CCIB LPA received a call from Theodore and his girlfriend, Linda Lubert. Theodore and Linda were in a skilled nursing (rehab) facility until May 9th, 2015. They were given 24 hours to move and ended up at this facility. When they arrived, they were not appraised, given any care plan or schedule. Both need assistance getting up, dressed and with hygiene, they have to beg for staff to help them. The doors to their rooms are locked at night and they are not allowed out of them. This happens during the day sometimes as well. Since he has arrived he has had to wait five days for assistance with showering, staff say they do not have time or the towels are in the dirty laundry. The reporting party (RP) stated that 92 year old resident Delora Lovelock DOB: 1/15/24 was residing in the facility from 5/15/2015 to 6/4/2015. Prior to admission the facility was informed that the resident had a broken femur and could not bear weight. The resident required two caregivers to transfer. Initially the facility assessed the resident and accepted her into the facility. Subsequently the resident was not provided the care necessary. The resident was transferred by one caregiver that resulted in the resident's trip to the emergency room where she was diagnosed with contusions to her ribs. The RP stated that prior to admission that the caregivers were trained in transferring resident however the caregiver had no idea how to transfer a resident from bed to
CCIB LPA Perryman-French received a call from a parent who was trying out this day care. She brought her one year old child to the program and then snuck around the corner to watch how the child adjusted. (Her child hung on to the gate for a few minutes and then wandered into the classroom. Caller then watch a Ms. Veronica grab a toy (or something) out of a little boy's hand (under two years old) and say "Give me that." Her grab was forceful enough and her tone loud enough that caller immediately reacted. Caller walked away for maybe a half hour, then could not handle the bad feeling she had. She returned and found her child had red swollen eyes and snot on the face, was clearly upset, but the teacher did not comforting, instead, just told
CCIB received SOC 341 reporting that during a meeting with the resident, James Brown (DOB: 09/18/84), social worker from Solano Adult Day Healthcare Center, the resident stated that staff Marco has hit him on multiple occasions. The resident kept repeating that he did not want to return to the facility. RP consulted with resident's previous social worker who affirmed that the resident has a history of making allegations that are not true due. Resident also has a history of Schizophrenia/Delusions. Facility staff stated that the resident has a history of making allegations whenever he is assigned a new social
During interview Ms. Stoker was alert, aware of surroundings, and answered all questions appropriately and independently. During the visit Ms. Stoker dry heaved into a garbage bucket, and visited the restroom once. The worker was at the residence for about 2 hours. Initially Ms. Stoker was shaking uncontrollable to the point of shaking the entire bed. At the end of the visit the shaking had stopped and Ms. Stoker was smiling and interaction with worker while talking about DELETEbeing a wife and mother. During visit Ms. Stoker received a call from Alacare Home Health. She informed them she was out of her pain medicine but did not inform them she was out of all medicine. On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
Background- The scenario in question involves a 77 year old widowed woman, Mrs. Zwick. Mrs. Zwick had a mild stroke, and was admitted and hospitalized as a inpatient for five days to ensure her condition was mitigated. After the five days, Mrs. Zwick was transferred to a skilled nursing facility for rehabilitation. However, Mrs. Zwick was uncomfortable, and tests revealed that she had manifested a hospital-acquired urinary tract infection. This infection was serious enough to require IV antibiotics and extended care for an addition forty days in the nursing facility. Upon discharge, Mrs. Zwick was prescribed several medications and had to get a walker. Due to her age and circumstances, Mrs. Zwick is enrolled in Medicare A, B and D she is not able to handle her bills and paperwork, so her daughter helps out. Mrs. Zwick and her daughter both were unaware that the urinary tract infection was considered a hospital-acquired condition all the nursing staff told them was that it had to be medicated.
CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017
CCIB Intake received e-mail from John Merrill regarding the initial complaint. The information is the same as the LIC802. LA City Fire Department of the location being problematic due to the number of request for medical assistance. There is a resident who is diabetic and a victim of strokes and kidney failure. He is reported to be in need of care and supervision. He was ordered by the Court to be placed at the location and is assigned to a Probation
CCIB LPA C Perryman-French received a call from Raylette (Letty). Her son Jason Laidley Jr. (DOB 03/10/89) lives at this location. Raylette did not know this was an ARF and when the facility was called (three way call with LPA), the facility staff could not or would not provide the facility license #. Lanterman Regional Center Staff provided LPA with the actual name and facility number. Jason wears a cochlear implant, his room mate recently broke it, now Jason has to write down any communication he has with anyone. Knowing this, staff sent him to the store on Sunday afternoon (07/26/15) to buy toilet paper and light bulbs for the facility. (Raylette noted that Jason's IPP also states he is not to leave the facility unsupervised.) Jason apparently
CCIB Intake received a formal complaint letter from California Advocates For Nursing Home Reform (CANHR) Long Term Care Justice and Advocacy stating the facility failed to maintain the required liability insurance to cover injury to resident or guest. CCIB received a SOC 341 via email from Maria Doreen Ainsleigh (medical SW) regarding Terrylee Young (DOB 2/18/1948). RP reported that Terrylee stated that some of the nurses treat her and other residents bad (did not state how). RP reported that Terrylee stated that the staff (names unknown) yells and scream at her and other residents (LPA contacted the SW and Terrylee neither parties could tell me what staff says). RP reported that Terrylee also stated that the Administrator Joe Johnson curses at her and the other residents when they are having problems in the facility. Apparently, the place was in state that the nurse was not able to sit down in his home. There was all kinds of debris on the floors, counters etc. He had a lot of dirty clothes piled up and it was quite unsanitary. The nurse left very emotional due to the state of his trailer/RV. The good news is that Mr. Adams will continue with the DIP program so we can help him that way. In the meantime though, I wanted to see if you were aware of his situation and if there might be anything additional that we could do to help Mr. Adams?
A SOC341 was received in the CCIB office submitted by reporting party Felicia Nault. According to the documents received, 61 year old resident Kenneth Pettit arrived in the dining room and informed staff member Juanita Rodriguez that his breakfast would need to be served prior to the regular scheduled breakfast time of 7:45AM. The resident informed the staff member he had an appointment. The staff member reassured the resident she would do her best to have the breakfast available in a timely manner. The resident's breakfast order was received by staff member Karen Castellon during the scheduled ordering time.
