During MP’s comprehensive assessment, the five-step evidence-based model was followed to ensure an inviting, non-threatening atmosphere as the foundation of a patient-centered interview process (Bush, 2016). Due to the impact that social determinants can have on one’s health and well-being, the necessity of evaluating MP’s determinants, as well as his understanding of the determinants was imperative. With open-ended, focused questions; active listening skills; and summarization, the following social determinants were assessed: economic stability, education, social & community context, health & health care, and neighborhood & built environment (U.S. Department, 2018). The assessment revealed that MP is a 26-year-old single male who lives with his mother in a safe, suburban neighborhood. MP has a high school diploma. He is not currently employed, nor is he seeking higher education opportunities at this time. In today’s society, there is still a great struggle with health care disparities and many lives are affected by the lack of this fundamental program in our society. There are millions of people who die each year because they are unable to afford quality healthcare. The debate still continues about healthcare inequalities, what causes this disparity and who are affected by it. Health care is more of a necessity rather than a luxury and even though skeptics may argue to the latter, it only underlines the importance of the need for the wellbeing and care of individuals. There are several factors that could contribute to the lack of health care in the United States which ranges from but not limited to race, gender, socio- economic status, and lack of insurance coverage. The truth is there is a great disproportion between who can really afford quality healthcare as appose to individuals who have it. One would imagine that an employed individual would easily afford quality healthcare but we could be no further from the truth, since one’s economic status is an essential determinant to its affordability.
Though the American healthcare system has made big steps towards providing affordable healthcare for everyone, there remains a growing population of people who fall through the system’s cracks. These people are the medically underserved. They are typically the victims of unfortunate life circumstances that has left them without health insurance, or with insurance that provides inadequate coverage. The underserved also includes those who have trouble accessing healthcare for any reason. Anyone with illnesses or disabilities that require assistance beyond their coverage, or people who live in remote areas where healthcare services are sparse fall under this title. The term also does not exclude those who have sufficient insurance and resources, but struggle to understand and navigate our increasingly complex system of healthcare.
According to Stevens (2013), the call to develop and implement evidence-based practice (EBP) within all healthcare disciplines is fueled by legislative demands for improvement in standard medical metrics such as mortality and morbidity. However, increasing demands by the public for evidence related to the metrics and outcomes of such concepts as quality of life illustrate what may be more important to the client (Stevens, 2013). The whole world has issues with healthcare and how to provide care for all their citizens. The United States is currently in the process of making changes to its healthcare system. There are currently multiple types of health insurance in the United States. Today, I will address the health care insurance I have, its products, source of my insurance, my out-of-pocket expenses, the level of coverage I receive with my plan, the major limitations to my coverage, and the process of receiving needed care, needed care in my plan including exams, how to get to a specialist if needed and the process for non-emergency care.
The community identified its priority health needs by doing Community Health Status Assessment(CHSA). The aim of this was to obtain feedback from residents regarding their health status, access to healthcare and their perception of community services. The study included residents of Atlantic County and surrounding areas in Camden and Cape May counties that are serviced by Atlantic County hospitals in2002. The first research component was a secondary data collection. This secondary data profile contained health and wellness statistics collected from New Jersey, the Department of Health and Senior Services NJDHSS, CDC, and U.S. Census Bureau data. In our research, we have compared three very different tools for ranking evidence based practice methods. Each of these tools could be effectively utilized in any healthcare facility or healthcare organization.
According to the QSEN Institute (2014), evidence-based practice is knowing how to identify gaps in existing evidence-based guidelines and finding ways to fix these disparities. Because the prison facility does not currently educate directly on the importance of proper and frequent hand washing, a lack in this evidence-based practice of hand washing education was identified and selected for implementation.

I have been working in community based services for three years now and one of the topics that I thought would be interesting to research would be the efficiency of evidence based practices within a community setting. I have myself been trained in several evidenced based practices which I have implemented with families. However, it has been a common experience among my colleges that many of these practices do not fit well within CBHI services. I would like to explore the issue and propose date to my field placement on how to better implement these evidenced based mental health practices within the community. I realized that this is a broad topic but I will work on refining this idea as I believe it will be valuable for my clients.
