CURRENT CLIENT HISTORY
The client is a 19-year-old African American male with a known history of sickle
cell anemia. He has been admitted to the hospital with severe periumbilical abdom-
inal pain, bilateral leg pain, and shortness of breath (SOB). The client has a 5-day
history of fever, productive cough, and increasing SOB. He is complaining of dry
mouth, and urine output has been low (last urinated 18 hours ago). He was diag
nosed with dehydration and RLL pneumonia in the ED. His pain began 2 days ago
in his abdomen and has progressed to his legs. He describes his leg pain as "an achy
bony pain." He has trouble walking because of the pain. He knows that he is having
a sickle cell crisis on top of his pneumonia. He also notes that he has diarrhea but
reports no blood in the stool.
PAST MEDICAL HISTORY
Sickle cell anemia diagnosed at 1 year of age. Splenic infarct 2 years ago. At least 10
prior admissions for sickle crisis, but at another hospital. Impaired vision in left eye
secondary to thrombotic event 5 years ago during a sickle crisis.
