These factors are called social determinants of health. The US healthcare system has traditionally been one of the most important determinants of health and health outcomes. Social determinants of health must be understood to achieve health equity and improve patient care. Uneven opportunities for health create social determinants that contribute to health disparities.   Purpose of the Study   The purpose of this study is to improve patient equity and use social determinants to improve patient care. Providing good patient care is the most important thing when it comes to healthcare. Creating good health environments for all is accomplished by addressing social determinants. To achieve this goal, healthy behaviors must be promoted, and conditions must be created that support health. Addressing social determinants goes beyond treating diseases; they contribute significantly to the delivery of healthcare. The root causes of health disparities must be identified and rectified before we can create more equitable healthcare. Everyone should have access to healthy living no matter their situation or background, no matter what their socioeconomic status may be.  Objective The objective of the study is to understand the role of social determinants in improving patient care and promoting health equity. By understanding the role of these factors, healthcare providers can identify areas where they are needed to improve patient care. Additionally, they can use this data to create targeted interventions that can help reduce health disparities and promote health equity. As a result, health outcomes will be improved, and health disparities will be reduced. It also seeks to identify the purpose and challenges associated with addressing these social determinants. The importance of understanding how social determinants can impact health outcomes and well-being in healthcare cannot be overstated. To achieve health equity and comprehensive well-being, healthcare must go beyond curing illnesses. Problem Social determinants are no doubt challenging to address. Many factors can contribute to this, including lack of awareness, insufficient resources, unequal access to healthcare, and systemic issues like poverty and discrimination that are difficult to change.
