On Thursday, August 27,2015, @ 5:29 pm ,Tyneice Daishae' Key( DJS Youth ) was redirected by Dennis Thomas ( Senior Residential Staff) to return to Tours Unit. Tyneice Daishae' Key( DJS Youth ) became verbally aggressive towards Telethia Menefee ( Residential Staff). Tyneice Daishae' Key ( DJS Youth) also began to curse and threaten to hit April Smith ( Supervisor of Tour Unit). Tyneice Daishae' Key( DJS Youth ) was redirected to her room to take time away. Tyneice Daishae' Key( DJS Youth ) was able to return to the unit and go to her room. Once in her room Tyneice Daishae' Key (DJS Youth) began to use her chair to hit the door with. I Shaleta Garrett have been providing financial support for Trevious Z. Garrett and Ziiana S. Garrett since birth. They, Ziiana and Trevious, have been residing with me, Shaleta Garrett, in my residence, 315 Powell Mill rd. since birth. I, Shaleta Garrett, provide shelter, clothing, hygiene, medical bills, school funds, food, and all of their other needs.
I spoke with Versie Crossland the cook at operation on 11/16/15. Ms. Crossland stated she was in charge the day of the incident while D'Andrea Franklin and Brittany Shavers were on bus route. Versie stated she believed Nelda Shaver the director was at her other operation in Queen City that day. Versie stated she was cleaning the kitchen up and preparing snack for the after school children when BreAnn Henderson came in the kitchen and asked for an ice pack. Versie stated she asked BreAnn what happened, BreAnn replied that Daniel had fell and hurt his forehead. BreAnn also told Versie to call his mom. Versie said she was walking to the office to get the phone when she saw D'Andrea Franklin pull up so she went out and told her what had happened.

My life story I was born on November 25, 1997. I"m a loving son, brother, nephew, grandson, and cusion. My life started out great and awesome. Four years after living a life my mom and dad had other child a little boy my brother named DJ. That was the start of a new life and someone to hang out with. Growing up Katherine Dunham never thought about dance. Dunham was one of the first African-American women to attend the University of Chicago and planned to earn a degree and become a working woman, however while she was attending university Dunham began taking classes with Ludmilla Speranza, a former dancer of the Moscow Theatre, and so her love affair with dance began.
Katherine Dunham was an African American women who was known for many things throughout her life, more specifically her dance career. Katherine was born on june 22nd , 1909 in a Chicago hospital, but moved to a small place in illinois. Katherines mom died when she was three and her father got remarried; moving them into a fairly white neighborhood. Growing up Katherine enjoyed writing and dancing. When she was 12 she wrote a book called “ Come back to Arizona” that is in volume 2 of The Brownie Book. Katherine started her college career at the University of Chicago where she studied anthropology. While in school she became greatly interested in studying dance abroad. On 11/16/15 I spoke with Nelda Shaver the owner/director at the operation. Ms. Shavers stated she was in Queen City at an operation she is opening there. Ms. shavers stated Versie Crossland was in charge while Brittany Shavers was on bus route that day. Nelda stated Brittany called her around 3:00 and told her Daniel had fell and cut his forehead. Nelda stated she then returned to the operation, but by the time she arrived Daniel had already been picked up.
Quinn Hogan was sitting in the living room during his 5 minutes break. When the timer went off, staff Augustine Oloyede prompted Quinn Hogan to the gym for his exercise block. Quinn Hogan started to engaged in property destruction; banging on wall,window, flipping the table and attempting to pass through the living room window. Staff Augustine Oloyede blocked Quinn Hogan from passing through the window. Quinn Hogan then attempted to dart through the main door, and staff Augustine Oloyede called for assistance. When Martin N arrived, Martin N attempted to block Quinn Hogan from darting out of the house. Quinn Hogan started to engage in aggression (pushing, grabbing, hitting and scratching) towards staffs Augustine Oloyede and Martin Nganga.
On Thursday, October 22, 2015, DJS Student Olivia Sierra Marousek appeared to be agitated and requested her PRN to help her settle. When the nurse Angie gave her the PRN pills she took the medication put them in her pocket and went to her bedroom. Candra Jackson followed Olivia to her bedroom and began to process with her using caring gestures and hurdle help to attempted to retrieve the medication. Olivia through the medication on the floor and Candra Jackson were able to retrieve the pills. Olivia exited her bedroom and was able to take her PRN medication. After taken her medication she walked over to the windows on the Thomas Unit and began to bang on the windows and walls, and threatening non DJS students and Candra Jackson.
REPORTER: The reporter/ Social Worker (Crystal) called with concerns for the victim, Ezekiel. Ezekiel has suffered from a stroke, encephalopathy, and his new baseline is that he’s alert but oriented to self only. The reporter said Ezekiel knows who he is, but he gets confused. Ezekiel can perform his daily ADL’s. Mr. Tuders came into the hospital for alter mental status, and he was found to have encephalopathy as well as having a stroke. The victim has a history of alcohol abuse. On 02/03/2016, the reporter had a meeting with the family (Shane and Ali), and they said that it wouldn’t be safe for the victim to come back to the home. Neuville peers approach Twyla about thepPhysical altercation that occurred early in the school day. Staff intervened quickly and removed Twyla off the Unit into the Neuville / Welbers hallway. Neuville and Tours students were transitioning from the center courtyard back to the Unit. Neuville students were in the process of going up the steps leading to the Neuville/Welbers units’ hallway. Tynoura C entered the stairway area, unauthorized and against staff advisement. Tynoura C ran up the stairway and was able to gain entry to the Neuville/ Welbers hallway. The reporting party (RP) stated a week prior to 3/31/17 resident youth Summer Dia Quint while in bed, a staff member named Cole (unknown) forcibly pulled the blankets off of her causing her to fall to the floor hitting her head. The staff member then uttered "I'm done with you!" and walked away slamming the door. After the incident, Summer felt dizzy, however the staff members failed to provide assistance. In addition another staff member named Demarios (last name unknown) grabbed Summer tightly causing her to fall down hitting her ankle, after which she sustained a bruise on her arm from the incident. Ravena Dhami died at the age of 93 from natural causes while asleep. She is survived by four children: Zara, Joshua, Justin and Aliza, nine grandchildren and two dogs. At the time of death, her principal endeavor was to motivate those around her to peruse their dreams. She will be honored for motivating and believing in those around her and be remembered by her children and grandchildren because she always was a person with high energy who loved taking care of her family. She will be missed by family, friends and loved ones because she was loving and caring. She was loved by her family and friends.
