Charlie Kelmeckis’s Assessment • Identifying Information: Charlie Kelmeckis is fifteen-year-old freshman in high school. Charlie identifies as a straight white male. His family, who recently found out about his traumatic past, is worried about his mental health and has referred him to us. His parents mentioned that Charlie showed no unwillingness or hesitation on their request for him to receive therapy. He had no visible physical disabilities and when asked he declared he had none. • Presenting Problem/Symptoms: Client’s aunt died in car accident when he was seven years old and feels responsible for it. • Medical Information: Charlie has no history of major surgeries, head injuries, or chronic illnesses. He currently takes medication for his anxiety he was diagnosis with last year. o Substance use history: Charlie substance use began when he started his freshman year in high school when his new friends introduced it to him. He has experimented with marijuana, LSD, and alcohol. Client has no preferred drug of choice and only participated in substance use when it was given to him on a few occasions. When on LSD he began to hallucinate and the flash backs of his aunt caused him to pass out on the side of the road where the police found him the next morning. No charges where filled. • Family History: Charlie grew up with his mother, father, one older sister, and one older brother in the suburbs. His father’s attention is most often on his older brother due to his football dreams. Charlie feels distant from his father, but when they do get time together, they have a good time. His mother was described as loving and constantly worried about Charlie. Charlie description of his 20-year-old brother, Chris, is a really good older brother who always defended him. Chris often fights with their sister who is the middle child, and his dream is to be a pro sports player. Charlie is very protective of his older sister who continues to be in abusive relationships. Client comes to treatment because she has been mandated by the court to receive services for her drug and alcohol usage. Client self-reports an extensive history of drug and alcohol usage, as well as, issues with controlling and maintaining her anger. Client is more concerned with her anger issues then her drug and alcohol usage. Client feels that if she can control her anger then she will not have to turn to drugs and alcohol. Client appears to be self-medicating with drugs and alcohol.
Charlie shows symptoms of post traumatic stress disorder and anxiety. He seems to also suffer from depression, which can root from PTSD. The symptoms that were correct of PTSD were having flashbacks of traumatic events. The symptoms of anxiety that were correct were worrying about feeling guilty, and not becoming happy. The symptoms of depression that were correct were blaming yourself for everything, and always putting yourself down. The things that were realistic about PTSD were having flashbacks, feeling guilty, not wanting to become happy, blaming yourself, putting yourself down, and worrying; for example, Charlie became mentally unstable as a result of being sexually abused as a child. For my diagnosis, I diagnose Charlie with having clinical depression and post-traumatic stress disorder. The reason I believe that Charlie has depression and PTSD is because he had exhibits symptoms and certain behaviors throughout the film. According to Mayo Clinic (n.d.), Depression can be defined as one who has persistent sadness or anhedonia with an addition of other symptoms that can affect the person’s thoughts and behaviors. Substance abuse is a tragedy that touches many lives. Abuse begins with a single use event that, with continued use and overindulgence, transforms into a battle. The abuser most always loses that battle. Personal relationships, social ties, and employment suffers. Irresponsible and erratic behavior becomes the norm, and though the abuser is aware on some level of the reckless and thoughtless acts that they commit, they continue to use and abuse their drug of choice. Mr. Charles was arrested for the possession of cocaine. Mr. Charles stated that the cocaine that was found was his friend. The drug test may imply that he has recently consumed cocaine. Although Mr. Charles scored a four on the Michigan Alcoholism and Screening Test (MAST), he seems to be at risk of chemical dependency. Mr. Charles has stated that he is constantly around drugs and alcohol. This could be a concern as he attempts to stop consuming alcohol or drugs.
Client appears to be very sad in regards of the gunshot incident, and the fact that her friends were involved in the incident. It appears to be that although this incident did not happen to the client directly, she is very distressed by it, and she feels a lot of pain and sadness to accept this event. Client appears to be hesitated to share her emotions with her best friends in regards of the incident. It seems like client has difficulties to accept that her friends use humor to cope with the gunshot incident. It appears that client is angry at her friends' parents based on her description although client further denied it. Client seems to want to control over things that she does not have control of such as when client mentioned that she does not want her friends to go back to work yet. Although client is feeling a lot of distress from the incident, it appears that client is aware of the importance of self-care. Client would likely benefit from increase social support, and self-care
Throughout the session the client was able to give me a timeline of his substance use. He kept looking down on the table. The client showed strength by recollecting his substance use history and his behavioral problems.
She remember enjoying the experience but reports continued experimentation with drugs two or three times per month until college there she stopped drinking following a binge-drinking episode prior to leaving for college in which she reports drinking until she passed out. Following this incident, she reported finding drinking sickening. Then drugs came into play. She now reports using drugs primarily on occasions, along with alcohol. Last use was two months ago, one glass of wine at a holiday party. No current abuse or dependency on drugs and alcohol. CPI arrived at the family residence located at 149 S Calhoun Avenue, Eatonville, FL 32751. CPI completed a joint visit with Taina L. Ramos FIS Counselor. Taina completed a BHC screen with Mr. Clayborne. Mr. Clayborne admitted to have a history of alcohol misuse and impulsive and aggressive behavior while under the influence. He began drinking while in his 20’s and drinks 1 to 2 times weekly. Mr. Clayborne drink preference is beer, but sometimes drinks vodka. He usually drinks when is stressed out. He has been to Alcoholics Anonymous meetings in Springfield, MA. The last meetings he attended was about 2 years ago. While in prison was seen by a psychiatric five times a day for 3 months, but is it not diagnosed with a mental health condition.
This writer met with the patient to address her non-compliance with treatment, referring to her attendance to the cocaine group and her AWOLs. This writer explained to the patient about the purpose of the clinical intervention as the patient continues to test positive for cocaine, opiates, and fentanyl. The patient appeared to be surprised to learn about the fentanyl results in her UA's and believes that her "dope" could have been laced. The patient is aware of her non-compliance and made excuses by reporting that she tends to oversleep as she often times cannot hear her alarm from her cellphone. In addition, when the patient AWOL from the clinic, she tends to relapse and her most recent was relapse was on 5/29/2017-3 bags of heroin by inhalation.

The client tested positive for both psychoactive drugs during a mandatory routine screening administered by her employer at a local grocery store. She indicated smoking marijuana (pot) every day and uses cocaine typically on the weekend. However, the client has an increase tolerance level of cocaine as evidence by her stating she is using "more and more cocaine to maintain the same effect.
