Health care is continuously changing. In the past several years there has been a large shift in the focus in hospitals. The focus has changed from the staff to patients, patient satisfaction, and cutting costs in order to budget more tightly (Tarrant & Sabo, 2010). Because of this, along with the extreme increase in a number of patients seen and the acuity of the patients, nurses are being overworked. As a result, role conflict and ambiguity is becoming an issue. The purpose of this paper is to discuss role conflict and ambiguity, its significance, and its application to nursing.

When contemplating health care policy changes, several economic issues in health care must be considered. These include the financial issues affecting the health sector and have an impact on health policies. Policy makers face unending challenges due to the health sector revenues that are always rising. Another challenge is decreased funding and failure of the health insurance services.

Health trends in the United States indicate that there are several real problems trending. First of all, the United States population is aging at a fairly rapid rate which means that the problems associate with old age are becoming more prevalent. Another issue that country must face is that obesity has become an epidemic that needs to be curtailed or the health system will not be able to help the people who are destined to adopt diseases associated with increased weight. Health care reform and health care itself has been a debatable subject for quite some time. Health care is a major issue that is found not only in the United States, but around the world. Controversy has gone around this subject due to the causes and effects of the reform and the matter of not being successful as many hoped. The World Prosperity Organization and the Institute of Medicine have two intriguing viewpoints about health care reform and what is needed to be done. The World Prosperity Organization touches bases on medical negligence, premedical education, money, mindset, medical school and the health care system itself. The Institute of Medicine focuses on the need for healthcare to be safe, effective, patient-centered, timely, efficient and equitable. As the viewpoints are noticeably different, they still wrap around the idea that health care reform has room for improvement to be more successful and not have a negative impact on people around the world. The both agree that something needs to be done and done soon.

As the baby boomer generation begins to reach retirement age, there has been much speculation as to what affect they will have on the health care industry. The baby boomer generation is the group of people born between 1946 and 1964. At one point, there were more than 76 million of them, of which 4 million died before 2000. With all this in mind, I will discuss what specific effects they will have now they are getting to that “elderly” stage.

Certain trends in demographics have to be carefully monitored in order to forecast those changes that may be needed in certain areas based on changes. Also, research indicates that healthy, well-insured patients tend to over-consume care without meaningful co-pays but poorer, sicker patients can be deterred even by moderate co-payments and suffer negative health consequences (Kane et al 1994). The overall health care industry has undergone fundamental change over the last decade. Most of the changes have occurred within the underlying business operation of the healthcare industry. Legislation in particular has had a profound impact on the health care industry. First, due to the Affordable Care Act of 2010, the nursing profession is undergoing a fundamental shift in regards to the patient experience. The U.S. health care system is now shifting the focus from acute and specialty care to that of primary care which requires a shift in business operations. Also, due primarily to that aging of the baby boomer generation, the need for primary car overall is shifting and will be needed heavily in the future. The last 10 years in particular has seen an increasing influx of retiring baby boomers that subsequently need care. A positive impact on the ACA legislation is that more individuals are now insured. As such, the need for primary care will also increase over subsequent years, particular within the minority population. This patient centric approach will require more care predicated on specific communities in a seamless manner. Furthermore, primary care physicians will be in high demand over the coming years.

Differences in perception between patients and clinicians on the severity of OM has been a major limitation in optimum management. In patients, the mildest symptoms can trigger discomfort, whilst clinicians look for more complex manifestations such as infections. The time at which patients are assessed may also contribute to variations in reporting. Therefore, the use of a tool that is able to accurately measure both the patient and clinician’s concern is required. Indeed, in the current study, an overall positive correlation between OMDQ and WHO was observed. This was further supported by positive correlations for each component of the scale. This is not unexpected since the WHO scale incorporates both objective assessments of the oral cavity and subjective assessments of the condition of the patient.

In fact, Voutsas (2011) argues that the U.S workforce is the most demographically heterogeneous workforce in the world and he believes that this is due to major changes and diversity .Borkowski (2012) also states that the significant changes in the US populations has been seen greatly in regards to gender, age ,and race and ethnicity .
