7/13 Worker went to the home of Crystal Sebastian. Worker knocked on the door and Crystal greeted the worker and invited the worker in. Crystal had a quarter sized hicky on the left side of her neck. As the worker and Crystal walk into the apartment Crystal shows the worker Josilynn's hover board for her birthday. The apartment was unorganized and messy. Worker and Crystal sat down in the living room to discuss court, ACR and Services. Crystal is working for the news paper route currently and seeking employment. Worker spoke about visitation and gave Crystal the 502 forms to fill out. Crystal spoke about visitation supervised and unsupervised. Robert jr is a registered sex offender stated Crystal. Her mother Gwen has DCFS involvement due to her sister was sexually abused by Crystal's step father. Worker spoke about how Gwen may be at fault because of not keeping her child safe.
Worker has requested information for Crystals chestnut and probation for the up coming court hearing, but has not heard back. Crystal states that her probation officer has been on vacation.

Crystal stated that the foster father is not talking nice to Josilynn, Josilynn reports. Josilynn reports that the foster father is saying, "none of your nosey ass business". Worker will address without telling the foster parent what was said, speaking about age appropriate conversations. Crystal reports when the worker informed the foster parent not to cut Josilynn's hair, the foster father told Josilynn that her mother should worry about getting the children back rather than them taking care of the children. Mr Finley stated that she made server attempts to contact her case manager, Kathy, to reschedule the home visit for another day, but Kathy did not return her calls for almost a month. When finally Kathy returned her call, Kathy was not willing to reschedule her home visit. Ms Finley stated that she asked Kathy if she listened her several messages, Kathy did not like that and became rude.

On this date worker received a call from Ms. Debra Dawkins, nurse at Amedisys home health, for the purpose of monitoring Ms. Brown's apartment application. She questioned worker about Ms. Brown's Oakman apartment applications. Worker informed Ms. Dawkins that she was not participating in placing Ms. Brown into an apartment. Ms. Shelly and she had talked about the arrangement and Shelly stated she would talk things over with the doctor and have Amedisiys SW to assist with the process, only if the Ms. Brown was capable of caring for herself.

On 9/21 /17, the worker went to home Mr. Jeffrey McCulley to complete a home visit for Tristan McCulley, who temporary is residing with his father and step-mother, Katie McCulley.
