In order to keep track all the hospital activities, the hospital management plans to establish a new database management system which are based on the following requirements:

• For getting a treatment, the patient must make an appointment. Patient is only allowed to book one slot of appointment at one time. When the appointment slot has been booked by a patient, other patients cannot book that appointment slot unless the respective patient cancels it. The required details for the treatment are the appointment ID, patient ID, doctor ID, date, time and the treatment description. The patient ID and doctor ID are required due to track the respective patient and doctor who involved for each appointment.

• For each patient, he/she must give his/her name, phone number and address to the hospital for recording purposes.

• While the doctor that is participated in the treatment, his/her doctor ID, name, phone number and the speciality (for example Psychiatrist, Neurologist and Allergist/Immunologist) must be kept in the database. This is to keep track the responsible doctor for each of the treatment / appointment. Each appointment is handled by one specialist doctor.

• Each doctor will be assisted by one or more nurse(s). However, a nurse will only serve to a doctor at one time. The database must keep the nurse ID, name, phone number and his/her position rank.

• For a convenience service, each treatment will be executed in the special room(s). One treatment can be conducted in one or many rooms; and one room can be used for many treatment / appointments. Draw an Entity Relationship Diagram (ERD) for the above scenario. Identify all entities, attributes, relationships, connectivity, primary key(s) and foreign key(s).
