On Thursday, September 3, 2015, I was dispatched to 3457 Hillsborough Road (Kroger Store) on a report of a Larceny call. Upon arrival I was met by Mr. Dominic Dyquan Henry, the store manager on duty. Mr. Henry advised me that a black male with a backpack entered the store and stole the listed items. Mr. Henry advised the suspect was seen by an employee, stuffing items in his backpack. The suspect then walked towards the exit. (See “Property” section for a complete description of the item.) The suspect fled on foot and was last seen traveling north on Hillsborough road towards downtown. Mr. Henry advised the suspect was a black male, 40-50 years old wearing blue jeans, a gray tee-shirt. He further advised the store does have surveillance
Upon arrival, I made contact with Walmart Loss prevention Officer, Fabiano Estrela, who was located in the loss prevention office with the subject, later identified as Nathan. The subject was cooperating with loss prevention and he willingly agreed to wait for the police.
While conducting the business check, I was alerted by Walmart Loss Prevention Officer, Marion Edwards, that a White, GMC Yukon, that was missing the front bumper was involved in an attempted theft of a coffee maker approximately two weeks prior. Edwards requested that I make contact with the vehicle, which was parked in row #7 in the Walmart parking lot. Edwards was requesting that I issues a trespass warnings to the vehicle's occupants.
Nottingham Forrest was playing Liverpool in the FA Cup semi-final and fans from both sides were excited to watch the match and hope their team would be victorious and qualify to the final. However, the 15th of April was a day that would go down in history as a day of tragedy and loss. Saturday, the 15th of April was the day of The Hillsborough Disaster, a human crush, that caused the deaths of 96 people and injuries of 766 more, changing the lives of thousands of people forever.
The Hillsborough tragedy is the worst tragedy in a stadium in the history of United Kingdom that resulted in the death of ninety six people. Disasters such as the Hillsborough tragedy do not just happen out of the blues but, are a sum of various mistakes as well as negligence or misjudgements. When these factors come together, the end result is a disaster that involves loss of lives and life threatening injuries. Before any event organizer or premises owner agrees to enter in such massive liabilities, it is important that all systems are critically evaluated in order to address any lapses in structural capacity, design or even security and management aspects that could result in human injury or death (The Hillsborough Stadium Disaster, 2009).
In December of 1996, Debra McCann and her two children were out shopping at a local Wal-Mart. The family was there to exchange and return merchandise previously purchased. After checking out late in the evening, the family was stopped at the entrance of the Wal-Mart store by an associate who thought that one of the children had been caught shoplifting previously. They were told that the police had been called and were told that they were not allowed to leave the store. Instead of actually calling the police, the store’s security officer was called. Sure that this was a case of mistaken identity, Debra McCann offered to show her identification, but was refused by the
Castro said the male suspect was wearing a grey shirt, black shorts, and pushing a shopping cart. Castro said the female suspect was wearing a white top, dark shorts, and carrying a large TJ Maxx reusable shopping bag. Castro saw the suspects go to different sections of the store concealing items in their shopping bag. The suspects attempted to leave the store with the unpaid for items. Castro contacted the suspects at the front entrance of the store. Castro told the suspects to give the items back and the male suspect handed Castro the items and the female suspect dropped additional loss on the ground. Both suspects left on foot south on Lake Ave.
On December 20th at approximately 5:30 p.m. A Hispanic male and a Caucasian male walked out of the Galloway ShopRite with carts full of unpaid merchandise. The Hispanic male took approximately $323.20 and the Caucasian male took about $409.18 of unpaid goods. Store managers went after them, and were able to recover $72.90 in merchandise. The remaining of the stolen merchandise could not be recovered. Management immediately contacted store Security personal, Victoria Winder, who call the police and took pictures of the license plate of the Toyota 4Runner the individuals took off in. The suspects were later identified as William Terpak Jr. and Jocsan Moradel-Flores using ShopRite’s video surveillance.
On 010/05/2016, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately1415 hours, officers were dispatched to the Rhatigan Student Center for a suspicious character. Dispatch notified all officers that they were still gathering more information at the time but the suspicious character was a black male with crutches. They also said the subject was yelling someone had a gun.
On 4/27/2018, I was conducting a business check of 8701 US 19 (Walmart) at which time I was approached by business loss prevention employee, Steven Uliano. Steven advised that a male subject identified as being Michael King was at a self-check out register and previously trespassed from this location.
A sketch of one suspect has been released, The man is described as white male, believed to be in his 50s, standing 5’ 7” and weighing 200-210 lbs. with very “pale” skin. He was wearing dark “Docker” pants with a pair of sandals. The man had long grey hair in a ponytail, with a long grey beard, braided in the middle.
Wit-1 described the suspect as a m/w, 20/30 years old, 140-150, light brown hair, wearing a maroon sweater, and unknown color baseball hat.
On April 18, 2016, at 1744 hours, I responded to Froggie’s Discount Liquor Store located at 1009 White Horse Pike for a report of a customer harassing other customers. I was further advised that the white male left the store on foot, traveling north on the White Horse Pike wearing a green jacket, blue jeans and carrying a white bag.
On 02-28-16 at 1333 hours, Officer Saunders #7720 and I were traveling east on Mountain Street approaching Lake Avenue. Officer Saunders saw a male White later identified as Suspect Ronald David Laurent, seating in front of the Popeye’s Chicken located at 899 N. Lake Avenue. Officer Saunders and I parked and exited our patrol unit.
Often regarded as one of the worst sporting disasters ever in the UK, and one of the worst in European football. This disaster involved 96 Liverpool and Nottingham Forest fans being crushed (to death) at the Hillsborough Stadium during the 1988-89 FA Cup semi-final. .
Disasters come in many shapes and sizes. While many are unavoidable, such as natural disasters, disasters with a largely human element can be prevented. While disasters often seem unpredictable, the clues are often there in front of us so long as we have the ability to recognize them and the ability to do something about them. This analysis explores the advantage an ergonomic approach provides in uncovering these clues and how that approach can provide creative solutions that benefit both the individual component and overall system function symbiotically.
