The main objective behind the Affordable Care Act was to ensure that affordable health care insurance was available to every U.S citizen. The policy issue that I have selected to discuss herein is the pay-for-performance payment model. I feel that this impacts a large number of our population and changes in this regard should be made. There is a growing trend in the United States called pay-for-performance. Pay-for-performance is a system that is used where providers are compensated by payers for meeting certain pre-established measures for quality and efficiency (What is Pay-for-Performance, n.a.). We are going to be discussing what pay-for-performance is. There are different aspects of pay-for-performance which include; the effects of reimbursement by this approach, the impact cost reductions has on quality and efficiency of health care, the affects to the providers and patients, and the effects on the future of health care.
The new healthcare reform act recently passed will be fully implemented by 2014. Every person living legally in the United States will be guaranteed, under the Patient Protection and Affordable Care Act, (PPACA), healthcare insurance. Across the United States, primary care doctors are already preparing for the full impact this will have on their practices. In keeping with the promise made by President Obama, physicians are already planning to increase their hours while trying to maintain patient care.
The payment incentives provided to healthcare providers under the PPACA will most likely lead to improved healthcare quality. In addition, reimbursement will shift from being based on quantity to being value-based which will in turn incentivize providers to provide higher quality care.

We still have many barriers to overcome if we to provide coverage and access to everyone who cannot afford it. Since I am earning a degree in Health Information Technology and already work in the health care industry this article is relevant to my field of study. Everyone knows that going to the doctor or hospital in the United States can cost a lot of money out of pocket, whether you have insurance or not. Today’s modifications in healthcare reform merits nurse executives to form associations with healthcare providers to remain viable and take a proactive stance in healthcare. Nurse executives must possess capabilities that can span the patient care involvement to assure the favorable outcomes. In order to foster the changes, nurse executives will need the skills found in the American Organization of Nurse Executive (AONE) competency domains which include communication and relationship-building, knowledge of healthcare environment, leadership, professionalism, and business skills.
Over 54% of Americans are against the Health Care Reform act and I am part of that percentage. This reform is asking Americans to give up some of their freedoms or risk getting a penalty. Many parties/groups are against this reform act as well and the following paper will show some of the arguments they have against Obamacare.
Pay for performance (P4P) system improves the quality of care by creating a patient-friendly, respectful, more humane and excellent patient care atmosphere( Stanowski, Simpson & White, 2015). I believe that patients should be treated with courtesy, respect and there should be a consumer involvement in a patient care plan. Today’s healthcare system is focused on patient’s satisfaction and health care outcome so I agree that reimbursement in the future is proofed to be based on the customer satisfaction. “Since 2012, hospitals have been rewarded and penalized based on the customer satisfaction”(Stanowski, Simpson & White, 2015), which is an excellent way to provide an optimal level of patient satisfaction. According to the research, there is a positive relationship between the pay for performance and patient satisfaction (Stanowski, Simpson & White, 2015).
Under payment, an ideal healthcare system will have the challenge of delivering higher quality for lower costs. The system’s payment reform will involve a transition from fee-for-service to global from systems that are value-based important for the achievement of the overall healthcare goals. An ideal healthcare payment system will give a great deal of support to value-driven system of healthcare delivery (Kent, 2013). The fee-for-service payment system will be of great importance to the healthcare system as it will help control the costs of health care.
