As second-year nursing students taking a counselling course, this was our second of two reflective journals about our journey of change. Using the transtheoretical model of change (Prochaska & DiClemente, 1982), on my first journal, I reflected on how anxiety affected different aspects of my life. I also identified myself as being ambivalent – a state of indecision, towards my ability to create healthier behaviours due to the stresses of nursing school (. However, I failed to acknowledge some of the underlying causes of my anxiety, and how they created thinking traps that enhanced negative thoughts and feelings. Thus, for this paper, I built upon my last journal by sharing how I moved towards the preparation stage of the transtheoretical model of change while using self-reflection, cognitive-behaviour therapy, systematic desensitization, and stress reduction. Lastly, I explored what I have learned about myself and the challenges I have encountered over the course of this project.
My first memory of experiencing anxiety was during my parents’ divorce. I was afraid of what would happen to my sisters and me. In addition to my parents’ divorce, I also started to feel anxious in school because I did not fit in with my peers due to having Asperger’s syndrome and feared that they would bully me. The Nursing and Midwifery Council (NMC) defines the role of a mentor as a nurse with specific training who is able to facilitate learning, and supports and supervises students in a practice setting (NMC 2008). Mentorship plays an integral part in the next generation of practitioners and nursing professionals play a vital role in guiding a process that allows the transference of knowledge, skills and attributes from healthcare professionals to the students they are working with (English National Board and Department of Health 2001).
On Tuesday, November 14th, I saw the patient F.O who came to the hospital for a 3 month prophylaxis. Patient does not have a history of hypertension and during his last appointment her BP was 120/50, but during this visit her blood pressure was high (BP: 147/68). The patient was informed of the hypertension and recommended to see her physician. Her ASA classification is II due to her high blood pressure. The patient reports no known allergies, and that she takes vitamin C and calcium supplement. Her last radiographs were taken in 2016 and patient was asked for her consent to take new 6 BWX. The purpose of this article is to provide nurses with a better understanding of nursing informatics in order to enhance patient care. It is essential for nurses to realize the significance of nursing informatics to their speciality. Nursing informatics can be beneficial to health care providers by developing innovative technologies, disease management, evaluating interventions, identifying data trends, monitoring patient outcomes, and patient education. Nursing administrators use nursing informatics to support clinical decision-making support, cost reduction, improve workflows, budgeting, costs, and savings. Additionally nursing informatics can assist and support nursing research by assessing patient outcomes, evidence-based practice, fundamental knowledge, and standardized terminologies.
The computer system created a dilemma for me. Even though I had been in urgent care before, the layout was different. It was not allowing me to access patient information much less open charts. The computer system was giving me access to a different clinic in town. I informed my preceptor immediately. However, my preceptor didn’t know how to fix it. My preceptor than asked another nurse for assistance, which she was able to fix the current issue. As the day progressed there were other computer issues I came across to. For example, the computer wasn’t allowing me to chart medications that I had given with my preceptor.
In nursing, Lewin 's change theory is frequently used. His theory contains three stages: the unfreezing stage, moving stage, and refreezing stage (Mitchell, 2013, p. 32). Lippitt 's theory is based on Lewin 's theory and includes seven linked phases the nursing process and uses similar wording (Mitchell, 2013, p. 33). Children pretend and use their imaginations to carry out their inner dreams. From playing doctor to wearing a firefighter’s hat as children we already have it set in our minds the roles we want to play in our adult lives. As a child I always had the desire to help people in need. If a friend fell while playing tag I would stop to help, when my younger siblings would cry I would always offer my candy and a hug. I learned that helping others made me feel good. It was something I loved. While becoming a woman I only embrace this love even more. Although I did not comprehend that these small gestures as a child would soon lead me into a path of nursing, I am truly aware that they have opened a place in my heart for a career as a nurse.
Electronic documentation provides clarity in orders for tests and medications as well as the ability to identify and properly bill for services rendered. The change to electronic documentation from paper charting is not an easy task, but one that is needed to remain current with the needs of healthcare, and will lead to improved medical records. There are five stages of change from the transtheoretical model of change and these are precontemplation, contemplation, preparation, action and maintenance. This model could be applied for many situations, for this instance, Jeff who has a high salt intake is being analyzed and compared through this model. First is the stage of precontemplation where a person may not be aware of his or her problem and may not even think about changing their ways. Jeff may not have the intention of changing, most likely because of the fact that he does not know that he has a problem of consuming too much sodium. The goal of the practicum with Professor Oakley, with Joining Forces for Families in Dane County, was to help me learn how to provide health education to the community. This practicum was a process, in which I had to learn to develop some skill such as listening, and talking to people from different background, adapting my views and my level of comprehension to the community.
I have taken several actions to improve my tracheostomy care skills and subsequently time management skills without having to rely on first hand in hospital experience. I conducted a literature review on the George Brown Library website (EBSCO search engine) for learning strategies that could help nursing students improve their clinical skills outside of the clinical setting. Throughout my years as a Gambian native from West Africa, I witness the lack of health services across the country as an adolescent. From there on, I decided to pursue a profession in the medical field. In 2014, I made the choice to apply to the nursing program at Towson University. I vividly remember rushing to the mailbox to open the letter, that would have read “congratulations, you have been accepted”, but for me, it was the opposite of what I had anticipated. A friend of mine informed me that the size of the letter indicated if I was accepted or not. Reading I was denied broke me into piece lead me to pursue another degree. Constantly, I thought to myself, what I would do with my future and how my parents will feel. It was already difficult to pay out-of-pocket every semester and work part-time while being a full-time student. Nursing is my gateway to accomplish other endeavors that would guide me to becoming a leader. My plan in life is to earn my Doctor of Nursing Practice (DNP) and also serve as a leader within the health community. After a few months, I assure myself that everything happens for a reason.
This assignment will present a nursing care study of a patient on a cardiac ward. The patient will be referred to as Ann to maintain confidentiality (NMC, 2008).
