The patient was managing well until 2 months ago when max his 13 year old golden retriever died Maggie his daughter, recently contracted us to as for a visit because she is concerned that he seems to have become more forgetful and weaker, and she thinks his clothes look bigger, so he may have lost weight recently. The nursing care plan should include: Assessment,6 nursing diagnosis,(3 physiological and 3 psychological) outcomes and goals, nursing action and evaluation. (ADPIE) format.
