I was running a production line for thin film optical fiilters for the verizon FIOS build out. The device had 27 layers and if the stress imparted was too high than the films would pop of, my production in line measurements were all within specification but we had massive failure, why?

a) the in line specifications was too wide, the process distribution moved but was within specifications so product was moved on, but was actually prone to failure. The specifications upper and lower limited were modified with new failure data.
b) the inline specification was too narrow
c) Verizon FIOS showed to many bad movies
