Chronic Blepharitis

Chronic blepharitis is a very common cause of ocular discomfort and irritation. The poor correlation between symptoms and signs, uncertain aeitiology and mechanisms of the disease process all combine to make management difficult. Anterior blepharitis affects the area surrounding the bases of the eyelashes and may be staphyloccoccal or seborrhoeic. Posterior blepharitis is caused by meibomian gland dysfunction and alterations in meibomian gland secretions. It is commonly thought of as a more persistent and chronic inflammatory condition than anterior blepharitis and there is an association with rosacea1.

They found that both treatments relieved signs and symptoms of posterior blepharitis after 3 weeks and that there were no significant differences between symptoms healing rate and foreign body sensation healing between the two. However, topical azithromycin was more effective in reduction of eye redness and doxycycline was more effective in meibomian glands plugging healing and reducing corneal staining8.

The results of these studies indicate that macrolides affect several inflammatory processes, such as neutrophil chemotaxis, the oxidative burst and apoptosis in phagocytes, and the production of pro-inflammatory cytokines. These agents decrease neutrophil chemotaxis, possibly by the downregulation of interleukin-6 (IL-6), IL-8, and tumor necrosis factor-α (TNF-α).5

It is thought to exert its antimicrobial effect by inhibition of protein synthesis. It prevents the binding of amino-acyl-tRNA to the messenger RNA-30S ribosomal subunit.11

Hypersensitivity to doxycycline, or any other ingredient in doxycycline tablets.
Children under 12 years of age The use of drugs of the tetracycline class, including doxycycline, during tooth development may cause permanent discolouration of the teeth (yellow-grey-brown). This adverse reaction is more common during long term use of the medication but has been observed following repeated short-term courses. Enamel hypoplasia has also been reported.
Pregnancy. Tetracyclines given during pregnancy affect teeth and skeletal development.
Nursing mothers. Tetracyclines are excreted into milk and affect teeth and skeletal development.11

For the treatment of acne vulgaris the recommended dose is 50 mg of doxycycline taken daily with food, for up to 12 weeks.11

Tetracyclines and erythromycin are the most commonly used oral antimicrobials. In many cases, long-term treatment is needed to maintain remission. The long-term use of antimicrobials is not well tolerated, however, because of side-effects, including gastrointestinal intolerance,
