I was very excited in my effort to locate and connect with the local LMFT chapter. I found out that there was none in my local area. So, I called the American Association of Marriage and Family Therapy in Baton Rouge Louisiana, they gave me a number of an LMFT representative who was located in Shreveport Louisiana which was about one hundred miles away from where I live. I generated a call to the LMFT representative in Shreveport but there was no answer. My next effort was to surf the Internet to search and locate counseling center in my hometown near me that has LMFT’s. I found a LMFT through the Internet and generated a call. Eichelberger was very enriching, we talked about CBT (cognitive behavioral therapy) and other various approaches that is applied in the profession. Also, it was indicated that the Granberry Counseling Center is geared toward a faith based approach which I have found the meeting very intriguing, but due to time constraints Mrs. Eichelberger shard small amounts of her experiences.
The next counseling center that I came to was the Family Solution Counseling Center. I enter into the center and presented myself to the receptionist as a student of Northcentral University showing my student ID card for verification requesting a five to ten minute with an available LMFT if it was possible. The receptionist told me that there was anyone available, everyone was busy. The receptionist asked for my number and gave me a pad to write it on. I wrote my name, number, University, and the reason for the interview then I departed for the next counseling center on my list. I arrived at Monroe Behavioral Health clinic requesting for an available LMFT. The receptionist contacted LPC employee and relayed the message that they did not have any MFT on staff but the LPC employee referred me to a LMFT at a women counseling center. After I left the Monroe Behavioral Health Clinic, I received a call from Family Solutions Counseling Center telling me that there was an available LMFT who was willing to meet with me at a preferred designated time. Marriage and Family Therapists (also known as MFTs) are relationship specialists. MFT focuses on the relationships that the client has and assists them to function as a stable individual and as a relationship parent. MFTs are trained assess to diagnose and treat individuals, couples, families and groups to achieve more adequate, satisfying and productive marriage, family and social adjustment (California Association of Marriage and Family Therapists). According to American Association for Marriage and Family Therapy (AAMFT) Marriage and Family Therapists are mental health professionals with a minimum of a master’s degree and two years supervised clinical experience. Marriage and Family Therapists are trained and licensed to independently diagnose and treat mental health and substance abuse problems. The role of a MFT is to help individuals and families to address issues and guide them in the situations and everyday life so that they can function within society. MFTs provide a wide variety of services for clients. Some services that MFT provide but are not limited to is diagnosis and treatment of mental and emotional disorders, marriage and relationship counseling, and pre-martial counseling. SC received a telephone call on 10/16/2015 stared 9:34 and end at 9:41 am from Tricia Crooks at Liberty Resources Home Choices (LRHC) Community Outreach and Enrollment Leader. Stating that she spoke Pa and he wants to resume his service order with LRHC for PAS service. SC informed SC that this information will first need to verify with Pa. SC expressed concerns about LRHC being able to fulfill service since they had the case unstaffed for over two weeks (09/25/15-10/15/2015). Tricia apologized on behalf of LRHC, and stated that they have someone assigned and is ready to go all is needed is the resumed service order ASAP. On 7/11/2015, CM did a visual and had client come to the social service office. CM completed Bi-Weekly ILP Review. In the meeting client appears to be wear out, and tired. She was constantly throbbing her forehead, like if she was having headache. CM inquires what the problem is. Client replies “she doesn’t like the shelter food and sometimes she doesn’t eat” CM advised the client to eat and nourished her body. CM also observed that client is depressed but she continues to refuse medical referral to see a psychiatrist and medical doctor. Client continues to mention her son who is in foster care, and the physical altercation she sustained many months ago here at this shelter. First, we will examine my initial clinical note on 8/15/16 with client, Rhonda Smith, session one. During this first session, I collected Rhonda’s demographic and intake information (Murphy & Dillon, 2015; Reamer, 2001). This included a signed consent for treatment, which we reviewed and all her questions were answered, as well as signed medical releases for previous therapy and agency records, i.e., DVIS, CPS, CASA, that will be requested (Murphy & Dillon, 2015; Reamer, 2001). Additionally, she was informed about HIPAA, patient privacy rights, billing practices, professional boundaries and expectations, and how to contact me during business hours, and after-hours crisis lines, and on-call assistance phone numbers for resources if it is outside of my business hours (Murphy & Dillon, 2015).
Reporting party (RP) stated he arrived at the facility to meet with resident George Dixon (7/11/52) on 3/29/17. Resident was recently placed at the facility after being released from the hospital. Staff did not allow RP to enter the facility so that he could conduct his metal health assessment for the resident. RP presented his business card from his agency but was still denied entry. The resident has been enrolled in RP's program since 02/01/16 and has been visited previously by Social Worker Kathy Kleinman and by staff Denise Ramirez on 3/10/17. Staff told RP that he has to contact the administrator to make an appointment to visit with resident. RP's agency faxed over the consent for treatment to the facility and the DMH contract. Patient's

Client continues to report if the Mental Health Counselor at Woodhull Hospital need to contact her and tell her why she need to be seeing and to show documents. According to the AAMFT, the therapist’s ethical responsibilities is that a therapist needs to non-discriminatory. The therapist needs to obtain informed consent to clients to an legally authorized person (AAMFT American Association for Marriage and Family Therapy, 2016). Therapists cannot abandon their client, and need to make arrangements for continued treatment (AAMFT American Association for Marriage and Family Therapy, 2016). The counseling relationship needs to be built on trust, the therapist much keep confidentiality for the client disclosures (AAMFT American Association for Marriage and Family Therapy, 2016).

The writer accompanied the consumer to her intake appointment at Northeast Guidance Center. The consumer came to the appointment in the clothes she slept in and did not wash her face or shower. The consumer was defensive during the intake appointment with one of the therapist. The consumer would not speak up and often time would have the therapist repeat the questions. Dr. Warren’s approach was very calming and warm which had a very positive effect on Erika. While watching the video you see the calming effect Dr. Warren’s approach had on Erika by watching body language. Erika was very engaged during the session she talked about confidence and when responding to Dr. Warren’s questions she gave good eye contact. She put thought into all her responses and there was even a time during the session when Erika stated explained how she needed more time to answer a question. Which told me she was answering all the questions honestly and was show value in this therapeutic process.
The testing center is located in building D, at the back of Admission office and border of counseling office. The office interior was looking pretty neat, quiet and the wall covering with full of clear glass. Also, the external of the office was creatively arranging with many comfy sofas and tables. I had brought my friend along with me to Testing Center to gather more information about the services provided. There were two staffs sitting at the help desk at the time we entered. I was sort of nervous to open the talk. As soon as we reached the desk, the lady named Maria has given us a smiley face and offered us for an assistant. It wasn’t how I expected at all because most of the staffs wouldn’t take the walk-in student. We started the conversation by introducing our name to her. Then we explained to her that we were given writing profile essay assignment from our instructor thus if she is willing to support us by contributing some information about Testing services. She
