CFT/ Treatment team meeting was held at the LifeTies program located in Ewing, NJ for Dwayne Arroyo (youth). In attendance were Jasmine Alexander (CM), Stephanie Mansfield (Residential therapist), Vivian Harmon (Program Director), Madeline Lloyd (CCS) and youth. My life is - hard working. I was born may 16 2003 i have a dad, a stepmom, 1 brother, 1 sister and a pug.I live in Elma we moved to the next house over before i was born my dad drives truck.I used to go to elma.

My name is Alice Vo. My interests or hobbies include basketball and video games.My best quality is that I’m very friendly. The thing I value most in life is family. I value my family because it is always there for me when I need it. Advice I would give another student about life is to treat people the way you want to be treated. My educational goal is to get good grades.

When I get home, I fix a snack, watch TV and complete my homework. Afterwards I fix dinner then relax with my family. Later, I bring my dog in from outside and take a shower. I usually complete all of my homework by five thirty, except for when I study. When I watch tv, i watch it for about four to five hours except on Mondays and Tuesdays, when I watch wrestling. Without a plan, life can be messy and confusing. I think it is important to have goals in life. I have three main steps to my life’s plan.First, I want to be a good soccer player.I want to be good on all soccer positions. I hope to work at least three hours a week maintaining my foot skills and agility. I plan to run a couple miles at least once every two days to keep myself in shape as well as work out in the gym three times a week. I will also be hanging out with my friends and having a fun time. For high school next year, I intend to continue to play soccer and achieve a high gpa. To do this, I will have to put in even more work on my academics and be committed to putting in effort on the soccer field. I would like the authorization to implement a Life Enrichment Team that would to add joy, purpose and meaning to the lives of residents/patients. The Life Enrichment Team would offer activities and programs that reflect the talents, interests, and personalities of the residents. The Team would be in charge of planning and implementing musical events, reading time, arts and crafts, games, cultural celebrations, and birthday parties.

Everyone has weakness and strengths in the seven life management skills. Every year people have been doing decisions or skills that may affect them tremendously or not at all. Usually, people don’t recognize situation or list the possibilities which will affect them in every way. Others don’t even understand what life management skills are. They are abilities that you have to learn as you grow up. Life management skills can be many different things. Some examples are decision making, organizations and many more. These don’t have to be my strengths but can also be my weakness, that I will work on.

Life is made up of duties, and responsibilities. One must factor in "me" time is to stay healthy. I faces many day-to-day responsibilities that I must complete or remember for work, and family life. I try my best to put my family first. There are daily tasks of keeping in shape and cleanliness. This would include my daily tasks which are laundry, washing dishes, sweeping, and cooking. "There also must do tasks as well, like paying bills, making sure the kids homework is completed and ready for bed.

Moreover, my life revolves around my current home. In my recent home, my mother was responsible for the cooking and cleaning and my father was responsible for the grocery shopping and paying

My life has always been normal. My usual week goes like this. On Sunday I get up bright and early and go to church. My whole family gets up at the same time then eats breakfast and talks about our plans for the week.
