Information should always be recorded clearly and accurately and should be signed and dated by the first aider. -Details of the injured or sick child (name, date of birth, main contact details)
- Details of the accident or incident (date, time, where it happened)

- Advice of further treatment recommended (e.g. hospital treatment)
- Information parents and carers (when and how parents have been contacted)
- Signature of the first aider, the date and time.
