We have not required group attendance on this unit for the last couple of months, but we do turn off the television and inform them that not participating in groups has to be documented in the record of care. Most of those that decline group activities are those that would be most likely to disrupt the groups, so it has worked well so far. I am concerned about how we can safely navigate the patient doors being unlocked all day with the setup of our units.
As a clinic manager, I have seen where collaboration failed because of one untrustworthy team member. She was untruthful of her professional and medical background, and so was hired to work full time at a local clinic at BCW. While employed, she took to her personal needs, and failed to accomplish her responsibilities as a medical provider. This resulted in clinic closure due to financial down turn and other issues. This program would be a resource at an accredited women's shelter in Milwaukee. During this time, childcare will be provided by volunteers at no cost to group participants. Sessions will be an hour and a half long and will incorporate a group therapy session, career assessments, and processing time for individuals to gain more information about the self and potential careers. Workshops would meet once a week to help women living the transitional phase of life. Group counseling sessions and individual counseling sessions are the primary method of delivery. This will encourage individuals to create a network of support during the week for times outside of the program.

Group activity included the following: learning personal high risk situations and early warning signs of relapse, identifying personal internal and external relapse triggers and strategies to cope/prevent relapse. Today actually turned out to be a pretty decent day. I picked to be on the adult unit. I was extremely nervous to say the least. It turned out to not be as bad as I had expected. The unit I migrated to was unit A. They house the patients who are in detox and those who suffer from depression. Unit B house patients with illnesses such as schizophrenia. After lunch they merged the two units into one because unit A only had 5 patients. The staff who work with unit A told said they didn’t think it was a good idea to let the two groups mix. One staff member I worked with said that the last time it happened it did not turn out great. She also told me that once their census go back us and they are released to go back to their separate unit that it is extremely difficult to get the patients back calm.

Teamwork and communication go hand in hand especially in health care environment. I was very interested to learn about wicked problems and curious to learn more on how to deal with such issues in a working environment. I also found it interesting when reading about “mirror neurons”, I should pay attention more to it. Having a positive mindset and relaying that to the team is very crucial. There is increasing evidence to show the combination of substance use disorder treatment and association with a mutual-help group (e.g., Alcoholics Anonymous, Narcotics Anonymous) increases the ability of individuals to remain abstinent in the long-term (Ferri et al., 2006; Moos & Moos, 2006a; Tonigan et al., 2000). There is also research that supports the effectiveness of changing social networks and embracing a mutual-help group (Bond et al., 2003; Ferri et al., 2006; Litt, Kadden, Kabela-Cormier, & Petry, 2007; Pagano, Friend, Tonigan, & Stout, 2004; Tonigan et al., 2000). One study reported that, in a 15-month period, AA attendees reported 70% days abstinent versus 62% of those who chose to not attend AA meetings (Litt et al., 2007).
