single session walk-in counselling, groupwork, psychiatric evaluation, crisis intervention and inpatient support as well as long term counselling. There are specialized outpatient supports including early psychosis intervention, programs for eating disorders, Cognitive Behavioural Therapy and Dialectical Behaviour Therapy, programs for children and youth as well as programs specific to geriatric mental health. Social work professions need to understand the importance of how individuals interact both with other people and their environment, to have an understanding how individuals are affected by these interactions (Rogers, p. 2). According to Rogers (2016), “Social workers are knowledgeable about human behavior across the life course; the range of social systems in which people live; and the ways social systems promote or deter people in maintaining or achieving health and well-being. Social workers apply theories and knowledge from the liberal arts to understand biological, social, cultural, psychological, and spiritual development (p. 2). Their work with clients begins with assessments to evaluations of intervention and is based in and supports of the core value system of the profession.
The service goals of Therapeutic Patient Navigation through the project would include individual and family counseling; linking patients to specialty care; assisting with medical adherence, participating in the health planning process; educating patients and family about the illness; providing extensive support with the daily functioning through providing wrap-around service referrals. The outcomes seek to improve symptom management, improve patient emotional support, and increase the patient quality of life by decreasing the emotional destress caused by lack of support, and confusion over navigating the medical system. Tackling the dynamic nature of the potential range of social work services to these patients ultimately shaped the program design, and widened the scope of practice of the social work team in the field, and required that crafted new model of social work.

A typical day for a social worker consists of helping those that are in need, providing appropriate resources, and empowering their clients. Before a social worker can help their clients, they will need to do an assessment. An assessment consists of learning about the individual and their everyday lives. This model is often used, since it focuses on the boundaries of the systems as it seeks to focus on each individual client (Schatz, Jenkins, & Sheafor, 1990). Beginning in the 1960s, the generalist perspective was created from the Milford conference, which stated that a generic approach to social work was necessary (Schatz et al., 1990). The generalist perspective continued to develop throughout the 1960s and 1970s, and it quickly became the base for all social work (Schatz et al., 1990). What occurred with the foundation of the generalist perspective was the interlacing of casework, group work, and community organization with the emphasis on understanding the interaction between individuals and their unique environments (Miller, Tice, & Hall, 2008). Social workers in the healthcare field can be very beneficial to an interdisciplinary team. Throughout the semester we have looked at how social workers bring their professional and distinct skill set and beliefs to these teams. There are many stereotypes and biases surrounding social workers and other professionals on interdisciplinary teams. Social workers also have different views than other healthcare professionals and use their skills in communication to help the clients they serve, along with various other skills that are unique to social workers differing from other professional in the healthcare field. My field placement was ( I am no loner at this site) at the Mercy Fitzgerald Hospital in the Psychiatric Medical Care Unit, an acute short stay unit. All patients are admitted after careful evaluation by a psychiatrist and/or a psychiatric nurse practitioner at the crisis center. The patients are first stabilized in the crisis center then transferred to the unit for further treatment and evaluation. Once on the unit a multidisciplinary team comprised of psychiatrists, psychiatric nurse(s), social worker(s), recreational therapist, peer specialists, mental health technicians and other medical professionals (as needed ) work closely with each other and the patient to develop a treatment plan and discharge planning.
Results, analyses, and assessments in this article would contribute to evidence based practice in social work. Social work is an expanding field in settings and situations such as these. To study more quantitatively with qualitative longitudinal study would improve how clinicians are able to accurately assess and serve those needs. In the generalist social work practice, a social worker is a change agent due to the expansive and complex umbrella of social and human services and roles. A change agent works to promote positive changes for the well-being of an individual, family, group, organization or community. In order to fulfill the tasks of a social worker, one should be trained and equipped with a wide variety of skills: such as setting appropriate boundaries, possessing and utilizing self-awareness and the ability to counsel or advocate for clients. In addition to many skills, the generalist social work practice includes a surplus of methods to either prevent or intervene when necessary. A method often utilized by social workers is the person-in-environment conceptualization. The person-in-environment perspective considers factors, such as familial or economic, beyond the individual to better understand the client’s behaviors or situation. Another method is the strengths perspective. The strength-based perspective focuses on the client’s or client system’s positive qualities to build upon those capabilities. And the recognition of strengths helps to achieve goals in a more client-led approach. Generalist practitioners are professionals with a wide range of knowledge and a repertoire of diverse skills in order to enhance the social functioning of all within society.
