BWC of incident On 6/16/18 about 1949 hours Sgt D. Comodeca #1790 and I were flagged down by AR Rufus Townsend in reference to an individual being “down” in the apartment next to Townsend. I knocked on the door and there was no answer. I opened the door and went inside of the apartment. While clearing the apartment I saw a male laying down in the floor of the room. There was blood on the bed, floor, and wall near the male. I cleared the rest of the apartment and called Lt. Roof #1525 and advised him of the situation. I spoke to Townsend in his chair outside of his front door and I asked him how he discovered the dead body. Townsend said he was sitting outside of his apartment in his chair around 1000 hours when the male asked Townsend

We approached the apartment complex from the east and met with WPD Sergeant Springob #2240,
I notified dispatch that Officer Albert and I were out at the RSC. I parked our patrol car on the north side of the RSC and entered the north door. I heard Officer Smith on the radio say he was out with the subject near the Credit Union. I observed from across the RSC Officer J. Smith and Officer R. Faison standing next to a black male all the way by the Book Store. As I walked closer, I could see the black male with both of his hands up in the air and he was getting down on both of his knees. When I got to the Credit Union, I saw a set of crutches and a black bag laying on the floor. I told Officer Albert to stay here with these items as I continued to go behind the black male. Officer Faison kept trying to talk to the subject but he was clearly ignoring him. The subject who was later identified as
Patrol officers arrived at 518 N. K Street and surrounded the house. Patrol officers called the victim and the suspect out of the house. The suspect was taken into custody without incident.
On February 9, 2011, at approximately 7:15 AM, I, GBI Special Agent Abigail Wilkes, arrived at 293 Stone Road to process a crime scene involving two deaths. When I arrived, the crime scene was secured with yellow tape. I met with Deputy Sheriff Ray Chandler. Chandler told me that the two victims, Steve Campbell and Sherry Campbell, had two gunshot wounds each. Sherry Campbell died in the master bedroom of the residence, but emergency medical services (EMS) took Steve Campbell from the scene. Chandler informed that the EMS personnel entered the residence through the front door and placed Steve Campbell on a stretcher. Steve Campbell 's injuries created a blood drip trail when he was transported to the ambulance. The drip trail began in the master bedroom, went through the hallway, out of the front door, and into the yard of the residence. Steve Campbell died on the way to the hospital.
Upon our arrival, a Hispanic male later identified as Juan Navarro opened the front door but not the security screen door of the home. I advised Navarro that I was the North Las Vegas Police Officer and was at his home because of the 911 disconnect. I asked Navarro if anyone else lived in the home to which he responded his three kids and the mother(Miriam Garcia) of his children lived there as well. I asked Navarro if we could speak to them to ensure that they were all in good health. At this point, Garcia came to the door and Navarro told me he would not open the door at that they were all "ok". Garcia then opened the screen door and when she did I noticed Navarro had two fresh diagonal scratches on his neck as well as two small cuts on his neck.
I arrived on scene at approximately 02:11 hours. I approached the front door and the victim, Hollie J. McIntosh (F/W, DOB: 07/26/1988) opened the door and let me inside. I inquired with McIntosh what happened. McIntosh stated she was shoved and choked by her boyfriend, James Dean Carvell (M/W, DOB: 01/25/1988). I observed a red mark on McIntosh’s neck. McIntosh advised Carvell was in the bedroom. McIntosh pointed to the bedroom. I entered the bedroom. Carvell was lying
Sgt. Grimm stated that on August 23, 2014, he performed a 1450X2347 tour as the patrol supervisor and PO Derek Almeida was his operator. At about 2008 hours, he responded to a radio run of a 10-34 male stabbed at 37 Weirfield Street. Upon arrival, he observed a male black [Mr. Carlton Smit h Jr.] bleeding heavily. The male black stated [to him] that his father had just stabbed him, while pointing to a black SUV driving away from the scene. Sgt. Grimm stated that he reiterated what the victim had said [to him] and he stated yes, my father just stabbed me. Shortly after, they followed the identified vehicle and subsequently pulled it over on Bushwick Avenue and Hancock Street. Sgt. Grimm Stated that as they approached the vehicle they notice blood all over the driver's side door. Mr. Carlto n Smith was properly identified and placed under arrest. Sgt. Grimm doesn't recalled who the UMOS that were present at the scene.
I first responded to 550 N. Main St. to make contact with Investigator Don Johnson, who was the on-call investigator. After speaking with Investigator Johnson and getting a basic understanding of what we knew so far. I went to Eastar Medical Center to see if I could speak with the victim, Aron Grootenboer.
Officer Thebeau and I were directed, by numerous people, to a rear bedroom inside of 814 N. Garfield Ave. I observed a blood trail from the driveway of 814 N. Garfield Ave, into the residence, and
I entered the residence and was directed to a bedroom on the north side of the residence. I entered the bedroom and observed a white female standing next to the bed. She had a large, blue in color, lump under her left eye. She was unable to open her left eye very much. I later identified her as Anna Williams by Arkansas drivers license. I asked Anna what had happened and she said she was trying to leave the residence and they hit her. I asked Anna who "they" was and she said Gilbert and Judy. Anna said they hit her with their fists. At about this time, Mulvane EMS arrived on scene and started to render aide to Anna as I collected her personal information.
On 7/19/2015 S/O EMT Perez was dispatched to FC-504 regarding a fallen male resident/Water leak. S/O EMT Perez knocked and announced his presence at the door and was greeted by the spouse of the fallen resident in question. S/O EMT Perez was then introduced to Mr. Jerome Bluestein who was up and walking in the apartment with the assistance of his walker without any complain of pain nor was he grimacing in anyway. Mr. and Mrs. Bluestien stated that Mr. Bluestien sat down to use the toilet when the tank broke and started expelling water. Mr. Bluestien did not complain of any pain and denied any care, after insuring Mr. Bluestien was no injured in anyway S/O EMT Perez turned his attention to the leak in the bathroom. As I exited my vehicle i was rushed by two females stating their cousin had been shot, then two other females appeared from the front corner of the house screaming "she's been shot do something". I observed a Black female lying half in the seat half in the floor board behind the drivers seat. I approached the vehicle and noticed the female was still breathing but bleeding profusely from the mouth area. While trying to keep the females out of the vehicle and secure the scene the best I could the female stopped breathing, I then checked for a pulse and was not able to find one.
On 08/20/2016 at 01:55 hours, Officers responded to the report of shots fired at 2121 Bethel Ave, Lot L. Upon arrival Officers located a crime scene in Lot L of Walter P. Taylor Homes. A short time later 3 victims showed up at Fort Sanders Hospital with gunshot wounds and made statements that they had been shot in Walter P. Taylor Homes. 2 of the victims were treated at Fort Sanders Hospital and 1 was transported from Fort Sanders to UT Hospital with more serious injuries (When the victim arrived at UT Hospital ER Staff advised that the injuries were non-life threatening). No suspect information is available at this time. Major Crimes is in the process of interviewing the victims at the time of this email.
I began my investigation by observing the exterior and interior of the residence, for identification purposes. The residence is a one story, “mobile“home facing south at the address above. I observe 2 vehicles in the driveway. I observed no visible evidence on the exterior of the residence or the surroundings grounds. I entered the residence through the back door located on the northeast side of the residence. I had approach to the living room area of the residence I observe the victim was on the living room floor flat on his stomach. The victim was wearing what appeared to be wearing a dark blue T-shirt with no underwire or pants. The victim appeared to have discoloration all though the body, but most noticeable on the face area, left hand and feet. The victim’s body seemed bloated most and the victims face was dark in color.
